We used first-pass radionuclide angiocardiography to assess filling fraction during the first third of diastole, peak filling rate and peak filling rate during the first third of diastole as indexes of left ventricular diastolic performance at rest and after upright bicycle exercise in 32 normal patients and 68 patients with coronary artery disease. The mean filling fraction was unchanged from rest to exercise in normal patients (47+/- 15% vs 46 +/- 13%; NS). Even in 49 coronary patients with normal (greater than or equal to 50%) ejection fraction at rest, filling fraction was less than that i

Double-chambered right ventricle is a rare form of right ventricular outflow tract obstruction caused by anomalous hypertrophy of muscle bundles in right ventricle. Cases most often occur in children and rarely in adults. Most cases (80-90%) are associated with ventricular septal defect. We describe a case of pulmonary atresia and ventricular septal defect with double-chambered right ventricle. The interesting clinical findings, ECG, echocardiography and angiocardiography features are described here.

A 60-year-old woman with systemic sclerosis, lung fibrosis, and primary biliary cirrhosis was admitted to our hospital because of heart failure. Ventricular aneurysms were found in the apex and the posterior wall of the left ventricle by angiocardiography; however, there was no sign of coronary artery stenosis. A myocardial biopsy specimen revealed diffuse focal myocardial fibrosis. In this case, the patient with systemic sclerosis developed a rare complication of ventricular aneurysms without coronary disease.

Ultrafast CT and MR angiography are noninvasive, accurate and robust techniques for preoperative diagnosis and planning of congenital heart disease and vascular abnormalities in pediatric patients. While they seem to be equivalent to conventional catheter angiocardiography for detecting vascular abnormalities, they are more accurate for the diagnosis of potentially life-threatening complications, such as tracheal, bronchial or esophageal compression. The value of ultrafast CT and MR angiography is demonstrated in 22 pediatric patients with vascular rings, slings and other vascular abnormalitie

Corrected transposition of the great arteries is an unusual congenital heart defect, above all at ages over 40. Echocardiography and invasive angiography are still cornerstones for its diagnosis. However other non-invasive techniques, such radionuclide angiocardiography and computed tomography, not so used in this malformation, can help us in its detection. We present 2 cases of corrected transposition of the great arteries in adults in which these methods were used and review the literature.

Four children with systolic thrills and harsh pansystolic murmurs in the second left and right intercostal spaces, radiating to the base of the neck, and suspected of having a subaortic ventricular septal defect and possible aortic prolapse underwent angiocardiography using axial projections. All demonstrated an unusual "tunnel-like" track into the right ventricle and no subaortic ventricular septal defect or prolapse. In our opinion, these physical findings suggest this abnormality and that axial angiography clearly delineates the lesion.

Congenital supravalvular pulmonic stenosis was diagnosed in a dog. A pulmonary artery-to-pulmonary artery anastomosis of a Dacron arterial conduit restored continuity of blood flow around the obstruction in the main pulmonary artery. By the 6th day after surgery, right ventricular systolic pressure averaged 85 mm of Hg, a decrease of 90 mm from the preoperative value. Angiocardiography confirmed the patency of the conduit. At examination 6 months after surgery, the dog was clinically normal.

It has been demonstrated that echocardiography is a useful and accurate noninvasive technique for the study of congenital cardiovascular diseases. This method provides significant information in that invasive cardiac catheterization and angiocardiography can be planned with greater facility. This is especially important in severely ill babies in whom invasive diagnostic methods are risky. It is particularly attractive in neonates in whom the clinical differentiation of lethal heart disease from septic shock is difficult, since in the latter situation the echographic findings are normal.

An 11-month-old boy with congenital pulmonary vein stenosis, partial anomalous pulmonary venous connection, and ventricular septal defect is described. Angiocardiography demonstrated stenosis between the right upper pulmonary vein and high superior vena cava and obstruction of the right lower pulmonary vein. For pulmonary vein stenosis, we performed transverse sutured plasty for the right upper pulmonary vein, followed by right lower lobectomy. In some patients, combined management for pulmonary vein stenosis is effective.

Aortopulmonary window was prospectively diagnosed by magnetic resonance imaging in a 9-month-old boy. The diagnosis was confirmed by cardiac catheterization, angiocardiography, two-dimensional and color doppler echocardiography. Spin-echo T1-weighted axial and coronal magnetic resonance images show a large windowlike communication between left ascending aorta and right wall of the main pulmonary arterial trunk. To our knowledge, this is the first time that the diagnosis of the aorticopulmonary window was achieved by magnetic resonance imaging.

