[CURRENT CRITERIA FOR THE DIAGNOSIS OF MYELOPROLIFERATIVE NEOPLASMS - WHAT IS IMPORTANT TO KNOW?].

The understanding of the basic molecular mechanisms of myeloproliferative neoplasms in the last few years led to updating of their diagnostic criteria in the recent classification of myeloid and lymphoid neoplasms by the WHO, which was published in 2017. The major changes relating to the diagnosis of myeloproliferative neoplasms include lowering of the hemoglobin threshold and mandatory bone marrow biopsy as major criteria for the diagnosis of polycythemia vera, as well as adding acquired mutation in either CALR or MPL in addition to the common JAK2V617F mutation as a major criterion for diagn

The use of bone-modifying agents in multiple myeloma.

Multiple myeloma is a hematological neoplasm characterized by abnormal proliferation of plasma cells in the bone marrow and is usually associated with increased bone pain and skeletal-related events such as pathological fracture and/or spinal cord compression. Myeloma bone disease results in changes in the bone-marrow microenvironment evidenced by increased osteoclastic activity and/or decreased osteoblastic activity, which negatively affect quality of life. Treatment of myeloma bone disease includes bisphosphonates or denosumab (bone-modifying agents). These agents do not induce the formation

Atypical chronic myeloid leukemia as defined in the WHO classification is a JAK2 V617F negative neoplasm.

Atypical chronic myeloid leukemia (aCML) as defined by the WHO classification is a rare hematopoietic stem cell disorder, which shows both myeloproliferative as well as myelodysplastic features. Because of the presence of neutrophilic leukocytosis, aCML may resemble chronic myelogenous leukemia. However, in contrast with the latter, aCML lacks a Philadelphia chromosome or the BCR/ABL fusion gene. The molecular pathogenesis of aCML and its relationship to other myeloproliferative neoplasms is unknown. To clarify these points, the presence of JAK2 V617F was examined by a retrospective analysis o

Spectrum of Bone Marrow Pathological Findings in Patients Undergoing Bone Marrow Biopsy: A Cross-Sectional Study.

Introduction Bone marrow biopsy is a diagnostic cornerstone in the evaluation of unexplained cytopenias and suspected haematological neoplasms; however, diagnostic frequencies vary widely across populations. This study describes the spectrum of bone marrow pathology recorded in a single-centre registry and explores, as a secondary exploratory aim, whether it varies by sex and age. Methods  This retrospective cross-sectional study analysed 181 bone marrow examination records, which constituted the unit of analysis; no patient identifiers were available to confirm unique-patient status. Diagnose

[Bone marrow biopsy: processing and use of molecular techniques].

The rapid technological development in diagnostic pathology, especially of immunohistochemical and molecular techniques, also has a significant impact on diagnostic procedures for the evaluation of bone marrow trephine biopsies. The necessity for optimal morphology, combined with preservation of tissue antigens and nucleic acids on one hand and the wish for short turnaround times on the other hand require careful planning of the workflow for fixation, decalcification and embedding of trephines. Although any kind of bone marrow processing has its advantages and disadvantages, formalin fixation

Bone marrow biopsy: interpretive guidelines for the surgical pathologist.

Ideally, the bone marrow core biopsy should be reviewed with knowledge of the clinical history, complete blood count, and findings in the peripheral blood and bone marrow aspirate smears. However, for a variety of reasons, the pathologist may receive the core biopsy and aspirate clot section without all of this information. Although this approach is not optimal, a great deal of valuable information can be generated from these specimens. Over the past 20 years, there has been considerable progress in the fields of flow cytometric analysis, immunohistochemistry, and molecular diagnostic studies

Chinese Patients With BCR::ABL1-Negative Myeloproliferative Neoplasms: Immunophenotype of Myeloid Monocytes.

INTRODUCTION: The main terms for typical BCR::ABL1-negative myeloproliferative neoplasms (MPNs) are essential thrombocythemia (ET), polycythemia vera (PV), and primary myelofibrosis (PMF). Monocytes in MPN patients are involved in their inflammation and form an important part of the pathophysiology of MPN patients. METHODS: We used flow cytometry to study the immunophenotype of bone marrow monocytes from MPN patients (N = 118) and to correlate it with clinical parameters (including genetics, pathology, blood counts, personal information). RESULTS: The results showed that bone marrow monocyte

Acute myelomonocytic leukemia with histologic features resembling sarcomatoid carcinoma in bone marrow.

