PURPOSE: Acetazolamide and CO2 are cerebral vasodilators whose specific effects in various brain regions have not been carefully defined. We investigated the effects of these agents in both larger cerebral and smaller, retrobulbar arteries, to compare their general cerebral vasodilatory influence with their specific ocular vascular effects.

We studied the effect of the carbonic anhydrase inhibitor acetazolamide on 24 patients with essential tremor by patient self-evaluation of functional disability, rating of motor task function, and clinical rating of tremor severity. Acetazolamide significantly reduced tremor severity, but there was no statistically significant change in patient self-assessment of function or motor task rating. Although side effects were common, over half the patients elected to remain on the drug.

Metabolic alkalosis is a common acid-base disturbance in critically ill patients. In many patients correction of fluid and electrolyte status does not fully correct the metabolic derangement. In this study we examined the effect of 500 mg of intravenous acetazolamide, after correcting for fluid and electrolyte abnormalities, on the acid-base status of 30 ventilated patients. The injection of 10 mg of acetazolamide into fertile chicken eggs on the 16th or 17th day of incubation resulted in a decreased accumulation of calcium by the developing embryo. A dose of 1 mg of acetazolamide did not affect embryonic calcium content. Although the eggshell serves as the primary source of calcium for the embryo, acetazolamide did not influence eggshell mobilization as measured by the release of 45Ca from labeled eggshells. Acetazolamide did decrease bone formation resulting in a statistically significant reduction in tibia dry weight and calcium content.

Five members in three generations of a family were affected by an illness that had many clinical features of the hypokalemic form of periodic paralysis (HPP). The present study demonstrated that acetazolamide (100 and 200 mg/kg, s.c.) induced severe gastric hemorrhagic ulceration in rats. The ulceration was aggravated by oral administration of HCl, but was inhibited by NaHCO3. Hepatocellular carcinoma (HCC) is the most common type of primary liver malignancy. Intra-arterial therapies such as drug-eluting bead transarterial chemoembolization (DEB-TACE) can be effective forms of locoregional treatment for HCC. Solid liver tumors such as HCC promote a biochemical tumor microenvironment (TME) that allows tumor recurrence. The TME creates an environmental acidic pH, which induces chemotherapy resistance and immunosuppression.
