Leber's hereditary optic neuropathy--case report and literature review.

CONTEXT: Leber's hereditary optic neuropathy is an important cause of progressive painless visual loss among young male patients. OBJECTIVE: To report on a case of a young patient with a clinical and neurophysiological condition suggestive of Leber's hereditary optic neuropathy, confirmed by genetic testing. CASE REPORT: We describe a 17-year-old male with progressive bilateral visual loss. Two maternal uncles had had similar patterns of visual loss. The patient had a history of smoking and alcohol abuse. Neuro-ophthalmological examination revealed visual acuity of 20/800 in both eyes, with

Peripheral neuropathy associated with acquired immunodeficiency syndrome. Prevalence and clinical features from a population-based survey.

We prospectively studied 40 hospitalized patients who had well-established diagnoses of acquired immunodeficiency syndrome. Patients with confounding risk factors for neuropathy were excluded; none of the study patients had known vitamin deficiency, alcoholism, or any metabolic, drug, or toxic factor. Clinical and electrophysiologic evidence of a distal symmetric polyneuropathy was found in 35% (13/37) of the patients. Symptoms and signs of neuropathy were usually mild, and painful dysesthesias were uncommon. Amplitude reduction of sural nerve action potentials distinguished all patients with

Case Report: Nutritional and Toxic Optic Neuropathy: A Diagnostic Dilemma.

SIGNIFICANCE: Nutritional and toxic optic neuropathies are rare disorders characterized by visual impairment due to optic nerve damage by a toxin, usually with coexisting nutritional deficiencies. Its pathophysiology is still unclear, and multiple mechanisms implicated act synergistically to bring about this condition. The decline in its incidence and its confusing clinical appearance make diagnosing nutritional and toxic optic neuropathies challenging. PURPOSE: This is an observational clinical case report of an atypical clinical case of a nutritional and toxic optic neuropathy with a subacu

Cognitive impairment and central motor conduction time in chronic alcoholics.

The non-invasive technique of transcranial magnetic stimulation (TMS) was used in 62 chronic alcoholics to assess the functional status of descending motor pathways. The main aims of this study were: to investigate asymptomatic upper motor neuron dysfunction in alcoholics as well as to assess its relationship with parameters reflecting the intensity of exposure to alcohol; and to evaluate a possible relationship between central motor conduction time (CMCT) prolongation and neuropsychological measures of alcohol-related brain damage. Compared to control subjects, chronic alcoholics exhibited a

Natural history of autonomic neuropathy in chronic liver disease.

To determine the natural history of autonomic neuropathy in chronic liver disease we used standard cardiovascular autonomic tests to evaluate prospectively 60 patients (33 male, 27 female) with initially well-preserved hepatic function. On initial testing, 27 patients (45%; median [range] age 56 [32-67] years) had vagal neuropathy. Autonomic dysfunction was equally common in patients with alcohol-related and nonalcoholic-related liver disease. The cumulative 4-year mortality rate in patients with vagal neuropathy was 30% compared with 6% in those with normal autonomic function. Multiple logist

Are the Protean Effects of Pentoxifylline in the Therapy of Diabetes and Its Complications Still Relevant?

Pentoxifylline (Px) has protean effects that can be utilized in the therapy of diabetes and its complications. There have been well-documented but often inconclusive improvements in peripheral arterial disease, foot ulcers, peripheral neuropathy, nephropathy, retinopathy, ischemic heart disease and cerebrovascular disease. In addition, non-alcoholic steatosis and steatohepatitis, which are closely associated with insulin resistance and type 2 diabetes, have been shown to improve with pentoxifylline. Surprisingly, pentoxifylline modestly improves insulin resistance through improvements in capil

Specific Micronutrient Intake Association with Diabetic Neuropathy Severity in Adults with Type 2 Diabetes.

BACKGROUND/OBJECTIVES: Diabetic neuropathy is a prevalent complication of type 2 diabetes mellitus (T2DM), but the contribution of dietary factors to neuropathy severity remains insufficiently characterized. This study investigated associations between dietary patterns, nutrient intake, and neuropathy severity in 300 adults with T2DM from Northeastern Romania. METHODS: Dietary intake was assessed using a validated food frequency questionnaire, and five dietary patterns were derived using principal component analysis. Neuropathy severity was analyzed as an ordinal outcome using logistic regres

[Comparison of electrophysiologic changes in persons with alcoholic polyneuropathy and polyneuropathy caused by carbon disulfide].

A group of 39 patients with alcoholic polyneuropathy (Alc) and another group of 33 patients with carbon disulphide polyneuropathy were studied by electrophysiological methods using an identical programme, and the results were compared. In both groups evidence was found of damage to the peripheral neuron of the type of axonal neuropathy with signs of axonal loss, with moderate slowing down of conduction velocity and with associated signs of regeneration. The dynamics of changes in the Alc group was greater. The cause of a slower rate of changes in the CS2 group seems to be related to a more gra

Disulfiram neuropathy: a neurofilamentous distal axonopathy.

