Endoscopic Lung Volume Reduction with Autologous Blood: What is the Evidence?

Biological lung volume reduction (BioLVR) is a novel and low-cost endobronchial treatment method aimed at reducing the volume of the target lung lobe using biological agents, including fibrin-based hydrogel, fibrinogen, and autologous blood (AB) with thrombin. BioLVR induces local inflammation, resulting in acute airway obstruction, resorption atelectasis, fibrosis, and finally tissue remodeling by contraction of the target lobe and reduction in the lung volume, similar to the application of hot water vapor and foam. In addition, patients with severe impairment in lung function and quality of

Detailed Analysis of the Characteristics of Sample Volume in Blood Culture Bottles.

Blood volume is the most important variable for the detection of microorganisms in blood cultures (BCs). Most standards recommend 40 to 60 ml blood, collected in several BC bottles filled up to 10 ml. We measured blood volume in individual BC bottles and analyzed the associations of hospital, bottle type, day of the week, daily sampling time, and age and sex of the patient with sampling volume and BC result. The variation in blood volume per BC bottle was analyzed in a mixed linear model using hospital, bottle type, weekday, sampling time, age, and sex as fixed factors and patient identificati…

A prospective controlled trial of the optimal volume for neonatal blood cultures.

BACKGROUND: Bacteremia is a frequent complication in neonatal intensive care units. Blood cultures are the standard for the diagnosis. It is a common practice to draw small blood volumes for culture from neonates in order to prevent anemia; however, this might compromise the test sensitivity. We examined whether using 1 mL of blood in a single aerobic bottle would improve the culture yield compared with our current practice of obtaining 2 samples of 0.5 mL of blood (aerobic and anaerobic bottles). METHODS: A prospective controlled study was conducted between December 2009 and September 2010 a…

Effects of acute blood volume expansion on respiratory mechanics in the rat.

BACKGROUNDS: The effects of acute blood volume expansion (BVE) on the respiratory mechanics of normal animals have been not extensively studied. The subject is of both theoretical and practical interest since BVE is a frequent medical intervention, and the associated increase in cardiac output may occur in different physiopathological situations. OBJECTIVES: To describe the changes in the parameters of respiratory mechanics occurring as an effect of acute BVE and the related increase in cardiac output. METHODS: We applied the end-inflation occlusion method in normal, positive pressure-ventil…

Influence of ventilation mode on neonatal cerebral blood flow and volume.

BACKGROUND: Cerebral hemodynamics is supposed to be influenced by the different ventilation approach. Ventilation support can be classified as non-invasive (N-CPAP) or invasive (SIMV and HFV), the last known to induce endotrauma. Our aim was the non-invasive NIRS assessment of neonatal absolute cerebral blood flow (CBF) and relative cerebral blood volume changes (DeltaCBV) during synchronized intermittent ventilation (SIMV), or high frequency ventilation (HFV) and nasal continuous positive airways pressure (CPAP). METHODS: An observational study in a tertiary referral NICU. CBF and DeltaCBV c…

Control of neurohypophysial hormone secretion, blood osmolality and volume in pregnancy.

In pregnancy, blood volume increases greatly and plasma osmolality is reduced, due to mild hyponatraemia despite sodium retention. In rats, both vasopressin and oxytocin neurones in the supraoptic nucleus are osmosensitive and have contrasting roles in these adaptations. Increased vasopressin secretion stimulates water retention by renal actions, while oxytocin is natriuretic, partly by stimulating cardiac atrial natriuretic peptide (ANP) secretion. In pregnancy, relaxin from the corpora lutea, acting via the lamina terminalis in the presence of pregnancy levels of oestrogen and progesterone, …

Haematological responses of repeated large volume blood collection in the horse.

The haematological response of regular, repeated blood harvesting was investigated in 40 Thoroughbred and non-Thoroughbred horses that donat … de that the removal of approximately 8 litres of blood (approximately 16 ml kg(-1)or 20 per cent of blood volume for a 500 kg horse) from blood donor horses every 3 weeks allows time for adequate recovery of haematological variables and does not result in adverse haematological changes.

Apnoea of prematurity and changes in cerebral oxygenation and cerebral blood volume.

