Validation of glucose and lactate in cerebrospinal fluid (CSF) on a Radiometer blood gas analyzer ABL90 Flex plus.

PURPOSE: Rapid determination of cerebrospinal fluid (CSF) glucose and lactate is required by emergency rooms and intensive care units. Long turnaround time (TAT) on test results negatively impacts timely diagnosis and treatment of neurological infections like meningitis. METHODS: The CSF glucose and lactate assays were evaluated on a blood gas analyzer, Radiometer ABL90 Flex Plus. Linearity, limit of quantitation (LOQ), and precision were determined using fresh and spiked patient CSF samples. Fifty-four fresh and 49 frozen CSF samples were used to compare the method againstAbbottArchitectC160

Things We Do for No Reason™: Arterial blood gas testing to screen for hypercarbic respiratory failure.

Venous blood gases (VBGs) are safe and effective at screening for hypercarbic respiratory failure. Multiple studies have validated that a VBG with a PvCO2 less than 45 reliably rules out hypercarbia. The use of VBGs for the initial screening of hypercarbia when hypercarbic respiratory failure is suspected can reduce the overall use of arterial blood gases which are more painful and take more time and expertise to perform.

Characteristics of pulse oximetry and arterial blood gas in patients with fibrotic interstitial lung disease.

BACKGROUND: Fibrotic interstitial lung disease (ILD) is frequently associated with abnormal oxygenation; however, little is known about the accuracy of oxygen saturation by pulse oximetry (SpO2) compared with arterial blood gas (ABG) saturation (SaO2), the factors that influence the partial pressure of carbon dioxide (PaCO2) and the impact of PaCO2 on outcomes in patients with fibrotic ILD. STUDY DESIGN AND METHODS: Patients with fibrotic ILD enrolled in a large prospective registry with a room air ABG were included. Prespecified analyses included testing the correlation between SaO2 and SpO2

[Research progress of diagnostic and therapeutic value of carbon dioxide-derived indicators in patients with sepsis].

Effectively assessing oxygen delivery and demand is one of the key targets for fluid resuscitation in sepsis. Clinical signs and symptoms, blood lactic acid levels, and mixed venous oxygen saturation (SvO2) or central venous oxygen saturation (ScvO2) all have their limitations. In recent years, these limitations have been overcome through the use of derived indicators from carbon dioxide (CO2) such as mixed veno-arterial carbon dioxide partial pressure difference (Pv-aCO2, PCO2 gap, or ΔPCO2), the ratio of mixed veno-arterial carbon dioxide partial pressure difference to arterial-mixed venous

Evaluation of Nasal Oxygen Administration at Various Flow Rates and Concentrations in Conscious, Standing Adult Horses.

This study evaluated the effects of various flow rates and fractions of oxygen on arterial blood gas parameters and on the fraction of inspired oxygen (FIO2) delivered to the distal trachea. Oxygen was administered to 6 healthy, conscious, standing, adult horses via single nasal cannula positioned within the nasopharynx. Three flow rates (5, 15, 30 L/min) and fractions of oxygen (21, 50, 100%) were delivered for 15 minutes, each in a randomized order. FIO2 was measured at the level of the nares and distal trachea. Adverse reactions were not observed with any flow rate. FIO2 (nares and trachea)

Transcutaneous and End-Tidal CO₂ Measurements in Hypoxia and Hyperoxia.

BACKGROUND: Transcutaneous measurement of carbon dioxide (CO₂) has been proposed for physiological monitoring of tactical jet aircrew because in some clinical settings it mirrors arterial CO₂ partial pressure (Paco₂). End-tidal monitoring in laboratory settings is known to give high-fidelity estimates of Paco₂.METHODS: The correspondence between end-tidal (PETco₂) and transcutaneous Pco₂ (tcPco₂) was examined in healthy volunteers under laboratory conditions of hyperoxia and hypoxia. Rest and exercise, skin heating and cooling, hyperventilation, and induced CO₂ retention were employed.RESULTS:

The problems of the clinical and laboratory diagnosis of pulmonary embolism.

