Surgical management of a giant left atrial appendage aneurysm and chronic atrial fibrillation.

Left atrial appendage aneurysm (LAAA) is a rare congenital cardiac anomaly that is associated with potentially life-threatening complications, including thromboembolism and arrhythmias. We report a 36-year-old male patient with giant LAAA (66 × 46 mm) and chronic atrial fibrillation (CAF) presenting with palpitations. Surgical resection of LAAA and Cox-Maze IV procedure were performed under cardiopulmonary bypass. The patient was discharged in sinus rhythm, remained asymptomatic, and had normal echocardiographic findings at follow-up.

Causal Machine Learning for Left Atrial Appendage Occlusion in Patients With Atrial Fibrillation.

BACKGROUND: Transcatheter left atrial appendage occlusion (LAAO) is an alternative to lifelong anticoagulation, but optimal patient selection remains challenging. OBJECTIVES: This study sought to apply a novel causal machine learning framework to identify patients who would benefit from LAAO vs a direct oral anticoagulant (DOAC). METHODS: We identified 744,190 adult patients with atrial fibrillation treated with either LAAO or DOAC between March 13, 2015, and December 31, 2019, using data from OptumLabs Data Warehouse. One-to-one propensity score matching was used to create a cohort where pa

Left atrial appendage occlusion in atrial fibrillation: shaping the future of stroke prevention.

Atrial fibrillation (AF) is the most common cardiac arrhythmia, significantly increasing the risk of thromboembolic events, particularly ischemic stroke. The left atrial appendage (LAA) is the predominant site of thrombus formation in patients with AF, making it a crucial target for stroke prevention strategies. Left atrial appendage occlusion (LAAO) has emerged as an important therapeutic alternative to oral anticoagulation, particularly in patients with contraindications to long-term anticoagulant therapy. This review examines the role of LAAO in AF management, discussing current indications

Safety of left atrial appendage amputation in cardiac surgery patients without atrial fibrillation.

BACKGROUND: Left atrial appendage amputation (LAAA) lowers the stroke risk in patients with atrial fibrillation (AF), but its safety and functional implications in non-AF patients are uncertain. OBJECTIVE: We aimed to assess early post-operative safety of LAAA in non-AF patients undergoing cardiac surgery. METHODS: This retrospective cohort study included 434 patients without AF (CHA2DS2-VASc ≥2) who underwent elective cardiac surgery at 2 centers (2015-2018). We compared 150 patients who underwent LAAA as part of the PREventive left atrial appenDage resection for the predICtion of fuTure At

Left atrial appendage closure in nonvalvular atrial fibrillation patients with percutaneous coronary intervention.

OBJECTIVES: Nonvalvular atrial fibrillation (NVAF) concomitant with coronary artery disease (CAD) may increase the risk of thromboembolism. Antithrombotic therapy for NVAF patients with percutaneous coronary intervention (PCI) remains contradictory and challenging. This study aimed to assess the safety and efficacy of left atrial appendage closure (LAAC) in a cohort of patients with NVAF and PCI. METHODS: A total of 109 patients undergoing LAAC procedures between March 2017 and December 2020 were categorized into 2 groups, Group I included 36 patients with PCI while group II included 73 patie

Indications for percutaneous left atrial appendage occlusion in hospitalized patients with atrial fibrillation.

AIMS: Percutaneous left atrial appendage occlusion (LAAO) is an alternative nonpharmacological therapeutic option for stroke prevention in patients with NVAF. However, no data exist on potential LAAO candidates' prevalence among 'real-world' NVAF patients. This study aimed to investigate the indications for LAAO in hospitalized patients with comorbid nonvalvular atrial fibrillation (NVAF). METHODS: This is a post-hoc analysis of the MISOAC-AF (Motivational Interviewing to Support OAC-AF, ClinicalTrials.gov: NCT02941978), randomized controlled trial, which enrolled NVAF patients hospitalized f

Left atrial appendage closure with the watchman device reduces atrial fibrillation management costs.

AIMS: To report hospitalization costs of patients with non-valvular atrial fibrillation (AF) submitted to percutaneous left atrial appendage closure (LAAC) with the Watchman device. METHODS: Pre- and post-procedural hospitalization AF-related costs were calculated using the DRG system (diagnosis-related groups) and compared. RESULTS: Between 2012 and 2016, 677 non-valvular AF patients underwent LAAC. Median time from first cardiac hospitalization to LAAC was 5.9 years (IQR 1.6-9.1) and median follow-up after LAAC was 4.8 years (IQR 3.6-5.6). LAAC mortality was 1.3% and follow-up mortality 16

Thoracoscopic Left Atrial Appendage Excision Plus Ablation for Atrial Fibrillation to Prevent Stroke.

Atrial fibrillation (AF) patients with a previous stroke are often at a high risk of recurrent stroke and bleeding. Anticoagulation therapy in such patients is a challenging dilemma. Thoracoscopic left atrial appendage excision (LAAE) plus AF ablation is an interventional approach offered to some AF patients. We hypothesized that this approach may be suitable as a secondary stroke prevention strategy for these high-risk patients. Between January 2013 and December 2016, a total of 44 patients (26 male; mean age 65.0 ± 9.1 years) with nonvalvular AF and a previous stroke or systemic thromboembol

Right atrial appendage aneurysm complicated with atrial septal defect: A rare case report.

