In blunt abdominal trauma in childhood, sonography should be the primary method for diagnostic imaging. This simple and non-invasive investigation can be readily performed even in cases with severe traumatic organ lesions. In cases not requiring immediate surgery the method can be used for monitoring and follow-up. Other diagnostic procedures such as IVP, CT, abdominal plain x-ray, angiography, scintigraphy do not become superfluous but should be used according to the clinical and sonographic findings.

Isolated injuries of the small bowel mesentery or mesocolon with subsequent bowel infarction due to blunt abdominal trauma are rare. Two cases are described: 1 involving the mesentery to the terminal ileum and 1 involving the transverse mesocolon and middle colic artery, both with bowel infarction. The modes of clinical presentation and management of patients with injuries to the mesentery, mesocolon and mesenteric vessels following blunt trauma are described.

This case demonstrated the feasibility of robotic-assisted exploratory laparoscopy in a hemodynamically stable trauma patient and abdominal wall repair with a favorable outcome. The patient presented with a stab wound at the left middle posterior flank. A computer tomography scan of the abdomen and pelvis demonstrated penetrating soft tissue injury to the left lateral abdominal wall with herniation of the omentum. A robotic-assisted laparoscopic approach was implemented to evaluate for visceral injury and to repair the abdominal wall. Diagnostic laparoscopy ruled out visceral and diaphragmatic

Ectopic kidney is a relatively uncommon anatomic variation that is usually detected incidentally in patients undergoing imaging for an unrelated reason. Most cases are asymptomatic and are often revealed by a complication; however, ectopic kidney is generally associated with higher risk of traumatic injury, urinary tract infection, renal calculi and other urologic conditions. We report the case of a 65-year-old male patient with a post-traumatic renal laceration on a previously undiagnosed ectopic pelvic kidney, with successful conservative treatment.

The authors report surgical treatment of a patient with a penetrating abdominal trauma followed by damage to inferior vena cava, liver, duodenum and pancreas, massive bleeding and large retroperitoneal hematoma. Suturing of inferior vena cava defect, liver and pancreas, Billroth II gastric resection, drainage of retroperitoneal space and abdominal cavity were carried out. Postoperative period was complicated by pneumonia and seroma in lesser sac that required percutaneous puncture. Antibiotic therapy was used postoperatively. The patient was discharged in 25 days after surgery.

A 60-year-old male was admitted to our major level 1 trauma centre following a fall from the fourth storey of a car park and landing initially on his feet on concrete. The primary survey was unremarkable apart from abdominal pain and localised peritonism in the right upper quadrant and lower lumbar midline pain. The secondary survey revealed bilateral complex calcaneal fractures, multiple vertebral fractures and sternal fracture. A trauma CT scan showed pericholecystic fluid and described by the radiology team either as cholecystitis picture or possible disruption of the gallbladder wall. Base

Blunt abdominal aortic injuries are extremely rare, diagnosed in less than 0.05% of all trauma admissions. Aortic injury caused by a seat belt during a car accident is often referred as "seat-belt aorta". We present a case of an 18-year-old woman, restrained back passenger involved in a vehicular collision, sustaining vertebral column and multiple rib fractures, mesenterium and colonic injury, and infrarenal aortic contusion with localized dissection and partial thrombosis.

A 19-year-old woman presented to our emergency department with crampy abdominal pain and per rectal bleeding 2 weeks after falling from a horse. She had been taking regular non-steroidal anti-inflammatory drugs (NSAIDs) for analgaesia. On arrival, she was tachycardic and tachypnoeic, with a lactate of 7.3 mmol/L. 'FAST' ultrasonography was unremarkable and CT scan showed thickened wall of the transverse colon. She underwent flexible sigmoidoscopy, which demonstrated "patchy inflammation and an isolated area of severe deep ulceration with nodularity and oedema". A presumptive diagnosis of "Infl

We have elucidated certain consistent patterns of the development of hepatic lesions associated with blunt abdominal trauma including primary and secondary local and distant ruptures of the liver of different localization, e.g. in the direction of the injurious force (central and anti-shock) and apart from it (peripheral). A kick in the stomach causes local (primary and secondary) or distant ruptures of the liver. A strong impact gives rise to local secondary and distant ruptures. Compression of the body with massive objects is associated with local primary ruptures whereas falling down on the

