Arm adductor with arm abduction in rotator cuff tear patients vs. healthy -- design of a new measuring instrument [corrected].

The incidence of (a)symptomatic rotator cuff tears is high, but etiologic mechanisms are unclear and treatment outcomes vary. A practical tool providing objective outcome measures and insight into etiology and potential patient subgroups is desirable. Symptomatic cuff tears coincide with humerus cranialization. Adductor co-activation during active arm abduction has been reported to reduce subacromial narrowing and pain in cuff patients. We present an easy-to-use method to evaluate adductor co-activation. Twenty healthy controls and twenty full-thickness cuff tear patients exerted EMG-recorded …

Cooperative binding of the outer arm-docking complex underlies the regular arrangement of outer arm dynein in the axoneme.

Outer arm dynein (OAD) in cilia and flagella is bound to the outer doublet microtubules every 24 nm. Periodic binding of OADs at specific sites is important for efficient cilia/flagella beating; however, the molecular mechanism that specifies OAD arrangement remains elusive. Studies using the green alga Chlamydomonas reinhardtii have shown that the OAD-docking complex (ODA-DC), a heterotrimeric complex present at the OAD base, functions as the OAD docking site on the doublet. We find that the ODA-DC has an ellipsoidal shape ∼24 nm in length. In mutant axonemes that lack OAD but retain the ODA-…

An extremely polymorphic locus on the short arm of the human X chromosome with homology to the long arm of the Y chromosome.

A genomic DNA clone named CRI-S232 reveals an array of highly polymorphic restriction fragments on the X chromosome as well as a set of non-polymorphic fragments on the Y chromosome. Every individual has multiple bands, highly variable in length, in every restriction enzyme digest tested. One set of bands is found in all males, and co-segregates with the Y chromosome in families. These sequences have been regionally localized by deletion mapping to the long arm of the Y chromosome. Segregation analysis in families shows that all of the remaining fragments co-segregate as a single locus on the …

Photobacterium damsela Necrotizing Fasciitis of the Arm.

Photobacterium damsela is a gram-negative bacterium that is known to cause infection, most commonly in marine animals and rarely in humans. Photobacterium damsela typically results in skin and soft tissue infections in humans and has been associated with necrotizing fasciitis in several case reports. After an initially benign presentation, P damsela necrotizing fasciitis often progresses rapidly to multiorgan failure and death. Here, we present a fatal case of P damsela necrotizing fasciitis after a minor cut while scaling a fish with a discussion of current literature and treatment strategies

Surgical robotic arm control for tissue ablation.

In the technology driven era, robot assisted surgery is gradually emerging as a revolutionized surgical procedure over traditional laparoscopic method. Despite the concerns about robotic surgery for minimally invasive surgical procedures, robotized surgical arms have been used in many hospitals. Certain surgical procedures require removal of a segment of an organ or body part like excision biopsy, linear thin layer of soft tissue, triangular mass, and tangential excision in burn management, where shaving-off at an angle of the tissue layer to be removed. For such minimally invasive procedures,…

Human mimetic forearm mechanism towards bionic arm.

The paper presents a mechanical forearm mechanism to extend the pronation and supination (P/S) movement of the trans-radial amputee when he/she is able to conduct the P/S motion partially with his/her remaining forearm. The P/S motions of the existing prostheses and robotic arms developed till now are completely different from those of the human. When we are to develop the robotic prostheses for the amputees, two important issues should be considered such as cosmetic issue to look like normal arm and motion harmony issue between robotic prosthesis and the amputated living arm. Most prostheses …

Fractures of the humerus during arm wrestling.

BACKGROUND/AIM: Humeral shaft fractures may occur as a result of arm wrestling. The aim of this study was to present our treatment of humerus fracture sustained during arm wrestling. METHODS: A total of six patients, aged 22 to 48, were treated at our department form January 2008 to January 2010 with open reduction and internal fixation and with hanging arm casts. A review of all the relevant literature on the subject was also presented. RESULTS: In all the cases, the fractures healed and function returned to normal. No patient had any neural or vascular compromise. CONCLUSION: Closed and o…

Hand up! Yawn and raise your arm.

BACKGROUND: In some cases of hemiplegia, the initiation of yawning is associated with involuntary raising of the paralysed arm. Reports are scarce in the literature, probably because the phenomenon has largely been overlooked. METHODS: We studied six patients from two neurologic units, and compared them with published cases from the last 200 years. Brain imaging typically shows a small vascular lesion most often located in the internal capsule. RESULTS: After comparison with experimental models in cats, we suggest that damage to the cortico-neocerebellar tract of the extrapyramidal system di…

Unilateral cerebellar lesions influence arm movements bilaterally.

Limb ataxia is seen as a sign of ipsilateral cerebellar dysfunction. However, imaging studies have shown a bilateral cerebellar activation d … ions had significantly slower movements not only for the ipsilateral but also for the contralateral arm. The time lag of these movements in patients was also significantly larger for both arms. In contrast, there was no significant difference in dysmetria at the endpoints. These findings indicate that both ipsilateral and contra-lateral movements of p …

Patient perception of the ischemic arm block.

OBJECTIVE: To investigate patient perception of the ischemic arm block. DESIGN: Retrospective study. SETTING: Fracture clinic of a major city hospital. PATIENTS/PARTICIPANTS: One hundred eighty-one consecutive patients who had an ischemic arm block were mailed a questionnaire. The response rate was 71 percent. RESULTS AND CONCLUSIONS: Forty-two percent of respondents reported significant pain from the tourniquet cuff, and 29 percent reported significant pain during manipulation or within the first hour after releasing the cuff. Statistical analysis showed that cuff pain was independent and…

[Hand-arm vibration syndrome in caisson miners].

Authors examined 43 caisson-miners with symptoms of the upper extremities because of suspicion of hand-arm vibration syndrome. Also vibration measurements were performed on the pneumatic hammer used by the workers. The acceleration of the vibration exceeded 2.5-3.5 times the maximum allowable level according to the ISO 5349. Symptoms and signs of hand-arm vibration syndrome were found in 39 cases (90.7%). The vascular, peripheral neurological and locomotor system of the upper extremities were affected in similar frequency: 54.8; 51.6 and 51.2%, respectively. The most common angiological altera…

Traumatic arm amputation, an uncommon parachuting injury.

Parachuting is a unique method of deploying troops. Accidents in parachuting are common, but usually carry minimal risk for severe injury. Most accidents occur during landing and injuries often affect the lower limbs and spine. Traumatic amputation of a limb is an extremely rare event in parachuting. We present a case of a parachutist who suffered a traumatic amputation of the left hand during jumping off the plane.

Osteocutaneous lateral arm flap in hand reconstruction.

Complex injuries of the hand often have severe soft tissue damage and associated intercalary segmental or distal loss of bone. Bone grafting to restore the skeletal scaffold is essential for soft tissue reconstruction. With a poor vascular bed, non-vascularised free bone grafts are not desirable. We describe 14 cases of vascularised humeral bone grafting for these difficult reconstructions. The bone was harvested as an osteocutaneous lateral arm flap. Constant osseous branches from the main pedicle of the posterior radial collateral artery which travel along the lateral intermuscular septum to…

Preservation of arm veins for arterial reconstruction.

The results of surgery for lower extremity salvage have improved steadily over the past decade. One of the principles accounting for this ad … tis, variations in venous anatomy, previous vein stripping, or other conditions. In these patients, arm veins (cephalic and basilic) have been used successfully for limb salvage. There are several requirements for the successful use of arm veins. These include a detailed knowledge of the anatomy of the cephalic and basilic veins, education of patients …
