Challenges to managing air pollution.

There is never enough money, time, or resources to do all the things that need to be done--a statement that is true both for governments and individuals. For governments, this fact implies that (1) priorities need to be established; (2) goals are essential to be set to address these priorities, partly with an eye toward maximizing the net benefits to society; and (3) policies need to be implemented to reach those goals efficiently and effectively. In this article, major challenges of managing air pollution in each of these areas are examined.

Air quality management in Mexico.

Several significant program and policy measures have been implemented in Mexico over the past 15 yr to improve air quality. This article provides an overview of air quality management strategies in Mexico, including (1) policy initiatives such as vehicle use restrictions, air quality standards, vehicle emissions, and fuel quality standards, and (2) supporting programs including establishment of a national emission inventory, an air pollution episodes program, and the implementation of exposure and health effects studies. Trends in air pollution episodes and ambient air pollutant concentrations…

Children's response to air pollutants.

It is important to focus on children with respect to air pollution because (1) their lungs are not completely developed, (2) they can have greater exposures than adults, and (3) those exposures can deliver higher doses of different composition that may remain in the lung for greater duration. The undeveloped lung is more vulnerable to assault and less able to fully repair itself when injury disrupts morphogenesis. Children spend more time outside, where concentrations of combustion-generated air pollution are generally higher. Children have higher baseline ventilation rates and are more physic…

Traffic, air pollution, and health.

This paper discusses emerging information on exposure to air pollution from traffic and health and raises two key questions with regard to the impact of traffic on public health: 1) is there sufficient evidence to infer that traffic is causing adverse health effects, and; 2) what is the magnitude of the adverse impact of traffic on health? Topics addressed include characterization of exposure to traffic-related pollutants; the findings of epidemiological studies to date; and investigative approaches and the related challenges - including bias, model-based conclusions, and sample size issues--i…

Otic barotrauma from air travel.

Otic barotrauma occurring during air travel involves traumatic inflammation of the middle ear, caused by a pressure difference between the air in the middle ear and the external atmosphere, developing after ascent or more usually descent. The pressure difference occurs because of failure of the eustachian tube to equilibrate middle ear and atmospheric pressures. It is a common problem, presenting with ear fullness, otalgia and deafness. Severe cases may result in tympanic membrane perforation and even round window membrane rupture. Of three randomized controlled trials, one showed that oral ps…

Air pollution and children's health.

Children's exposure to air pollution is a special concern because their immune system and lungs are not fully developed when exposure begins, raising the possibility of different responses than seen in adults. In addition, children spend more time outside, where the concentrations of pollution from traffic, powerplants, and other combustion sources are generally higher. Although air pollution has long been thought to exacerbate minor acute illnesses, recent studies have suggested that air pollution, particularly traffic-related pollution, is associated with infant mortality and the development…

Credible investigation of air accidents.

Within the United Kingdom the Air Accidents Investigation Branch (AAIB) has been used as a model for the other transport modes accident investigation bodies. Government Ministers considered that the AAIB's approach had established the trust of the public and the aviation industry in its ability to conduct independent and objective investigations. The paper will examine the factors that are involved in establishing this trust. They include: the investigation framework; the actual and perceived independence of the accident investigating body; the aviation industry's safety culture; the qualities…

Air travel-associated venous thromboembolism.

Long-distance air travel is increasing and cases of venous thromboembolism (VTE) following air travel have attracted both considerable public attention and legal claims against airlines. VTE is a common disorder worldwide with a notably high incidence in older individuals. Many biochemical factors that lead to, or accentuate, thrombus formation are associated with increased risk of VTE. These factors include thrombophilia, activated protein C resistance and factor V Leiden, prothrombin gene mutation, antiphospholipid antibodies, protein S and protein C deficiencies, and methylene tetrahydrofol…

Ambient air pollution and health.

The adverse health effects of air pollution became widely acknowledged after severe pollution episodes occurred in Europe and North America before the 1960s. In these areas, pollutant levels have decreased. During the last 15 years, however, consistent results, mainly from epidemiological studies, have provided evidence that current air pollutant levels have been associated with adverse long- and short-term health effects, including an increase in mortality. These effects have been better studied for ambient particle concentrations but there is also substantial evidence concerning gaseous poll…

Health issues of air travel.

