Association of dorsal inferior frontooccipital fasciculus fibers in the deep parietal lobe with both reading and writing processes: a brain mapping study.

Alexia and agraphia are disorders common to the left inferior parietal lobule, including the angular and supramarginal gyri. However, it is still unclear how these cortical regions interact with other cortical sites and what the most important white matter tracts are in relation to reading and writing processes. Here, the authors present the case of a patient who underwent an awake craniotomy for a left inferior parietal lobule glioma using direct cortical and subcortical electrostimulation. The use of subcortical stimulation allowed identification of the specific white matter tracts associate…

Praxis and writing of the left hand may be served by different callosal pathways.

A patient who had damage to the leftward outflow of the body of the corpus callosum experienced the development of agraphia, but not apraxia, of the left hand. In another case reported recently, a similar lesion produced the same intriguing dissociation. We suggest that the principal interhemispheric controls for praxis and for writing of the left hand operate at different levels of the corpus callosum.

[Gerstmann's syndrome in a 9 year old boy].

INTRODUCTION: Gerstmann's syndrome encompasses the tetrad of finger agnosia, agraphia, acalculia and right-left confusion and is associated with lesions of the left angular gyrus, situated at the confluence of the temporal, parietal and occipital lobes. The localizing value of this syndrome has been questioned because multiple mechanisms can account for each of the four components of the syndrome. This clinical association is infrequent in children and it is impossible to diagnose in early stages of life because of parietal lobes have a slow functional development during childhood. CLINICAL C…

Differential reorganization of fusiform gyrus in two types of alexia after stroke.

Lesions affecting the left fusiform gyrus (FG) commonly result in dyslexia and recovery largely depends on efficient reorganization of the reading network. We performed a follow-up fMRI study to elucidate the reorganization patterns of the FG according to the recovery of reading ability in two patients (MH with pure alexia and KM with alexia with agraphia) after stroke involving the left FG. Initially, MH was an effortful letter-by-letter (LBL) reader, and she improved to become a proficient LBL reader. The initial fMRI results showed scattered activation on occipital and ventral temporal cort…

Properties of graphic motor plans in the writing system.

The present study explores the extent to which properties of abstract graphemic representations are maintained at the post-graphemic level of graphic motor plans, where the sequences of writing strokes for producing the letters in a word are represented. On the basis of results from a stroke patient (NGN) who has a deficit affecting the activation of graphic motor plans, we explore the post-graphemic representation of 1) consonant/vowel status of letters; 2) geminate (double) letters, such as the BB in RABBIT; and 3) digraphs, such as the SH in SHIP. Through analyses of NGN's letter substituti

[Development of pure alexia due to a lesion in the left fusiform gyrus in a patient with hypertrophic pachymeningitis].

We present the case of a patient with pure alexia due to a restricted lesion in the left fusiform gyrus. A 59-year-old right-handed female, with a 7-year history of rheumatoid hypertrophic pachymeningitis suddenly developed reading and writing difficulties. Neuropsychological examinations revealed the presence of alexia for both Japanese kanji (morphograms) and kana (phonograms); mild agraphia predominantly for kanji; and word-finding difficulty. Brain magnetic resonance imaging (MRI) revealed a high signal intensity lesion in the left fusiform gyrus on fluid attenuated inversion recovery (FLA…

[Callosal infarction after bilateral occlusion of the internal carotid arteries with hemineglect syndrome and astasia-abasia].

We report a patient who had a callosal infarct, showed by Computer Tomography and Magnetic Resonance Imaging, without injury to the adjacent hemispheres. Angiography revealed an occlusion of both internal carotid arteries. Besides interhemispheric disconnection with unilateral left agraphia, left tactile anomia, left ideomotor apraxia and left ear auditivoverbal extinction, the patient showed signs of left hemineglect and astasia-abasia, both persistent.

Gerstmann's syndrome and unilateral optic ataxia in the emergency department.