Disulfiram is used to treat alcoholism and is known to cause peripheral neuropathy: few reports of biopsied human nerves have revealed axonal degeneration and loss of myelinated fibers. We studied a 22-year-old woman with severe sensorimotor neuropathy following treatment with disulfiram for 6 months. Histologic studies of the sural nerve revealed a neurofilamentous axonopathy with rare enlarged axons distended by neurofilaments. Disulfiram is converted enzymatically to carbon disulfide, which causes neurofilamentous distal axonopathy in animals. Similar changes in human nerve after disulfiram

Toxic optic neuropathy.

Toxic optic neuropathy is a complex, multifactorial disease potentially affecting individuals of all ages, races, places, and economic strata. Etiology includes nutritional, environmental, toxicologic, and genetic factors. Most cases of nutritional amblyopia are encountered in disadvantaged countries. However, toxic amblyopia related to drug treatment or alcohol abuse is also encountered in the Western world. Typically, toxic and nutritional optic neuropathy is progressive, with bilateral symmetrical painless visual loss causing central or cecocentral scotoma. There is no specific treatment fo

[Optic neuropathy induced by thinner sniffing].

A case of optic neuropathy induced by thinner sniffing is reported. A 17-year-old girl, who had been sniffing a lacquer thinner for three months, suffered acute blindness followed by optic atrophy. Brain computed tomography revealed symmetrical low attenuation areas in the bilateral putamen. An analysis of the thinner by gas chromatography showed that its major components in a vaporized state were methyl alcohol and metyhl acetate. The optic neuropathy was induced by these solvents. In the diagnosis of the intoxication of mixed organic solvents, the measurement of each solvent in its vapor pha

Case Report: Disulfiram-induced optic neuropathy.

BACKGROUND: Toxic optic neuropathy (TON) is characterized by bilateral, symmetric vision loss with damage to the papillomacular bundle, central or cecocentral scotomas, and impaired color vision. While TON has been well-documented with medications such as ethambutol and linezolid, disulfiram-induced optic neuropathy remains rare with limited case reports in the literature. CASE PRESENTATION: We report two cases of disulfiram-induced TON in patients with alcohol use disorder. Case 1 involved a 52-year-old man who developed painless, progressive bilateral vision loss after 9 months of disulfira

Thiamine Deficiency Masquerading As Guillain-Barré Syndrome.

Thiamine (vitamin B1) deficiency may be associated with serious neurological manifestations, including peripheral neuropathy, paralysis, confusion, paresthesia, pain, dysarthria, and nystagmus. The primary etiology of thiamine deficiency is consumption of a diet deficient in thiamine. In the United States, the most common cause of thiamine deficiency is alcohol use disorder due to the poor dietary intake that is often associated with this diagnosis. Thiamine deficiency also develops at an increased frequency in patients who have had gastric bypass surgery. Other medical conditions less commonl

The nutritional status of patients with an alcohol problem.

73 patients with an alcohol problem seen in a hospital medical unit over a two year period were examined specifically for signs suggestive of nutritional deficiency and of organ or system damage due to alcohol. A seven day dietary recall was carried out on most of them. Clinical signs of nutritional deficiency were relatively uncommon but nine patients had Wernicke-Korsakoff syndrome and ten had peripheral neuropathy. Thiamine was present in less than recommended amounts in 81.5% of the diets. Vitamin D and vitamin C were the next most common dietary deficiencies. 66.1% were deficient in energ

Tobacco-alcohol amblyopia: a diagnostic dilemma.

INTRODUCTION: Tobacco alcohol amblyopia is an outdated term for a rare condition characterized by visual impairment due to tobacco and alcohol abuse usually associated with nutritional deficiencies. The more accurate term used now is Nutritional Optic Neuropathy. The visual impairment generally presents as a centrocecal scotoma. Its pathophysiology is poorly understood but it is generally attributed to toxic effects of cyanide and B12 deficiency. DESIGN/SUBJECTS: 61 year old male presented with one month history of altered mental status and progressive, painless, bilateral vision loss in the

[Polyneuropathies--symptoms, causes and diagnostic procedure].

The diagnosis of a polyneuropathy (PNP) is particularly a clinically based diagnosis. The polyneuropathic syndrome consists of typical symptoms like paraesthesias, pain, cramps and fasciculations as well as characteristic signs like loss of tendon reflexes, weakness and wasting of muscles, sensory loss, cranial nerve palsy and alterations of the autonomic nervous system. The analysis of manifestation-types is very important for the differential diagnosis. The symmetrical sensory type of manifestation can be found in the beginning of almost all toxic neuropathies as well as in uremic and diabet
