Intraocular lens power calculation for planned ametropia: a clinical study.

A prospective series of 25 eyes received an intraocular lens (IOL) of power calculated for planned ametropia, by means of the formulae of R. D. Binkhorst, from data of axial length, corneal curvature, and postoperative anterior chamber depth. All the postoperative refractions were within the +/- 2 D range from the predicted refraction, confirming the clinical value of such calculation. A retrospective study of 100 eyes which had received a +19 D power Binkhorst IOL showed a wide range of change in refraction extending up to the +/- 6 D range, indicating that a 'standard' power IOL cannot be re

Corneal oxygen uptake under a soft contact lens in phakic and aphakic eyes.

A simple and comfortable procedure is used to measure the decay of oxygen content of a soft contact lens on a human cornea when the anterior surface of the lens is blocked from further supply of oxygen. Calculations from this measurement give the steady-state oxygen tension under the soft contact lens and the oxygen flux into the cornea when the eye is open but wearing a contact lens. When the procedure is applied to phakic and aphakic human eyes, the results indicate that for both states, the oxygen uptake by the cornea is about the same when the uptake levels are above 1 microliter/cm2 . hr

Glaucoma as a problem in intraocular lens implantation.

The literature has been investigated on the prevalence of preexisting and complicating glaucoma in intraocular lens surgery. Few exact data are available. The problem of the management of preexisting primary or postoperative secondary glaucoma in pseudophakia is discussed, in particular the inevitable restrictions the intraocular lens imposes on medical therapy and eventual glaucoma surgery. Preexisting glaucoma should therefore be a relative contraindication to intraocular lens surgery.

Visual acuities of monocular IOL and non-IOL aphakic children.

The vision of 341 monocular aphakic children was corrected with contact lens, intraocular lens, or spectacles and the visual results of each series were compared. The acquired and infantile contact lens and IOL cataract patients achieved similar levels of aphakic visual acuities. Seventy percent of the IOL and 64% of the contact lens-acquired cataract patients achieved 20/20 to 20/40, while 22% of the IOL and 16% of the contact lens infantile cataract patients also reached this level. Twenty patients wore monocular spectacles and none achieved adequate levels of visual rehabilitation. Forty-si

An over-refraction program for the TI 59 card-programmable calculator.

Over-refraction provides a powerful method for the refractive examination of the aphakic or highly ametropic patient. Nevertheless, in the past, practical difficulties that attend mathematical or graphical combination of over-refraction and original spherocylinders have tended to limit the clinical use of over-refraction methods. Programmable calculators, now available inexpensively, render over-refraction clinically practical. An over-refraction program for the Texas Instruments TI-59 Card-Programmable Calculator is presented. Program running time, exclusive of data entry and readout, is unde

Iseikonic lens implantation in anisometropia.

The implantation of an iseikonic prepupillary intraocular lens in unilateral aphakia usually requires a slight (2 D) myopia of the implanted eye if the two eyes are isometropic. On the other hand, the implantation of an iseikonic implant in one eye of a anisometropic (asymmetric) eye pair can result in unexpectedly high changes in spectacle ametropia, due to differences in axial length or corneal power. The changes in ametropia after iseikonic lens implantation are discussed in relation to preoperative corneal and axial anisometropias. Alternative procedures to iseikonic implantation are descr

[Retinal detachment in the aphakic eye (author's transl)].

The picture of retinal detachment following cataractextraction is studied in 86 cases of aphakic detachment, and the hypothesis that two different forms of detachment exists is proposed. In order to study this hypothesis the time-interval, the refraction and the type of tear is analysed. The results seem to indicate that two forms are present. The first one, the so-called "non: aphakic" detachment, appears in the predisposed eye and will occur after a short time-interval. The other form, the "true" aphakic detachment appears at a later stage and though this form more often shows no clear-cut t

Custom-made anterior chamber intraocular lens for aphakia: a case report.

BACKGROUND: Aphakia without adequate capsular support requires alternative intraocular lens (IOL) fixation strategies. Standard options include sulcus-fixated posterior chamber IOLs and retropupillary iris-claw lenses, both of which depend on specific anatomical prerequisites. When these are absent, anterior chamber IOLs (AC-IOLs) become one of the few remaining options; however, the only widely available angle-supported model has been the rigid Kelman Multiflex (PMMA), unchanged since its FDA approval in 1980. We describe the development and three-year clinical outcome of a custom-made, folda
