Cardiac arrest in the delivery room after spinal anesthesia for cesarean section: a case report and review of literature.

Cardiac arrest in pregnancy is a rare event due to different cause. When it occurs after spinal anesthesia a cause that can explain this event is the Bezold-Jarish Reflex (BJR). A cardiac arrest occurs in a pregnant women after spinal anesthesia admistered for urgent caesarean section. During this event perimortem caesarean delivery (PMCD) is the rapid surgical way that can improve maternal and fetal outcomes. In this situation, it is very important to have a multidisciplinary team of midwives, obstetricians, anesthetists, neonatologists, intensivists that is able to perform perimortem caesare…

Perioperative management of oral anticoagulation.

Based on an individual assessment of risk factors for arterial or venous thrombosis and the risk of postoperative bleeding, this article outlines the preoperative and postoperative approach to anticoagulant management. Preceding this is a brief description of the therapies most commonly used in the perioperative period. The prevention of arterial thromboembolism is considered separately from the prevention of venous thrombosis. Perioperative management of anticoagulation can cause anxiety for patients, surgeons, anesthetists, and those who manage long-term anticoagulant therapy. Good communica…

Laryngospasm-induced pulmonary edema.

Laryngospasm is one of the more common forms of airway obstruction encountered by an anesthetist. Therapy usually is straightforward, with resolution of the obstruction normally occurring within minutes. In some cases, however, the patient's vigorous inspiratory efforts may lead to a rapidly deteriorating form of pulmonary edema. Two cases are presented that are quite typical of the development of this complication. The etiology, recognition and management of this form of non-cardiac pulmonary edema is discussed.

[Dealing with children and parents in anesthesia-Tips and tricks].

A stay in hospital and an operation are always an exceptional situation for children and their parents, which is accompanied by great uncertainty and fear. The aim of this article is to show what possibilities exist as a caring anesthetist to accompany a child and the parents through the operation and that anesthesia remains a good memory. The effect of communication on a verbal and nonverbal level is discussed. The focus is on dealing with children and their parents, taking the influence of the psychological developmental stages of children into account and on presenting helpful coping strate

Undiagnosed Thyrotoxicosis Storm in a Patient Presenting With a Perioperative Sinus Tachycardia and Hypertensive Crisis After Induction of Anesthesia for Emergent Bipolar Hip Hemiarthroplasty After Traumatic Femur Neck Fracture: A Case Report.

The clinical picture of sympathetic overdrive under anesthesia can be caused by many syndromes, which should be considered and excluded. Thus, the anesthetist needs a high level of suspicion and vigilance to recognize the diagnosis early and act accordingly. Emphasizing the pitfalls encountered in this exceptional case scenario, we present this rare case of a malnourished patient on antipsychotic medications facing an urgent operation concomitant with a thyroid storm, a situation that can rapidly progress to a decompensating multiorgan failure.

Consent for anaesthesia: guidelines from the Association of Anaesthetists.

INTRODUCTION: Guidelines on consent for anaesthesia were first issued by the Association of Anaesthetists in 1999 and last revised in 2017. These new guidelines respond to the changing ethical, legal and clinical landscape in which anaesthetists practice. Since 2017, the Montgomery judgment has changed medical consent practice. The General Medical Council standards on decision-making and consent and the National Institute for Health and Care Excellence guidance on shared decision-making also reinforce the need for high standards of patient advice and counselling. METHOD: A Working Party, comp

Cardiac arrest in obstetric patients receiving anaesthetic care: results from the 7th National Audit Project of the Royal College of Anaesthetists.

The 7th National Audit Project (NAP7) of the Royal College of Anaesthetists studied peri-operative cardiac arrest. Additional inclusion criteria for obstetric anaesthesia were: cardiac arrest associated with neuraxial block performed by an anaesthetist outside the operating theatre (labour epidural analgesia); and cardiac arrest associated with remifentanil patient-controlled analgesia. There were 28 cases of cardiac arrest in obstetric patients, representing 3% of all cardiac arrests reported to NAP7, giving an incidence of 7.9 per 100,000 (95%CI 5.4-11.4 per 100,000). Obstetric patients were

[Metamorphosis of Nurses: The Development and Breakthroughs of Advanced Practice Registered Nurses].

BACKGROUND: The introduction and development of the advanced practice registered nurse (APRN) is a global trend in nursing. However, the development of APRNs in Taiwan remains uncertain and lacks necessary consensus. PURPOSE: This research study aimed to explore the views and suggestions of nursing experts in industry, government, and academia regarding the development of APRNs (clinical nurse specialists, case managers, certified clinical registered nurse anesthetists, and certified nurse-midwives) in Taiwan. METHODS: Data were collected from March to August 2017. Sixty-four experts partici…

Inhalational versus propofol-based total intravenous anaesthesia: practice patterns and perspectives among Australasian anaesthetists.