BACKGROUND: Abdominal injury has been shown to be an independent risk factor for pulmonary complications in patients with extremity injuries. We propose to characterize orthopedic patients with severe abdominal trauma. We hypothesize that operative fractures of the thoracolumbar spine, pelvis, acetabulum, or femur increase systemic complications in patients with blunt abdominal injury.
METHODS: A retrospective review of patients presenting to a Level I trauma center with abdominal injury between 2000 and 2006 was performed. Adult patients between the ages of 18 years and 65 years with high-en

CONTEXT: There is limited experience with pancreatic endotherapy in patients with pancreatic injury due to trauma.
OBJECTIVE: To retrospectively evaluate our experience of endoscopic management of pancreatic trauma.
PATIENTS: Eleven patients (10 males and 1 female; mean age: 21.8±11.9 years) with pancreatic trauma.
INTERVENTION: Endoscopic therapy. Patients with pseudocyst and a gastroduodenal bulge were treated with endoscopic transmural drainage. Pseudocysts without bulge or patients with external pancreatic fistula were treated with transpapillary drainage.
RESULTS: Seven patients (6 ma

Timely identification of specific injuries in a polytrauma case is of paramount importance in order to reduce morbidity and mortality. Unfortunately, some of these injuries are subtle and can be missed on initial primary and secondary assessments. In this paper, we report one such injury in a case of a 16-year old motorcyclist who complained of abdominal pain over the right lumbar region after the motorcycle handlebar hit his abdomen. Although initial assessment was uneventful, he was subsequently diagnosed to have a traumatic abdominal wall herniation on abdominal computed tomography after mo

INTRODUCTION: Abdominal organ injury in a primary blast type is always challenging for diagnosis. Air containing abdominal viscera is most vulnerable to effects of primary blast injury. In any patient exposed to a primary blast wave who presents with an acute abdomen, an abdominal organ injury is to be kept in a clinical suspicion.
AIM: Study various abdominal organ injuries occurring in a primary type of blast injury.
MATERIAL AND METHODS: All those who had exploratory laparotomy for abdominal organ injuries after a primary blast injury for a period of 10 years from January 1998 - January 2

A 30-year-old man was injured when the large motorcycle he was riding crashed into a power pole. Upon arrival at our institution, the patient complained of abdominal pain; blood pressure at admission was 160/70 mmHg, and the heart rate was 112 bpm. Abdominal ultrasonography showed a small collection of fluid in Morrison's pouch. A chest X-ray film showed a right rib fracture. Multidetector computed tomography (MD-CT) revealed pleural effusion, a hepatic lesion, and a dissection of the abdominal aorta distal to the renal artery. Because of the complicated intraluminal injuries, the insertion of

Although pseudoaneurysms after penetrating extremity trauma are well described, we describe an unusual case of residual occult aortic injury after an initial attempt at repair that was recognized on postoperative imaging. Reoperation with primary resection and end-to-end repair was accomplished successfully. Because this entity is so unusual, we review strategies to avoid and recognize its occurrence. Early imaging allows early identification of aortic pseudoaneurysms should they occur, and will preclude delayed manifestation of complications, including death. Our case illustrates the utility

PURPOSE: To evaluate the extent of liver injury after the onset of sepsis and abdominal compartment syndrome (ACS) in rats.
METHODS: We divided 60 rats into four groups of 15. Group 1 was the sham group. In group 2, sepsis was induced by cecal puncture and ligation; in group 3, ACS was created by placing a catheter in the abdominal cavity; and in group 4, both sepsis and ACS were induced simultaneously. Liver sections stained with hematoxylin-eosin were assessed pathologically, and liver injury was defined by the following five pathological patterns: spotty necrosis, capsular inflammation, po

Presented in this work is a rare injury of a blunt abdominal trauma in a child. Besides a partial rupture of the kidney and a retro-/intraperitoneal haematoma, a further injury occurred from the accident: an initially clinically indetectable tear of the A. iliaca communis which was found intraoperatively and with systematic CT analysis. Traumatic blood vessel lesions of the abdominal aorta and in particular the iliac blood vessels are very rare in children. By such violent impact injuries, it is therefore vital to perform a clinical examination of the foot pulse, systematic analysis of radiolo
