High-density EEG power topography and connectivity during confusional arousal.

Confusional arousal is the milder expression of a family of disorders known as Disorders of Arousal (DOA) from non-REM sleep. These disorders are characterized by recurrent abnormal behaviors that occur in a state of reduced awareness for the external environment. Despite frequent amnesia for the nocturnal events, when actively probed, patients are able to report vivid hallucinatory/dream-like mental imagery. Traditional (low-density) scalp and stereo-electroencephalographic (EEG) recordings previously showed a pathological admixture of slow oscillations typical of NREM sleep and wake-like fas…

Autonomic nervous system arousal and cognitive functioning in bipolar disorder.

OBJECTIVE: Previous theories about the etiology of cognitive dysfunction in bipolar disorder (BD) emphasized trait factors such as neurological impairment. State factors, other than mood symptoms, that may exacerbate functional deficits have not yet been considered. The purpose of this study was to examine autonomic nervous system (ANS) arousal following cognitive challenge. The study compared patients with BD and healthy controls (HC) in physiological measures and neuropsychological test scores. METHODS: Thirty euthymic patients with BD and 22 HC completed the study. Participants completed m…

Hypofunction of right temporoparietal cortex during emotional arousal in depression.

CONTEXT: Neuropsychological models of depression highlight temporoparietal hypofunction associated with low emotional arousal in major depressive disorder (MDD). These models were derived from indirect measures such as neuropsychological tests and electroencephalography alpha band power. OBJECTIVE: To determine if high-arousing stimuli directly modulated activity in attention and arousal-related sensory brain regions in patients with MDD. DESIGN: Between-group comparison (patients with MDD vs healthy control subjects) of neuromagnetic oscillatory activity driven by flickering emotional and n…

MRI of female genital and pelvic organs during sexual arousal.

We utilized contrast enhanced magnetic resonance imaging (MRI) to delineate the anatomy of the female genital and pelvic organs during sexual arousal. Eleven healthy pre-menopausal women and eight healthy post-menopausal women underwent MRI of the pelvis while watching an erotic video. A 1.5 Tesla MR system was used to produce T1-weighted images following administration of MS-325, a gadolinium-based blood pool contrast agent. Selected structural dimensions and enhancement were measured prior to and during sexual arousal. In both pre- and post-menopausal subjects, vestibular bulb and labia mino…

Individual differences in cognitive performance due to right hemisphere arousal.

The purpose of this study was to investigate the relationship between individual differences in cognition and asymmetric patterns of cortical arousal in normal right-handed adults. Leftward asymmetry on a chimeric faces task was correlated with the ability to recognise faces and facial expression, and certain aspects of social skill. The correlation with face and emotional expression recognition was significant only for subjects who reported using a non-verbal strategy. These results further support the role of the right hemisphere in various aspects of social cognition, but also point to the …

Hypercapneic arousal responses in children with congenital central hypoventilation syndrome.

Congenital central hypoventilation syndrome (CCHS, Ondine's curse) is generally thought to be due to insensitivity of the central chemoreceptors to carbon dioxide. Children with CCHS have absent ventilatory responses to both hypercapnea and hypoxia, suggesting either abnormal central and peripheral chemoreceptor function or abnormal central integration of chemoreceptor input. Because ventilatory and arousal responses to respiratory stimuli are distinct from each other, if children with CCHS have complete chemoreceptor dysfunction, one would predict that both ventilatory and arousal responses t…

Effects of experimentally adopted sexual schemas on vaginal response and subjective sexual arousal: a comparison between women with sexual arousal disorder and sexually healthy women.

The present study evaluated and compared the effects of experimentally adopted sexual schemas on vaginal response, subjective sexual arousal, and affect in 17 women with Female Sexual Arousal Disorder (FSAD) and 17 sexually healthy women. Positive and negative cognitive schemas were presented to participants before viewing sexually explicit video segments. They were asked to temporarily adopt both schemas, and vaginal response, subjective sexual arousal, and affect were measured in each schema condition. Participants in both groups had significantly greater vaginal response and reported more s…

Astrocyte activation in the ventrolateral medulla modulates breathing and arousal states.

Astrocytes play vital roles in regulating brain states across organisms. Specifically, they regulate breathing behaviors and associated brain states, including facilitating transitions between breathing phases by sensing changes in O₂ and CO₂ levels, regulating the sleep-wake cycle, and impacting arousal and wakefulness. Here, we test the hypothesis that astrocytes in the ventral respiratory column (VRC) are important for arousal and sigh generation in alert mice. Our results reveal that a subset of Aldh1l1 cells in the VRC are activated prior to sigh generation and are recruited by hypoxia. C…

Cortical synchrony is reduced in Alzheimer's disease and relates to arousal state.

