Exercise and Nutrition Interventions for Prehabilitation in Hepato-Pancreato-Biliary Cancers: A Narrative Review.

Gastrointestinal (GI) cancers constitute over 25% of global cancer cases annually, with hepato-pancreato-biliary (HPB) cancers presenting particularly poor prognosis and challenging surgical treatments. While advancements in clinical care have improved post-operative outcomes over time, surgery for HPB cancers remains associated with high morbidity and mortality rates. Patients with HPB cancer are often older, diagnosed at later stages, and have a higher prevalence of co-morbid conditions, leading to reduced life expectancy, suboptimal post-operative recovery, and increased recurrence risk. Long-term follow-up of patients with familial adenomatous polyposis undergoing pancreaticoduodenal surgery.

Adenomatous polyps and adenocarcinomas of the periampullary region are the most common upper gastrointestinal neoplasms encountered in familial adenomatous polyposis (FAP) patients. Tumors arising from the liver, biliary tract, and pancreas have also been reported. The purpose of this study was to review the clinical outcome of FAP patients after pancreaticoduodenal surgery for periampullary neoplasms. Of the 61 individuals participating in our prospective FAP registry, 8 underwent surgical resection of periampullary neoplasms between 1987 and 1998. The charts of these individuals were reviewe

Diabetes and the hepatobiliary system.

Diabetes mellitus can lead to metabolic changes that alter normal hepatic and biliary function and structure. These alterations in hepatic and biliary function and structure are usually benign, but in certain situations lead to significant, disabling disease. This article reviews the hepatic and biliary complications of diabetes, with emphasis on epidemiology, diagnosis, and management, as well as the glucose intolerance seen in liver disease.

[Peritoneovenous Shunting as a Palliative Therapy for Refractory Ascites after Biliary Cancer Surgery].

The quality of life of patients with recurrence of biliary cancer is remarkably diminished by the occurrence of refractory ascites caused by peritoneal dissemination or portal vein obstruction. In this study, we examined the effectiveness of peritoneovenous shunting (PVS) in patients with refractory ascites after recurrence of biliary cancer. PVS was performed 7 times in 5 patients, with 6 shunts (85.7%) placed from the left-side abdomen to the left subclavian vein. During the median observation period of 136 days, the ascites and symptoms of abdominal distension disappeared in 4 patients. Sev

Inflammatory polyp in the common bile duct with pancreaticobiliary maljunction.

A 63-year-old woman was admitted because of epigastric pain and obstructive jaundice. Endoscopic retrograde cholangiopancreatography and intraductal ultrasonography revealed a 25-mm papillary nodule in the middle to inferior portion of the common bile duct (CBD). Pancreaticobiliary maljunction (PBM) was also identified. Contrast-enhanced computed tomography also showed an enhanced nodule in the CBD, and we suspected intraductal papillary neoplasm of the bile duct. We performed pylorus-preserving pancreatoduodenectomy. Postoperative pathological examination revealed an inflammatory polyp in the

Response of BRCA1-mutated gallbladder cancer to olaparib: A case report.

Gallbladder cancer (GBC), although considered as a relatively rare malignancy, is the most common neoplasm of the biliary tract system. The late diagnosis and abysmal prognosis present challenges to treatment. The overall 5-year survival rate for metastatic GBC patients is extremely low. BRCA1 and BRCA2 are the breast cancer susceptibility genes and their mutation carriers are at a high risk for cancer development, both in men and women. Olaparib, an oral poly ADP-ribose polymerase inhibitor, has been approved by the Food and Drug Administration and the European Commission for the treatment of

Treatment Strategies for Biliary Tract Cancer: A Surgical Perspective from Historical Evolution to Neoadjuvant Therapy.

BACKGROUND: Biliary tract cancer (BTC) is an aggressive malignancy with poor prognosis despite curative resection, necessitating multidisciplinary treatment strategies beyond surgery alone. METHODS: This narrative review summarizes pivotal clinical trials and guideline-defining studies on adjuvant and neoadjuvant therapies, including BILCAP, BCAT, PRODIGE 12, and ASCOT. RESULTS: Adjuvant chemotherapy, particularly capecitabine, has become standard after resection, although optimal regimens remain controversial. Neoadjuvant therapy shows promise for improving patient selection and R0 resectio

Finding the Right Partner: Triplet Therapy for First-Line Advanced Biliary Tract Cancers.