A 75-year-old right-handed woman presented to the emergency department with simultanagnosia and right unilateral optic ataxia. Moreover, the patient had agraphia, acalculia, digital agnosia and right-left disorientation, consistent with complete Gerstmann's syndrome. This case highlights the concurrence of Gerstmann's syndrome and unilateral optic ataxia in the acute phase of a left middle cerebral artery stroke.

Pure Gerstmann's syndrome from a focal lesion.

It is controversial whether a focal lesion can specifically induce Gerstmann's syndrome (dyscalculia, left-right disorientation, finger agnosia, and agraphia). Also, Gerstmann's tetrad has been attributed to other cerebral symptoms, particularly aphasia. We examined a patient who had all four symptoms of Gerstmann's syndrome, without other symptoms or signs, and who had a discrete left parietal lesion.

Signs of interhemispheric disconnection in marchiafava-bignami disease.

We report three cases of Marchiafava-Bignami disease. The diagnosis was made on clinical grounds. Evidence of interhemispheric disconnection was found in all three cases. This included left hand anomia, left-sided apraxia, left-hand agraphia, and left-sided pseudoextinction to both visual and tactile stimuli. In two cases, there was a left-ear extinction on a dichotic listening test.

[Dual neural circuit model of reading and writing].

In the hypothetical neural circuit model of reading and writing that was initially proposed by Dejerine and subsequently confirmed by Geschwind, the left angular gyrus was considered as a unique center for processing letters. Japanese investigators, however, have repeatedly pointed out that this angular gyrus model cannot fully explain the disturbances observed in reading and writing Kanji letters in Japanese patients with various types of alexia with or without agraphia. In 1982, I proposed a dual neural circuit model of reading and writing Japanese on the basis of neuropsychological studies …

Transient left-sided alien hand with callosal and unilateral fronto-mesial damage: a case study.

A patient who showed a transient left alien hand after a subarachnoid haemorrhage, due to a ruptured aneurysm of the anterior communicating artery, is described. No left agraphia and left apraxia could be found. A CT-scan including coronal sections showed a right medial frontal hypodensity and different ischaemic lesions in the anterior part of the corpus callosum. The possible anatomic site of the lesion responsible for the permanent form of alien hand is discussed.

[Dyslexia of digits and numbers in multiple sclerosis].

In a 32-year-old man with remitting-progressive multiple sclerosis a right-side hemiparesis and alexia of digits of numbers developed during an exacerbation. While reading aloud the patient changed digits, left them out or transposed. No changes were observed in reading of letters, words and no agraphia was present. Alexia persisted for about one month. The location of alexia in the occipital lobe and its association pathways with the left parietal lobe are discussed.

Immunotherapy-associated dysgraphia as an early neurocognitive manifestation of immune effector cell-associated neurotoxicity syndrome: clinical characteristics, mechanistic insights, and assessment challenges.

BACKGROUND: Immune effector cell therapies, including chimeric antigen receptor T-cell (CAR-T) therapy and T-cell-engaging antibodies, are associated with immune effector cell-associated neurotoxicity syndrome (ICANS), a clinically recognized immune-mediated neurotoxicity. Among its neurological manifestations, writing impairment (dysgraphia), including paligraphia and other forms of writing disturbance, has emerged as a potentially sensitive clinical marker of higher-order cortical dysfunction. OBJECTIVE: This review synthesizes current clinical evidence on ICANS-associated dysgraphia, chara

[Dysgraphia].

Dysgraphia (or handwriting difficulties) is common. It is often associated with neurodevelopmental disorders. In France, it is confirmed by the BHK test. Associated anxiety must be taken into account, as many young people can fall into vicious circles of avoidance. Subtypes of dysgraphia are found according to underlying neurodevelopmental difficulties (motor, visuo-spatial or reading difficulties).

Infarction in the territory of the anterior cerebral artery.

Infarction in the anterior cerebral artery (ACA) territory is an uncommon cause of stroke. The clinical findings of ACA infarctions are not fully characterized but include contralateral hemiparesis, urinary incontinence, transcortical aphasia, agraphia, apraxia, and executive dysfunction. We report a patient with a large right ACA infarction, who in addition to previously reported findings also had a complete hemiplegia, profound sensory neglect, and micrographia.