A 3-day-old neonate with severe central cyanosis was found to have a right to left shunt across a secundum atrial septal defect, due to prominence of the Eustachian valve. Two-dimensional echocardiography with Doppler and color Doppler allowed a precise morphological and pathophysiological diagnosis. Cardiac catheterization and angiocardiography confirmed the diagnosis. The patient is asymptomatic 12 months after surgical excision of the Eustachian valve and closure of the atrial septal defect, performed at two months of age.

Two patients with the classical clinical, angiographic, and echocardiographic signs of tricuspid atresia are reported, where the correct diagnoses were made at operation; one child had cor triatriatum dexter, the other child total anomalous systemic venous drainage to the left atrium. The literature on cor triatriatum dexter and total anomalous systemic venous drainage is discussed and a new angiographic sign for the differentiation from tricuspid atresia is reported: nonopacification of the right atrial appendage with right atrial angiocardiography.

We report an unusual case of bicuspid aortic valve in a 67-year-old man who had aortic valve replacement for combined severe aortic stenosis with moderate aortic regurgitation. Both preoperative echocardiography and angiocardiography failed to recognize the bicuspid nature of the aortic valve, which was discovered only at surgical pathology examination. The atypical anatomic findings of the valve consisted of a "cleft-like" appearance of the ventral leaflet formed by the two conjoined cusps together with a fenestrated rudimental raphe.

Infections occurring after aortic valve surgery, whether valvuloplasty or replacement, commonly affect the valve itself. However infection of the aortic suture line alone is extremely rare. Such cases with endarteritis can be diagnosed at autopsy or by angiocardiography. In this report a patient with a vegetation at the aortic suture line which was diagnosed by echocardiography is presented. By two-dimensional echocardiography, a very mobile and echo-dense mass protruding from the ascending aorta was observed on the recordings through the suprasternal notch. This diagnosis was surgically confi

The case of a thoracopagus with atrial and ventricular connection and of a thoracoischiopagus with atrial connection and multiple cardiac defects are reported. For the assessment of cardiac fusion, ECG, radionuclid-studies and CT are of limited value. By means of angiocardiography it is possible to classify patients in those with pericardial union only, with venae-cavae - or atrial connection and with ventricular connection. While separation of twins with pericardial fusion is often performed, successful separation of twins with atrial fusion is only once described, of twins with ventricular f

A patient had multiple bilateral stenoses of the pulmonary artery and its branches with systemic hypertension associated with mild stenoses of the renal arteries. Cardiac catheterization and angiocardiography are important in the evaluation of the degree of stenoses and pulmonary hypertension. This case suggests that in a child or young person with hypertension and a loud precordial murmur, lesions other than coarctation of the aorta may be present. Unexplained systemic hypertension requires further investigative workup which is essential for proper treatment and long-term management of these

A diastolic decrescendo murmur was heard along the left sternal border in 21 patients with advanced renal failure. Eight patients had organic aortic insufficiency. Among the remaining 13 patients, angiocardiography was carried out in 6; aortic regurgitation was demonstrated in 4, while 2 had stenosis of a coronary artery. Two of the 5 patients with positive angiography have died. At autopsy the aortic ostium was normal. The diastolic murmur in these patients is thought to be functional. It disappears during treatment with hemodialysis and renal transplantation.

Three patients with supravalvular aortic stenosis are described. One was a typical, sporadic-type supravalvular aortic stenosis, another a predominant aortic valve stenosis as part a complex heart lesion and the third inflammatory shrinking of the anulus fibrosus with an inflammatory aortic valve stenosis. The means of diagnosis are discussed. Angiocardiography and aortography are essential in order to demonstrate the pathology, haemodynamics, relation to the coronary arteries and the sinus of Valsalva, and in order to show other abnormalities. The clinical diagnostic features of right ventricular myxoma are described in a recent patient and related to the 15 cases previously reported. The presence of a pulmonic systolic ejection murmur with a delayed (120 to 140 msec.) and accentuated pulmonic second sound, or calcification in the region of the right ventricular outflow tract should suggest this lesion. Cardiac catheterization with angiocardiography in diagnostic. Ga scanning may assist in the diagnosis and followup after surgical removal of the myxoma. Early surgical removal will avoid the possibility of sudden death.
