A 6-year-old boy presented with gynaecomastia. There was no clinical or biochemical evidence of excessive androgenic or glucorticoid activity, but urinary oestrogen levels were raised. An adrenocortical adenoma, demonstrated by x-ray, was surgically removed. Oestrogen levels fell immediately. 3 years later the boy shows complete regression of the gynaecomastia and no signs of recurrence.

OBJECTIVE: Adrenal cortical carcinoma (ACC) is a rare and aggressive endocrine malignancy. Clinical symptoms are mainly related to excess hormone secretion. Hypercortisolism and virilisation are among the most common presentations.
METHODS: We report a case series of five patients with ACC, three of which presented as adrenal incidentalomas. Additionally, a literature review on current diagnosis and management of ACC was performed.
RESULTS: ACCs are often incidentally detected because of the liberal use of medical imaging. Management of ACC remains challenging, and the poor prognosis makes e

PURPOSE OF REVIEW: Over the last 20 years, the genetic landscape of adrenal tumours has been transformed by the identification of multiple susceptibility genes for the various tumour types. We review these recent developments here, and describe current recommendations for genetic testing in patients with tumours of the adrenal medulla and extra-adrenal paraganglia or the adrenal cortex.
RECENT FINDINGS: Phaeochromocytomas (adrenal medulla tumours) and paragangliomas, aldosterone-producing adenomas, primary macronodular adrenal hyperplasia, primary pigmented nodular adrenocortical disease and

The pathogenetic mechanisms underlying the onset of adrenocortical tumors (ACTs) are still largely unknown. Recently, more attention has been paid to the role of insulin and insulin-like growth factor (IGF) system on general tumor development and progression. Increased levels of insulin, IGF-1 and IGF-2 are associated with tumor cell growth and increased risk of cancer promotion and progression in patients with type 2 diabetes. Insulin resistance and compensatory hyperinsulinemia may play a role in adrenal tumor growth through the activation of insulin and IGF-1 receptors. Interestingly, appar

According to the current view a semiautonomously ACTH-producing pituitary microadenoma is the true cause of pituitary-dependent Cushing's syndrome in most instances. Only exceptionally does the disease seem to be caused by a functional pituitary or hypothalamic disturbance of cortisol regulation. A newly discovered rare etiology is ectopic production of CRF. Cushing's syndrome is still most reliably diagnosed by abnormal adrenocortical function tests based on corticosteroid determinations. However, determination of plasma ACTH concentrations and computer-assisted tomography of the pituitary or

Adrenocortical adenoma is a benign neoplasm derived from cells of the adrenal cortex. The myxoid variant of this tumor is extremely rare. To our knowledge, only 23 cases of myxoid adrenocortical adenoma have been reported so far and 19 of them mentioned the pseudoglandular pattern. We reported a new case of 56-year-old Chinese female patient whose left adrenal gland was shown a neoplastic lesion by computed tomography (CT) and magnetic resonance (MR) imaging. Histopathological study showed that the mass was a myxoid adrenocortical adenoma with a pseudoglandular pattern. Then, we performed immu

Spironolactone bodies were observed in an adrenal cortical adenoma that was removed from a patient with primary hyperaldosteronism (Conn syndrome) treated preoperatively with spironolactone. The electron microscopical evaluation of this adrenal cortical adenoma shows origin of spironolactone bodies from whorls of endoplasmic reticulum in cells with the cytoplasmic features of those from the zona glomerulosa of the adrenal gland. There was no evidence that the bodies were derived from mitochondria, which confirms recent ultrastructural findings in patients treated with spironolactone. These bod

Adrenal mixed corticomedullary tumors (MCMTs) are rare lesions showing a mixture of two cell populations of cortical and medullary lineage. We describe an MCMT case presented in a 56-year-old woman with a history of arterial hypertension and high levels of aldosterone, accompanied by a review of the literature. The adrenalectomy specimen showed a well-circumscribed nodule of 30 mm in size, containing 60% of cells with a cortical phenotype (positive for α-inhibin and melan-A) and 40% of cells with a medullary phenotype (positive for chromogranin-A, GATA-3 and somatostatin receptor 2). There was

OBJECTIVE: Benign adrenocortical adenomas are frequently diagnosed on cross-sectional imaging performed for unrelated reasons. Up to 15% to 20% of adrenal nodules are bilateral, representing bilateral adenomas or primary bilateral macronodular adrenal hyperplasia.
METHODS: Mild autonomous cortisol secretion (MACS), diagnosed based on an abnormal dexamethasone suppression test, is seen in 19% to 44% of patients with adrenal adenomas. Distinguishing unilateral from bilateral MACS in patients with bilateral nodules is important to guide appropriate therapy and relies on imaging phenotype, and, i

The adrenal glands are paired endocrine organs that produce steroid hormones and catecholamines required for life. Adrenocortical carcinoma (ACC) is a rare and often fatal cancer of the peripheral domain of the gland, the adrenal cortex. Recent research in adrenal development, homeostasis, and disease have refined our understanding of the cellular and molecular programs controlling cortical growth and renewal, uncovering crucial clues into how physiologic programs are hijacked in early and late stages of malignant neoplasia. Alongside these studies, genome-wide approaches to examine adrenocort

Clinical features and the management of adrenal insufficiency are considered with reference to etiology, diagnosis, and treatment. Specifically considered are the management of patients with Addison's disease and metastases. Adrenal insufficiency may go unrecognized in the cancer patients because features such as nausea, vomiting, anorexia as well as orthostatic hypotension are not specific and may be mistakenly attributed to advancing malignancy or side effects of therapy. Prompt recognition and treatment of adrenal insufficiency can avert potentially life-threatening situations.

PURPOSE OF REVIEW: Corticosteroids are commonly used in the treatment of neuro-oncology patients. Unfortunately, the benefits of steroids must be balanced against the potential development of complications. This review provides guidance for the usage of steroids in the management of neuro-oncology patients.
RECENT FINDINGS: Steroids should only be used as a temporizing measure in brain tumor patients to minimize symptoms secondary to mass effect from the tumor. For instance, because of the long half-life of corticosteroids, dosing should be limited to daily or twice per day, with the second d

Adrenocortical carcinoma (ACC) is a rare, aggressive malignancy with symptoms arising from mass effect or hormonal excess, particularly hypercortisolism. Accurate diagnosis requires comprehensive clinical, hormonal, radiologic, and histopathologic evaluation. Surgical resection is the only curative treatment. Treatment decisions are guided by risk stratification based on stage, Ki67 index, and resection margin status. Mitotane, the only Food and Drug Administration-approved drug for ACC, is combined with etoposide, doxorubicin, and cisplatin as the standard treatment for patients with recurren