Every day in the United States the airline industry boards over 1.7 million passengers for a total of 600 million passengers per year. As these passengers enter the cabin of their aircraft few are aware of the artificial environment that will protect them from the hazards of flight. Passengers are exposed to reduced atmospheric pressure, reduced available oxygen, noise, vibration, and are subject to below zero temperatures that are only a quarter inch away-the thickness of the aircraft's skin. Over the past decade there have been both technical and lay articles written on the perception of poo…

[Air trapping revealing endobronchial tuberculosis].

We report a case of endobronchial tuberculosis located in the main left bronchus suspected on a CT scan and assessed by fiberoptic bronchoscopy. This localisation is not unusual, and is often associated with a normal chest radiography. Air trapping present on high resolution CT scan led to focus the examination on proximal airways and to suspect their involvement. This observation underlines the value of expiratory CT scan in demonstrating proximal stenosis of main bronchi.

Air swallowing in Rett syndrome.

The possible causes of excessive swallowing of air leading to bloating, which is common in Rett syndrome (RS), were investigated during feeding and at rest. Seven individuals with RS aged between 4 and 33 years (three with air bloat) underwent feeding videoflouroscopy and concurrent respiration monitoring. The results were compared with a randomly selected group of 11 individuals, aged between 2 and 16 years, with quadriplegic cerebral palsy and feeding problems, some of whom had mild air bloat. All individuals from both groups had isolated pharyngeal swallows and several mouth breathed; this …

Air bags and ocular injuries.

PURPOSE: This investigation retrospectively examined ocular injuries associated with air bag deployment to gain a better appreciation of potential risk factors in motor vehicle accidents. National statistics regarding the efficacy of air bags were reviewed. METHODS: Review of the literature from 1991 to 1998 identified 44 articles describing 97 patients with air-bag-induced ocular injuries. Variables extracted from each case were age, sex, height, position in the car, eye wear, vehicle impact speed, visual acuity, and specific ocular injuries. RESULTS: Corneal abrasions occurred in 49% of oc…

Cerebral air embolism: acute imaging.

Iatrogenic cerebral air embolism secondary to right subclavian vein recatheterization was imaged acutely by computed tomography (CT) and magnetic resonance imaging (MRI). However, CT showed intravascular air with misleadingly high attenuation values sampled to a minimum of -39 HU. Diffusion-weighted imaging, not previously reported in this setting, clearly showed hyperintense acute infarctions in corresponding vascular territories 8.5 hours postprocedure (less than 6 hours after referable symptomatology noted), whereas T2-weighted fluid-attenuated inversion recovery and turbo gradient spin ech…

Lung cancer and air pollution.

Epidemiologic studies over the last 40 years suggest rather consistently that general ambient air pollution, chiefly due to the incomplete combustion of fossil fuels, may be responsible for increased rates of lung cancer. This evidence derives from studies of lung cancer trends, studies of occupational groups, comparisons of urban and rural populations, and case-control and cohort studies using diverse exposure metrics. Recent prospective cohort studies observed 30 to 50% increases in lung cancer rates associated with exposure to respirable particles. While these data reflect the effects of ex…

Air embolism during awake craniotomy.

A case of venous air embolism during awake craniotomy is described. Chest pain and electrocardiographic changes were the major presenting features. Capnography detected a decrease in end-tidal carbon dioxide and demonstrates its use as an aid in the diagnosis of air embolism in the awake patient. Following this episode persistent hypoxaemia developed secondary to acute pulmonary oedema and a pleural effusion.

[Air gun injuries in children].

The authors describe four cases of air rifle child injuries. One was a potentially lethal injury, a penetrating wound of the transverse colon and injury to the stomach. During urgent laparotomy the perforations of the gastrointestinal tract were sutured. In the remaining three children the injuries were less serious and affected soft tissues. In the discussion the authors summarize data from the literature concerning injuries to the head, neck, chest, abdomen and extremities, incl. lethal injuries. In instances when a penetrating abdominal injury cannot be ruled out, the authors emphasize the …