The aim of this study was to analyse the behaviour of cerebral oxygenation and cerebral blood volume (CBV) in preterm infants during apnoea by means of near-infrared spectroscopy (NIRS). The sum of oxygenated and deoxygenated haemoglobin, the total cerebral haemoglobin (Hbtot) corresponds to CBV, whereas the difference of oxygenated minus deoxygenated haemoglobin (HbD) represents a value for cerebral oxygenation. During 2 hours of daytime sleep, 72 polygraphic tracings (including NIRS) were done in 58 premature infants. The main criteria for study entry were clinically evident episodes of apno…

Volume effect of Ringer's solution in the blood during general anaesthesia.

Ringer's solution expands blood volume by between 20% and 25% in awake volunteers, but little is known about its volume effect during general anaesthesia. In 35 men with a mean age of 70 years, the power of various intra-operative factors to change blood volume as estimated by the haemoglobin dilution method were analysed by multiple linear regression every 10 min during transurethral resection of the prostate performed under enflurane anaesthesia. The results show that blood volume increased by 60% of the infused Ringer's solution and by 35% of the irrigating fluid absorbed directly into the …

Regulation of blood volume-implications for cardiovascular pathophysiology in sleep apnoea.

Volume homeostasis plays an important role in the regulation of the cardiovascular system and maintenance of haemodynamics. The heart-kidney axis represents the central part of the volume regulating system: the heart senses changes in the volume status and influences renal function via neural and humoral pathways in order to compensate for disturbances in volume homeostasis and prevent under- or overfilling of the heart. An undisturbed circadian rhythmicity of volume homeostasis, renal function and secretory pattern of volume regulating hormones may be of physiological importance. Disturbances

Modulation of erythropoietin formation by changes in blood volume in conscious dogs.

1. A possible influence of the filling of the circulatory system on the plasma concentration of erythropoietin, which is the major regulator of erythrocyte formation, was investigated in conscious dogs. 2. Over an experimental period of 5 h, the animals were subjected to either haemorrhage (hypovolaemia), blood volume expansion (hypervolaemia), or exchange transfusion of blood with dextran (isovolaemic anaemia). 3. A reduction of blood volume by 20% induced by haemorrhage increased plasma erythropoietin levels approximately 1.5-fold in the absence of significant changes in haematocrit. 4. An e…

Effects of detraining on cardiovascular responses to exercise: role of blood volume.

In this study we determined whether the decline in exercise stroke volume (SV) observed when endurance-trained men stop training for a few weeks is associated with a reduced blood volume. Additionally, we determined the extent to which cardiovascular function could be restored in detrained individuals by expanding blood volume to a similar level as when trained. Maximal O2 uptake (VO2max) was determined, and cardiac output (CO2 rebreathing) was measured during upright cycling at 50-60% VO2max in eight endurance-trained men before and after 2-4 wk of inactivity. Detraining produced a 9% decline…

Urinary and hemodynamic responses to blood volume changes in fetal sheep.

Because of the potential importance of the fetal kidney in regulating blood volume and amniotic fluid volume, and their composition, we studied renal and cardiovascular responses to changes in blood volume in 12 chronically-catheterized fetal sheep at 110-135 days gestation. During the hour following volume expansion with 50 ml of maternal whole blood, blood pressure rose by 14%, cardiac output by 11%, renal blood flow by 68% and urine output by 36%. Packed red cell volume rose from 31.3 to 34.0%, suggesting a net fluid loss from the circulation through the kidneys, as well as through peripher…

Pulmonary mean-transit-time blood volumes in anesthetized dogs.

Pulmonary mean-transit-time volume (PBV) of 12 anesthetized dogs was measured repeatedly by using pairs of dye-dilution curves recorded from the pulmonary artery (PA) and left atrium (LA) after right atrial injections of indocyanine green. In 345 pairs, the areas of the extrapolated LA curves exceeded those of the PA curves by 8% (sd, 14%), indicating that delayed color development of the dye-blood mixture is not a serious source of error in standard dye-dilution techniques. In eight dogs, measurement every 3–5 min before and after 2- or 5-min infusions of morphine (1 mg/kg) revealed no change
