Mortality and morbidity in suspected acute myocardial infarction in relation to ambulance transport.

In 681 patients admitted to the coronary care unit (CCU) at Sahlgrenska hospital between 1 May 1983 and 31 May 1984, due to suspected acute myocardial infarction (MI), the hospital mortality and morbidity were related to whether the patients were transferred to hospital by ambulance or not. In the ambulance group acute MI developed in 48% (during the first 3 days in hospital) compared with 41% in the non-ambulance group (P = 0.10). The overall mortality rate was 10.4% in the ambulance group versus 3.8% in the non-ambulance group (P = 0.001). Corresponding figures for MI patients were 19.3% ver

[Dopplerography of the venous return of the foot in the healthy subject. Preliminary to a study of ambulation].

The direction of venous return was monitored continuously by the Doppler method at 4 MHz, in order to study the effects of posture and certain kinesitherapeutic maneuvers on venous return in the foot of young, healthy and trained adults (kinesitherapy monitors, yoga assistants). It was noted that: the acceleration of venous return in the foot was expiratory in the recumbent position and inspiratory in the standing position; dorsiflexion (leg on foot or toes on heel) caused a more pronounced spurt phenomenon than plantar flexion; massage repeated at high frequency is less beneficial for venous

Coarctation hypertension is renovascular, modified by ambulation. Coarctation hypertension renovascular variant.

In coarctation, bipedal exercise induces a pressure triad: exaggerated systolic arm hypertension, unchanging leg pressure, and markedly increased systolic gradient. Constancy of leg pressure derives from the lower body sharing the poststenotic compartment with the kidneys. Exercise-induced poststenotic hypotension stimulates the juxtaglomerular apparatus (JGA) to raise renal pressure to pre-exercise levels. Ambulation during the greater part of each day stimulates the JGA repetitively. Thereby, this chronic Single-Kidney-Goldblatt model is modified by increased plasma renin, fluid volumes, and

The energy expenditure of spina bifida children during walking and wheelchair ambulation.

Energy expenditure was measured during walking and wheelchair ambulation among a group of 22 spina bifida children. Walking energy expenditure was generally higher than during wheelchair ambulation and significantly higher again than that expected for normals matched for weight. The energy expended during both types of locomotion related to the weight of the subjects and not the site of lesion. Physical apathy, excessive weight and increased energy expenditure tended to be connected. Data are presented which will be of use in assessing the physical effort involved in the two types of locomotio

The Macmillan-Scottish ambulance partnership: driving change in end-of-life care.

OBJECTIVES: Unnecessary hospital admissions for palliative emergencies strain resources and may not meet patients' end-of-life preferences. This study evaluates the impact of a 3-year partnership between Macmillan Cancer Support and the Scottish Ambulance Service (SAS), launched in 2022, to strengthen community-based palliative care and establish alternative referral pathways. METHODS: A retrospective comparative analysis of SAS call data linked to emergency department (ED) outcomes was conducted for 2022 and 2023. We assessed changes in key metrics: acute-care hospital admissions, ambulance

Digital Twins for Decision Making: Combining Ambulance and ED Facility Resources.

Emergency department (ED) walk-ins and ambulance arrivals are often modelled independently, overlooking their interdependence and the impact of fixed treatment space (TS) capacity. An established discrete event simulation algorithm has been tailored as the basis of a digital twin (DT). Driven by regular streams of incoming ED presentations, patient off-stretcher time (POST), ambulance bay and TS occupation across a 24-hour period contingent on patient flow and ED TS capacity was simulated. The simulation was calibrated using 19,570 ambulance records and data from a 250-bed hospital (2021-2022)

Frequent users of the ambulance service in a Swedish region: a retrospective cohort study.

BACKGROUND: This study aimed to describe the characteristics of frequent users of ambulance services and their corresponding ambulance missions. METHODS: A retrospective cohort study using register data from a Swedish region (approximately 308,000 inhabitants) was conducted. The dataset covered ambulance missions dispatched between 2019 and 2023; data from 2020-2022 were analyzed, with the extended timeframe (2019-2023) used to identify frequent users. Frequent users were defined as patients with four or more ambulance missions within a 365-day period, divided into moderate (patients with 4-1

Neighborhood Characteristics, Ambulation Ability, and Social Vulnerability of Individuals with Spina Bifida.

OBJECTIVE: This study examined the characteristics of neighborhoods where people with Spina Bifida (SB) live, driving distances, and the relationship of those factors to ambulatory ability. DESIGN: Data from patients attending an adult and pediatric SB clinic in the National Spina Bifida Patient Registry were matched to the 2022 CDC/ATSDR Social Vulnerability Index using patient zip codes. A multinomial logistic regression was used to predict ambulation ability, and Spearman's Rho correlations were used to assess the relationship between driving distance to clinic, nearest physical therapist

The Staff Observation Aggression Scale - Revised for Ambulance Services (SOAS-RA).

INTRODUCTION: Ambulance personnel frequently encounter aggression in dynamic and unpredictable environments. Despite growing awareness of workplace violence in healthcare, few validated tools exist for systematic documentation in ambulance services. OBJECTIVE: This study aimed to adapt and validate the Staff Observation Aggression Scale - Revised (SOAS-R) for use in ambulance services (SOAS-RA), and to examine the relationship between SOAS-RA severity scores and staff's subjective perceptions of incident severity using a Visual Analogue Scale (VAS). METHODS: Using a modified Delphi method, a

Characterizing Stair Ambulation Kinetics and the Effects of Dual Tasking in Parkinson's Disease.

Background: Stair ambulation is a complex motor task that presents a substantial fall risk for people with Parkinson's disease (PwPD) who often have postural instability and gait difficulty (PIGD) and experience unpredictable freezing of gait (FOG) episodes. While dual-task (DT) interference during level walking is well-documented, its impact on stair ambulation, an everyday, high-risk activity, remains poorly understood. Objective: The aim of this study was to quantify the impact of dual tasking on patterns of motor control during stair ambulation using kinetic data from The Stair Ambulation

AI-driven universal lower-limb exoskeleton system for community ambulation.

Exoskeletons offer promising solutions for improving human mobility, but a key challenge is ensuring the controller adapts to changing walking conditions. We present an artificial intelligence (AI)-driven universal exoskeleton system that dynamically switches assistance types between walking modes, modulates assistance levels corresponding to the ground slope, and delivers assistance timely based on the current gait phase in real-time. During treadmill validation, AI-based assistance reduced metabolic cost by 6.5% compared to 3.5% for conventional assistance. We expanded testing the controller