Suspecting and diagnosing venous thrombosis and pulmonary embolism remain major clinical problems. For a variety of reasons, including the patient's pre-existing cardiac and pulmonary status and the release of humoral mediators, pulmonary emboli evoke varied responses in different patients. Symptoms and signs of pulmonary embolism are not specific; they depend on the size and the hemodynamic and humoral consequences of the embolus. Conversely, they may not be present at all. Chest radiographic and electrocardiographic findings are nonspecific, and may be most helpful in establishing the presen

Lysine hydrochloride for the control of metabolic alkalosis: a clinical report.

Many physiopathological states can produce metabolic alkalosis that must be promptly corrected as soon as it is dangerous. In our study we report the effectiveness of lysine hydrochloride to correct this condition in patients. This drug lowers the pH, reduces the bicarbonate stores, and leads to normal blood gases.

[Ciaglia blue rhino: a modified technique for percutaneous dilatation tracheostomy. Technique and early clinical results].

OBJECTIVE: Elective tracheostomy has become an established treatment modality in modern intensive care medicine, and the number of percutaneously performed tracheostomies is steadily increasing. The Ciaglia Blue Rhino (CBR) represents another percutaneous technique in which the tracheostoma is formed by one-step dilation. Our study presents the technique itself and the early clinical experiences. METHODS: In 20 adult patients on long-term ventilation, CBR was done. After puncture of the trachea in typical manner, dilation of the tracheostoma was achieved in one single step by means of a curve

Effect of venous hypercarbia and hyperventilation on myocardial contractility in canine haemorrhagic shock.

To study the effect of venous hypercarbia on myocardial contractility, haemorrhagic shock was produced in six healthy mongrel dogs by ex-sanguination of 15 ml of blood/kg body weight every 20 minutes till a loss of 45 ml/kg was achieved. After recording haemodynamic and respiratory parameters, the dogs were hyperventilated by positive pressure ventilation for 30 minutes and haemodynamic and blood gas parameters reassessed. During haemorrhagic shock, mean cardiac output decreased from 4.23 l min to 0.98 l min (p < 0.01), stroke index from 2.25 to 0.35 ml/kg (p < 0.05) and left ventricular strok

Sloping alveolar plateaus of CO2, O2, and intravenously infused C2H2 and CHClF2 in the dog.

To investigate the role of the various mechanisms assumed to contribute to the slope of the alveolar plateau, two test gases exhibiting identical solubility but two-fold differing diffusivity, acetylene (C2H2) and chlorodifluoromethane (Freon 22, CHClF2), dissolved in saline were intravenously infused in 10 anesthetized, paralyzed, artificially ventilated dogs (mean body mass, 18 kg). The partial pressures of C2H2, CHClF2, CO2 and O2 during a constant-flow single-breath washout maneuver were recorded by mass spectrometry and analyzed in terms of slope of the alveolar plateau (phase III) and se

Changes in arterial and transcutaneous oxygen and carbon dioxide tensions during and after voluntary hyperventilation.

The purposes of our study were (1) to investigate whether a 3-min short-term hyperventilation leads to posthyperventilatory hypoxemia and (2) to assess the role of transcutaneous blood gas measurements for monitoring oxygen and carbon dioxide changes during the after the test. In 10 male volunteers arterial and transcutaneous blood gases were measured simultaneously before, during and after a 3-min voluntary hyperventilation maneuver. Baseline arterial PO2 increased from 13.7 +/- 0.4 kPa (103 +/- 3 mm Hg) to 18.6 +/- 0.3 kPa (139 +/- 2.3 mm Hg; p < 0.005 compared to baseline) during hyperventi

Effects of routine hyperventilation on PCO2 and PO2 in normal subjects: implications for EEG interpretations.

There are few data in the EEG literature describing the time course of hyperventilation-(HV) induced changes in blood gases, despite this being a routine activating procedure. We studied changes in blood gases and EEG in nine normal adult subjects before, during, and after HV. The mean PCO2 fell 18 mm Hg from the baseline during HV and recovered in 7 min. The mean PO2 rose 7 mmHg during HV and fell to 25 mm Hg below baseline 5 min after HV. The PCO2 recovery period is longer than is usually assumed in clinical EEG. The PO2 fall to a nadir at 5 min after the end of HV suggests that close attent

Reevaluation of the disulfiram-alcohol reaction in man.

The intensity of the disulfiram-alcohol reaction is found to depend on the blood acetaldehyde level, a disulfiram-induced predisposition reflected by alkalosis, and an individual predisposition reflected by dopamine-beta-hydroxylase activity.
