Liquor made quicker: alcohol as a synthetic reagent for molecules in anesthesia.

Ethanol was an early anesthetic, and chemists transformed it into better ones. Hypnotic/anesthetic/analgesic molecules prepared from ethanol include barbiturates, benzocaine, chloral hydrate, chloroform, diethyl ether, ethyl chloride, ethylene, etomidate, meperidine, paraldehyde, phenacetin, procaine, tribromoethanol, and urethane. Ethanol was sometimes mixed deliberately with the other anesthetics, and John Snow's inhaled amylene came from the "fusel oil" fraction of rotgut whisky.

[Current diagnosis and therapy of malignant hyperthermia].

Malignant hyperthermia is an autosomal dominant disorder which results in a severe reaction to anesthetic agents in approximately 0.0005 to 0.5% of patients exposed to general anesthesia. Malignant hyperthermia is characterized by severe muscle rigidity, myoglobunuria, high fever, tachycardia, and arrhythmia precipitated by depolarizing muscle relaxants (succinylcholine) and inhalational anesthetic agents (halothane). When MH occurs, the anesthetic agents must be discontinued immediately and dantrolene should be administered.

Anesthesia for pediatric external beam radiation therapy.

BACKGROUND: For very young patients, anesthesia is often required for radiotherapy. This results in multiple exposures to anesthetic agents over a short period of time. We report a consecutive series of children anesthetized for external beam radiation therapy (EBRT). METHODS: Five hundred twelve children < or = 16 years old received EBRT from January 1983 to February 1996. Patient demographics, diagnosis, anesthesia techniques, monitoring, airway management, complications, and outcome were recorded for the patients requiring anesthesia. RESULTS: One hundred twenty-three of the 512 children

[Anesthetic management of a multiallergic patient scheduled for cholecystectomy].

A 64-year-old woman was scheduled for cholecystectomy. Her past history revealed that serious anaphylactic reactions including generalized flushing and urticaria, severe hypotension and unconsciousness which occurred after eating crab four years ago. Puncture and/or intradermal skin test and subsequent lymphocyte stimulation test to several drugs commonly used in perioperative period were performed prior to anesthesia. Positive reactions to intravenous anesthetics and muscle relaxants, and negative reactions to inhalational and local anesthetics were found. Famotidine and ketotifen fumarate we

Perioperative Anesthesia Management: The Role of MicroRNAs.

MicroRNA (miRNA) is a small RNA molecule that does not code for proteins, and organ- and disease-specific miRNAs are being investigated as diagnostic tools and therapeutic targets, particularly for cardiovascular disease and cancer. Much remains unknown about how anesthetics, other drugs, and perioperative management affect miRNAs, but miRNA-targeted drugs might eventually be used perioperatively. This review examines changes in miRNA expression related to anesthesia management. Sevoflurane results in gene expression patterns that differ by organ. The author investigated changes in miRNA expre

Snake Sedation and Anesthesia.

Snakes can be more challenging to anesthetize compared with other animals because of anatomic and physiologic differences, a wide range of patient sizes, and variable responses to anesthetic agents. Snakes have preferred optimal temperature zones, which, along with physiologic characteristics, such as the ability to shunt blood toward or away from the lungs, can have an impact on anesthesia. Injectable agents, including benzodiazepines, α2-agonists, opioids, propofol, and alfaxalone, as well as inhalant anesthetics can be used to anesthetize snakes. Pain management must be incorporated to the

Treatment of Super-Refractory Status Epilepticus: A Review.

PURPOSE: Super-refractory status epilepticus (SRSE) presents management challenges due to the absence of randomized controlled trials and a plethora of potential medical therapies. The literature on treatment options for SRSE reports variable success and quality of evidence. This review is a sequel to the 2020 American Epilepsy Society (AES) comprehensive review of the treatment of convulsive refractory status epilepticus (RSE). METHODS: We sought to determine the effectiveness of treatment options for SRSE. We performed a structured literature search (MEDLINE, Embase, CENTRAL, CINAHL) for st

[Sevoflurane in airway management using fibrobronchoscopy: apropos of 2 difficult orotracheal intubation cases].

We report two cases of difficult orotracheal intubation in which intubation with a fiberoptic bronchoscope was performed after induction with an inhalational anesthetic. Mild inhalational anesthesia with sevoflurane was started fairly rapidly, with no respiratory incidences. The tubes were then inserted through a fiberoptic bronchoscope, with no airway management problems.

A Case Report of an Incidental Ultrasound Finding in a Suspected Malignant Hyperthermia Patient.

Malignant hyperthermia (MH) is a rare but life-threatening genetic disorder of the skeletal muscles triggered by inhalation anesthetics or succinylcholine. A 49-year-old female developed symptoms of MH shortly after a lumbar surgery. Despite being insidious, MH was diagnosed based on the clinical grading scale. We incidentally discovered fine fasciculations in extremities while inserting an ultrasound-guided arterial catheter. On receiving dantrolene, her symptoms improved within 20 minutes; a subsequent ultrasound revealed no fasciculations. Although halothane contracture testing was not avai

A Comparative Study of Cell Specific Effects of Systemic and Volatile Anesthetics on Identified Motor Neurons and Interneurons of Lymnaea stagnalis (L.), Both in the Isolated Brain and in Single Cell Culture.

