Pityriasis rubra pilaris (PRP) is an inflammatory condition of uncertain etiology. We present a case of PRP that demonstrates acantholysis on histopathologic examination, which is a characteristic feature that increasingly is recognized to aid in the diagnosis. We also review the clinical presentations and treatment options for this condition.

The case of a 9-year-old girl with a history of blistering lesions restricted to the right side of her body is described. Several family members are also affected in a symmetrical fashion. Histological examination showed acantholysis similar to a 'dilapidated brick wall', corresponding to a rare presentation of mosaicism in Hailey-Hailey disease.

Pemphigus vulgaris (PV) blistering occurs as a result of the disruption of intercellular contacts among keratinocytes, or acantholysis. The hallmark of PV acantholysis in vitro is considered to be the retraction of keratin intermediate filaments (KIF) onto the nucleus, which parallels with loss of cell-cell adhesion and rounding up of keratinocytes. We present two middle-aged patients with pruritic, crusted scalp erosions. Skin biopsy showed epidermal acantholysis with IgG and C3 intercellular deposits on direct immunofluorescence, leading to the diagnosis of localized pemphigus vulgaris. Resolution of the lesions without relapse occurred after low doses of oral prednisone and intralesional triamcinolone acetonide.

Immunoglobulin A (IgA) pemphigus is a rare autoimmune blistering disease characterized by epidermal acantholysis and neutrophilic infiltrates, as well as intraepidermal IgA deposits. We report the course of benign familial pemphigus (Hailey-Hailey disease) in a 45-year-old woman. While this condition was in remission the patient started to suffer from pityriasis rosea. A biopsy of this coexisting dermatosis revealed the typical features of Hailey-Hailey disease (suprabasal acantholysis) and of pityriasis rosea within the same lesion.

In the skin lesion of a patient with pemphigus vegetans, Charcot-Leyden crystals were observed in the eosinophilic abscess cavities of the epidermis. The finding is documented for the first time. In addition, some of the intraepidermal eosinophilic abscess cavities have been shown to be due to accumulation of eosinophils in dilated acrosyringia, but their formation from acantholysis and spongiosis can not be entirely excluded.

Parenteral passive transfer of human pemphigus vulgaris IgG (PV IgG) into neonatal mice reproduces the cutaneous disease. We used this model to study the role of complement in the development of acantholysis in three steps. IgA pemphigus is a rare intraepidermal autoimmune disease characterized by the presence of intercellular IgA deposits, intraepidermal acantholysis with infiltration of neutrophils, and circulating IgA autoantibodies against keratinocyte cell surface components.