The Oncology Grand Rounds series is designed to place original reports published in the Journal into clinical context. A case presentation is followed by a description of diagnostic and management challenges, a review of the relevant literature, and a summary of the authors' suggested management approaches. The goal of this series is to help readers better understand how to apply the results of key studies, including those published in Journal of Clinical Oncology, to patients seen in their own clinical practice.

Circular RNA-Based Molecular Computation Enhances Plasma Biomarker Detection in Biliary Tract Cancer.

Molecular diagnosis of biliary tract cancer (BTC) remains a significant clinical challenge due to the lack of sensitive and specific diagnostic tools. Although large panels based on multi-biomarkers have demonstrated potential in enhancing diagnostic accuracy, their clinical application is hindered by complexity and high costs. To overcome these limitations, we have developed a DNA-based molecular computation system that integrates the detection of plasma circular RNAs (circRNAs) with molecular computation, enabling intelligent and rapid diagnosis of BTC. By identifying and validating a specif

Prognostic value of the modified Glasgow prognostic score in biliary tract cancer patients: a systematic review and meta-analysis.

BACKGROUND: Biliary tract cancer (BTC) is an invasive adenocarcinoma affecting the hepatobiliary system, but high recurrence rates highlight the need for more effective adjuvant approaches. The modified Glasgow prognostic score (mGPS) has been explored as an independent prognostic indicator in patients with BTC. However, consensus on its prognostic value is lacking. This meta-analysis aimed to comprehensively assess the association between mGPS and diverse clinical outcomes in BTC by systematically analyzing relevant studies. METHODS: A systematic search approach was used to look for eligible

Unresectable biliary tract cancer: Current and future systemic therapy.

For decades, treatment of advanced biliary tract cancer (BTC) was confined to the use of chemotherapy. In recent years however, the number of therapeutic options available for patients with unresectable BTC have drastically increased, with immunotherapy and targeted treatment gradually joining the ranks of guideline-recommended treatment regimens. The aim of the present review is to summarise the current knowledge on unresectable BTC focusing on epidemiology, anatomical distribution and current strategies for systemic treatment. We further outline ongoing clinical trials and provide an outlook

Dysplasia and Early Carcinoma of the Gallbladder and Bile Ducts: Terminology, Classification, and Significance.

Most precursor lesions and early cancerous changes in the gallbladder and bile ducts present as clinically/grossly inapparent lesions. Low-grade dysplasia is difficult to define and clinically inconsequential by itself; however, extra sampling is required to exclude accompanying significant lesions. For high-grade dysplasia ('carcinoma in situ'), a complete sampling is necessary to rule out invasion. Tumoral intramucosal neoplasms (ie, intracholecystic and intraductal neoplasia) form radiologically/grossly visible masses, and they account for (present in the background of) about 5% to 10% of i

Hepatic granulomas: a 15-year experience in the Royal Adelaide Hospital.

The clinical and pathological associations of hepatic granulomas in patients who presented to the Royal Adelaide Hospital between January 1, 1968 and February 29, 1984 were reviewed retrospectively. Cases of primary biliary cirrhosis were excluded. Of 59 patients with hepatic granulomas, clear associations with diseases were identified in 42 (71%) patients. These were sarcoidosis (seven cases), chronic liver disease (12 cases), biliary tract disease (three cases), tuberculosis (four cases), Q-fever (three cases), other infections (four cases), drug hypersensitivity (four cases) and neoplasms (

Epidemiologic Trends and Factors Associated With Overall Survival for Patients With Hepatobiliary Neuroendocrine Neoplasms in the United States.

BACKGROUND: Neuroendocrine neoplasms (NENs) constitute a heterogeneous group of rare tumors that most commonly originate from the gastrointestinal tract and lungs, with a consistently rising global incidence over recent decades. Hepatobiliary neuroendocrine neoplasms (HB-NENs), defined as primary NENs arising in the liver and biliary tract, are exceedingly uncommon, accounting for less than 0.5% of all NENs. Owing to their low incidence, atypical clinical presentations, and the paucity of large-scale datasets, no distinct World Health Organization (WHO) classification criteria have yet been es