1. A comparative descriptive analysis of systemic (sodium pentobarbital, sodium thiopentone, ketamine) and volatile (halothane, isoflurane, enflurane) general anesthetics revealed important differences in the neuronal responses of identified motor neurons and interneurons in the isolated central nervous system (CNS) and cultured identified neurons in single cell culture of Lymnaea stagnalis (L.). 2. At high enough concentrations all anesthetics eventually caused cessation of spontaneous or evoked action potentials, but volatile anesthetics were much faster acting. Halothane at low concentratio

Environmental and clinical rationale for the use of desflurane: a review of the literature.

Desflurane is a fluorinated ether anesthetic with several clinical benefits, including resistance to defluorination (reducing nephrotoxicity), rapid emergence, cardioprotective properties, and faster recovery of airway reflexes than other volatile agents. However, its use is increasingly restricted because of environmental concerns, and the European Union has finalized regulations to phase out its use from January 2026, citing its global warming potential. Although its 100-year global warming potential exceeds that of comparable agents, the most appropriate emission metric remains debated. Rad

Increasing the reflection efficiency of the Sedaconda ACD-S by heating and cooling the anaesthetic reflector: a bench study using a test lung.

BACKGROUND: As volatile anaesthetic gases contribute to global warming, improving the efficiency of their delivery can reduce their environmental impact. This can be achieved by rebreathing from a circle system, but also by anaesthetic reflection with an open intensive care ventilator. We investigated whether the efficiency of such a reflection system could be increased by warming the reflector during inspiration and cooling it during expiration (thermocycling). METHODS: The Sedaconda-ACD-S (Sedana Medical, Danderyd, Sweden) was connected between an intensive care ventilator and a test lung.

Anaesthesia defined (gentlemen, this is no humbug).

Our charge was to define anaesthesia as produced by inhaled anaesthetics. A definition may be useful to an understanding of the anaesthetic state, and it may guide studies of the mechanisms by which anaesthesia is produced. All inhaled anaesthetics act on the central nervous system to produce two reversible conditions, immobility and amnesia, that define the anaesthetic state. No other reversible, clinically useful, conditions are essential to the definition. Some conditions are unmeasurable (unconsciousness), not present for all inhaled anaesthetics (relaxation), or are not present at anaesth

Xenon: anesthesia for the 21st century.

Xenon is a naturally occurring, gaseous element that comprises 0.000008% of air, or 0.05 parts per million. It was discovered by Ramsey and Travers in 1898. Xenon is found on the Periodic Table in group 0, which is the group commonly referred to as the noble or inert gases. It is obtained by fractionally distilling liquefied air. Xenon has been studied sporadically within the discipline of anesthesia as a replacement for nitrous oxide. Because it is a naturally occurring element, xenon is not a pollutant. It is not an occupationally hazardous gas. It is neither teratogenic nor fetotoxic, as is

Sevoflurane and thoracic epidural anesthesia for trans-sternal thymectomy in a child with juvenile myasthenia gravis.

Literature on anesthetic management of juvenile myasthenia gravis (JMG) for thymectomy is limited. Recently, use of inhalational agents and total intravenous anesthesia with propofol and remifentanyl has been reported. All these techniques individually or in combination have been tried to avoid the use of muscle relaxant. We report successful use of sevoflurane as sole anesthetic agent for intubation and in combination with thoracic epidural anesthesia for intraoperative anesthetic management in a 5-year-old child with JMG.

Anesthetic effects on mitochondrial ATP-sensitive K channel.

BACKGROUND: Volatile anesthetics show an ischemic preconditioning-like cardioprotective effect, whereas intravenous anesthetics have cardioprotective effects for ischemic-reperfusion injury. Although recent evidence suggests that mitochondrial adenosine triphosphate-regulated potassium (mitoK(ATP)) channels are important in cardiac preconditioning, the effect of anesthetics on mitoK(ATP) is unexplored. Therefore, the authors tested the hypothesis that anesthetics act on the mitoK(ATP) channel and mitochondrial flavoprotein oxidation. METHODS: Myocardial cells were isolated from adult guinea p

Effect of Anesthesia Gases on the Oxygen Reduction Reaction.

The oxygen reduction reaction (ORR) is of high importance, among others, because of its role in cellular respiration and in the operation of fuel cells. Recently, a possible relation between respiration and general anesthesia has been found. This work aims to explore whether anesthesia related gases affect the ORR. In ORR, oxygen which is in its triplet ground state is reduced to form products that are all in the singlet state. While this process is "in principle" forbidden because of spin conservation, it is known that if the electrons transferred in the ORR are spin-polarized, the reaction o

Choice of anesthetic alters the circulatory shock pattern as gauged by conscious rat endotoxemia.

A standardized rat endotoxin shock model was used to assess the differential effects of four commonly used anesthetics upon hemodynamics before and during endotoxin shock. Forty-nine male Sprague-Dawley rats weighing 307 +/- 4 g were divided into five groups:freely-moving conscious, enflurane (2%), isoflurane (1.4%), pentobarbital (6 mg.kg-1.h-1 i.v.), and ketamine (45 mg.kg-1.h-1 i.v.). Anesthetic doses were chosen as 1.0 MAC equivalent. Anesthetized rats were intubated and ventilated with oxygen. The right carotid artery was cannulated with a thermocouple-catheter for aortic blood pressure,