Right atrial appendage aneurysms (RAAAs) are rare heart malformations, presenting as isolated anomalies or co-existing with other structural heart diseases. We describe a rare case of RAAA complicated with an atrial septal defect (ASD). The diagnosis was established using transthoracic echocardiography and confirmed using cardiac magnetic resonance imaging. To treat the ASD and reduced right atrium volume load, ASD transcatheter closure was performed. On echocardiography performed 3 months post discharge, the RAAA was observed to have reduced in size compared to that presurgery. Six years late

Left atrial appendage exclusion during mitral valve surgery and stroke in atrial fibrillation.

PURPOSE: The purpose of this study was to determine whether surgical left atrial appendage (LAA) exclusion performed during mitral valve surgery is associated with a reduction in cerebrovascular events in patients with atrial fibrillation. METHODS: We retrospectively studied patients with atrial fibrillation who underwent mitral valve surgery from 1/1/2001 through 12/31/2014. We screened 1352 patients using ICD-9 codes and included 281 patients in the study. The primary end point was a composite of strokes and transient ischemic attacks occurring within 5 years after surgery. Secondary end po

Epicardial ablation of focal atrial tachycardia arising from left atrial appendage in children.

Focal left atrial tachycardia (FLAT) although a common cause of supraventricular tachycardia(SVT) among children, the one's arising from left atrial appendage (LAA) present a unique challenge for successful ablation because of anatomical location. We present two children with FLAT arising from the epicardial LAA, successfully mapped and ablated through percutaneuous epicardial approach.

Determinants of left atrial appendage volume in stroke patients without chronic atrial fibrillation.

BACKGROUND: Left atrial appendage (LAA) volume has been shown to be increased in patients with acute cryptogenic stroke. Atrial fibrillation (AF) is a well-recognized risk factor but it is not the only one associated with LAA enlargement. The aim of the study was to clarify the multifactorial etiology of LAA enlargement in cardiogenic stroke/TIA patients without AF. METHODS: Altogether 149 patients with suspected cardioembolic stroke/TIA (47 females; mean age 61 years) underwent cardiac CT. Diagnosed AF on admittance was an exclusion criteria but 24-hour Holter ambulatory ECG revealed paroxys

Transcatheter closure of patent foramen ovale, atrial septal defect and left atrial appendage.

Intracardiac defects such as atrial septal defect (ASD) and patent foramen ovale (PFO) are common forms of congenital intracardiac apertures which can be successfully closed percutaneously. Since the initial description of an atrial septal defect closure device in the mid 1970s by King and Mills, transcatheter closure of atrial septal defects and patent foramen ovale using various devices has now become an established practice in many centers. The left atrial appendage is a trabeculated remnant of the embryonic left atrium. This is an important source of emboli related to atrial fibrillation.

[Atrial lead placement in the right atrial appendage during recent or chronic arrhythmia].

An atrial arrhythmia could be encountered during the atrial lead implantation. The lead placement must subsequently be delayed after restitution of the sinus rhythm or completely abandoned. The authors investigate the atrial lead placement during atrial arrhythmia and the lead performance at 6-month follow-up. The study population was 65 patients aged 78.5 years, 42 males and 28 structural heart diseases. They were implanted for sick sinus syndrome (n=14), atrioventricular block (n=44), infra-hisian conduction abnormality (n=7) in association with an atrial fibrillation (63.1%), an atrial flut

Inverted left atrial appendage presenting as a left atrial mass after cardiac surgery.

Inversion of the left atrial appendage can masquerade as a new left atrial mass. Failure to be aware of this entity can result in unnecessary diagnostic and therapeutic procedures. If the entity is diagnosed intraoperatively, treatment is simple with external reduction and ligation.

Left atrial, pulmonary vein, and left atrial appendage anatomy in Indigenous individuals: Implications for atrial fibrillation.

BACKGROUND: Indigenous Australians experience a greater burden of AF. Whether this is in-part due to differences in arrhythmogenic structures that appear to contribute to AF differences amongst other ethnicities is not known. METHODS: We studied forty individuals matched for ethnicity and other AF risk factors. Computed tomography imaging was used to characterise left atrial (LA), pulmonary vein (PV), and left atrial appendage (LAA) anatomy. RESULTS: There were no significant differences in LA diameters or volumes between Indigenous and non-Indigenous Australians. Similarly, we could not det

The Left Atrial Appendage: Anatomy, Electrophysiology, and Clinical Implications for Atrial Fibrillation and Stroke.

PURPOSE OF REVIEW: The left atrial appendage (LAA) is nowadays recognized as both an arrhythmogenic substrate and the dominant source of thromboembolism in atrial fibrillation (AF). In this review we tried to summarize recent data on the anatomy, physiology, and imaging of the LAA, and examine how these findings help guide rhythm-control and stroke-prevention strategies. RECENT FINDINGS: High-resolution mapping confirms the LAA as a non-pulmonary-vein trigger source in persistent and post-ablation AF, supporting LAA electrical isolation in selected patients, though isolation may induce mechan

Post-cardioversion anticoagulation in atrial fibrillation with left atrial appendage occlusion: Necessary or not?

BACKGROUND: The role of post-cardioversion oral anticoagulation (OAC) in patients with atrial fibrillation (AF) and left atrial appendage occlusion (LAAO) remains unclear. Limited evidence exists regarding stroke, systemic thromboembolism (SSTEs), and bleeding risks in this population. OBJECTIVE: To evaluate the clinical outcomes following electrical cardioversion (eCVN) in patients with AF and prior LAAO. METHODS: We conducted a retrospective cohort study using the TriNetX Research Network to identify patients with prior surgical or percutaneous LAAO, no history of stroke, and no OAC use fo