INTRODUCTION: The brain is a complex dynamical system, influenced by arousal state. Cortical synchrony supports information processing and is disrupted in Alzheimer's disease (AD). Locus coeruleus (LC) integrity and pupillometry index arousal system structure and function. METHODS: Sixty-four AD and 26 controls underwent resting-state pupillometry-fMRI. Neuromelanin MRI and Addenbrooke's Cognitive Examination were conducted. Mean and standard deviation of blood oxygen level dependent (BOLD) phase coherence yielded synchrony and metastability, respectively. Leading Eigenvector Dynamics Analysi…

Neuronal rhythmicity and cortical arousal in a mouse model of absence epilepsy.

OBJECTIVES: Absence seizures impair psychosocial function, yet their detailed neuronal basis remains unknown. Recent work in a rat model suggests that cortical arousal state changes prior to seizures and that single neurons show diverse firing patterns during seizures. Our aim was to extend these investigations to a mouse model with studies of neuronal activity and arousal state to facilitate future fundamental investigations of absence epilepsy. METHODS: We performed in vivo extracellular single unit recordings on awake head-fixed C3H/HeJ mice. Mice were implanted with tripolar electrodes fo…

The role of arousal in maintaining the relationship between insomnia and gastrointestinal conditions.

The relationship between gastrointestinal (GI) conditions and sleep disturbance has been well established. With a higher-than-average prevalence of sleep disturbance in individuals with GI conditions, it is imperative to better understand the maintaining factors driving this comorbidity. Although there are separate, ongoing investigations into both the biological mechanisms and interventions for the sleep and GI relationship, there is a considerable need to further specify common and mutually influential pathways. In our review, we highlight arousal as both a unifying feature of insomnia and v…

Effect of Zolpidem on nocturnal arousals and susceptibility to central sleep apnea.

PURPOSE: Arousals may contribute to the pathogenesis of sleep-disordered breathing (SDB) and central sleep apnea (CSA). We aimed to determine the effect of the nonbenzodiazepine hypnotic zolpidem on the frequency of respiratory-related arousals and central apnea in patients with moderate-to-severe SDB. We hypothesized that zolpidem decreases the severity of SDB by decreasing the frequency of respiratory-related arousals. METHODS: Patients with apnea-hypopnea index ≥ 15 events/hour and central apnea-hypopnea index ≥ 5 events/hour underwent a sleep study on zolpidem 5 mg and a sleep study with …

Role of serotonergic dorsal raphe neurons in hypercapnia-induced arousals.

During obstructive sleep apnea, elevation of CO2 during apneas contributes to awakening and restoring airway patency. We previously found that glutamatergic neurons in the external lateral parabrachial nucleus (PBel) containing calcitonin gene related peptide (PBelCGRP neurons) are critical for causing arousal during hypercapnia. However, others found that genetic deletion of serotonin (5HT) neurons in the brainstem also prevented arousal from hypercapnia. To examine interactions between the two systems, we showed that dorsal raphe (DR) 5HT neurons selectively targeted the PBel. Either genetic…

Neural correlates of subjective arousal and valence in health and panic disorder.

Aberrant emotion processing is a core characteristic of panic disorder (PD). Findings concerning the underlying neural pathways remain inconsistent. We applied functional magnetic resonance imaging (fMRI) in the context of a task based on the circumplex model of affect. This model links affective states to two underlying neurophysiological systems: arousal and valence. Twenty-two healthy participants and 20 participants with PD rated arousal and valence in response to affective faces during fMRI. In healthy controls, we found that arousal modulated the hemodynamic response in the parahippocamp…

Disruption of the ascending arousal network in acute traumatic disorders of consciousness.

OBJECTIVE: To determine whether ascending arousal network (AAn) connectivity is reduced in patients presenting with traumatic coma. METHODS: We performed high-angular-resolution diffusion imaging in 16 patients with acute severe traumatic brain injury who were comatose on admission and in 16 matched controls. We used probabilistic tractography to measure the connectivity probability (CP) of AAn axonal pathways linking the brainstem tegmentum to the hypothalamus, thalamus, and basal forebrain. To assess the spatial specificity of CP differences between patients and controls, we also measured C…