Increasing evidence suggests that total intravenous anaesthesia (TIVA) may be the preferred anaesthetic for cancer resection surgery. To assist the preparation of a randomised controlled trial (RCT) examining Volatile (versus TIVA) Anaesthesia and Perioperative Outcomes Related to Cancer (VAPOR-C) we developed an 18-question electronic survey to investigate practice patterns and perspectives (emphasising indications, barriers, and impact on cancer outcomes) of TIVA versus inhalational general anaesthesia in Australasia. The survey was emailed to 1,000 (of 5,300 active Fellows) randomly selecte

An update on cystic fibrosis and implications for anesthesia.

Cystic fibrosis is a recessive inherited disorder that affects about 30,000 Americans. The majority of infants born with cystic fibrosis now reach adulthood, making surgical procedures performed in this patient group more common. The disease is a heterogeneous disorder caused by widespread dysfunction of exocrine glands and involving salivary, sweat, digestive, and pulmonary secretions. The broad, variable, and sometimes confusing array of clinical manifestations of cystic fibrosis include obstructive pulmonary disease, pancreatic insufficiency, abnormally high sweat electrolyte concentrations

Intraoperative ventilatory settings during abdominal surgery among 1,091 anesthesia providers.

BACKGROUND: Data are lacking regarding current ventilatory settings during abdominal surgery. Expert recommendations emphasize the need for protective ventilation, and several tools are now available to individualize positive end-expiratory pressure (PEEP). METHODS: We conducted a nationwide cross-sectional survey among French anesthesiologists and nurse anesthetists in November 2025 to assess ventilatory settings during abdominal surgery, both for laparotomy and laparoscopy. Protective ventilation was defined as the combination of low tidal volume (6-8 mL/kg predicted body weight), low-to-mo…

Use of Thromboelastography 6s in Trauma: An Integrative Review for Anesthesia Providers.

Trauma-induced coagulopathy is a major cause of preventable morbidity and mortality in adult trauma patients. Thromboelastography (TEG) has been shown to reduce blood product waste, morbidity, and mortality when used to guide trauma resuscitation. The TEG 6s is a new cartridge-based device that shifts the utility of TEG to the point-of-care. The purpose of this integrative review is to synthesize the current evidence regarding the use of the TEG 6s device in adult trauma patients with an emphasis on anesthesia. A comprehensive literature search was conducted following PRISMA 2020 guidelines, w

Improving the efficiency of sevoflurane delivery during general anaesthesia by educating and motivating anaesthetists to utilise the Volatile Efficiency Ratio.

Volatile anaesthetic agents such as sevoflurane contribute to greenhouse gas emissions, and selecting low fresh gas flows on anaesthetic machines minimises their waste. Facilitating improvements in sevoflurane use requires the education, motivation, and standardised evaluation of anaesthetists. There is currently no standard of practice related to the efficiency of anaesthetic gas delivery per case. We conducted a multi-component study termed 'Low With The Flow' (LWTF) to directly address these requirements by educating and motivating anaesthetists to reduce fresh gas flow and thereby sevoflur

Advances in diagnosing and managing fetal anomalies have enabled the rapid evolution of maternal-fetal surgery (MFS). These surgeries are br … he fetus and its surgical correction can ensure optimal anesthetic care. Certified registered nurse anesthetists (CRNAs) may have limited experience with MFS, but knowledge of these considerations will enable safer delivery of anesthesia. The type of anesthesia for MFS is determined by the surgical procedure. Open surgeries and EXIT procedures commonly require …

Thoracic spine stab injury with a retained knife.

A female was admitted to a Major Trauma Centre with an isolated thoracic spine stab injury involving a retained knife. The patient was haemodynamically stable with an unremarkable peripheral neurological examination. A CT scan with three-dimensional image reconstruction showed the knife blade lodged in the T11 vertebra with its tip close to the spinal cord, aorta and inferior vena cava. A multidisciplinary trauma team, including anaesthetists, vascular, neurosurgeons and general surgeons, agreed on the treatment strategy. The lodged knife was safely withdrawn in the operating theatre with the

Anesthesia Exposure in the Young Child and Long-term Cognition: An Integrated Review.

Millions of children every year undergo seemingly safe general anesthetics for surgical procedures and imaging studies. Anesthetic agents have been shown to cause detrimental effects on brain cell survival and cognitive function in animals. As a result, the safety of general anesthetics in children is an active field of investigation. The objective of this review is to evaluate the human research on anesthesia neurotoxicity in the young child. Three databases were searched for studies on anesthesia exposure in infants and children. Positive clinical outcomes in several studies showed no differ