We report a case of primary acute myelomonocytic leukemia involving the bone marrow that resembled sarcomatoid carcinoma. The neoplastic cells in bone marrow biopsy specimens formed cohesive-appearing clusters and cords separated by an immature fibroblastic proliferation and myxoid stroma. Blasts in the bone marrow aspirate smears formed clusters and sheets, and a subset of blasts exhibited erythrophagocytosis. Dysgranulopoiesis was also present. Lineage was confirmed by immunohistochemical analysis of formalin-fixed, paraffin-embedded tissue. The tumor cells showed strong reactivity for lysoz

Plasma Cell Neoplasm Mimicking Metastatic Bone Disease in a Breast Cancer Survivor: A Case Report Highlighting the Role of Serum Protein Electrophoresis.

Breast cancer, a prevalent solid tumor, is the most common cancer among women worldwide. Hematological malignancies, such as acute myeloid leukemia, myelodysplastic syndrome, and acute lymphoblastic leukemia, are more frequent in breast cancer survivors, while plasma cell neoplasms are less common. We report the case of a 44-year-old woman with breast cancer who underwent neoadjuvant chemotherapy and surgery, presenting one year later with bone lesions and anemia. Initially suspected to be metastatic bone disease, the findings of elevated serum total protein and globulins prompted serum protei

Reconstructing skeletal homeostasis through allogeneic hematopoietic stem cell transplantation in myelofibrosis.

Myeloproliferative neoplasm-associated myelofibrosis is a clonal stem cell process characterized by pronounced bone marrow fibrosis associated with extramedullary hematopoiesis and splenomegaly. Allogeneic hematopoietic stem cell transplantation (allo-HSCT) represents the only curative treatment leading to bone marrow fibrosis regression. Here we provide an in-depth skeletal characterization of myelofibrosis patients before and after allo-HSCT utilizing clinical high-resolution imaging, laboratory analyses, and bone biopsy studies. Despite unimpaired bone microarchitecture at peripheral skelet

Mast cells as a therapeutic target in myeloproliferative neoplasms.

Mast cells have been implicated as mediators of bone marrow fibrosis and pruritus in myeloproliferative neoplasms (MPNs) with JAK2V617F or calreticulin mutations. We hypothesize that potent KIT inhibitors, already in clinical use for systemic mastocytosis, have therapeutic potential for the treatment of MPNs by directly targeting mast cells.

Three myeloproliferative neoplasms: An overview.

A group of rare hematologic cancers, myeloproliferative neoplasms (MPNs) evolve when bone marrow dysfunction causes overproduction of one or more blood cell types. This article explores the diagnosis, treatment, and nursing care of patients diagnosed with one of three classic MPNs: essential thrombocythemia, polycythemia vera, and primary myelofibrosis.

Mechanobiology and Primary Cilium in the Pathophysiology of Bone Marrow Myeloproliferative Diseases.

Philadelphia-Negative Myeloproliferative neoplasms (MPNs) are a diverse group of blood cancers leading to excessive production of mature blood cells. These chronic diseases, including polycythemia vera (PV), essential thrombocythemia (ET), and primary myelofibrosis (PMF), can significantly impact patient quality of life and are still incurable in the vast majority of the cases. This review examines the mechanobiology within a bone marrow niche, emphasizing the role of mechanical cues and the primary cilium in the pathophysiology of MPNs. It discusses the influence of extracellular matrix compo

SRSF2 mutation reduces polycythemia and impairs hematopoietic progenitor functions in JAK2V617F-driven myeloproliferative neoplasm.

SRSF2 mutations are found in association with JAK2V617F in myeloproliferative neoplasms (MPN), most frequently in myelofibrosis (MF). However, the contribution of SRSF2 mutation in JAK2V617F-driven MPN remains elusive. To investigate the consequences of SRSF2P95H and JAK2V617F mutations in MPN, we generated Cre-inducible Srsf2P95H/+Jak2V617F/+ knock-in mice. We show that co-expression of Srsf2P95H mutant reduced red blood cell, neutrophil, and platelet counts, attenuated splenomegaly but did not induce bone marrow fibrosis in Jak2V617F/+ mice. Furthermore, co-expression of Srsf2P95H diminished

Low-grade MALT lymphoma involving multiple mucosal sites and bone marrow.

Mucosa-associated lymphoid tissue (MALT) lymphomas are indolent neoplasms which tend to remain localized for a long time before spreading. We describe here the case of a 36-year-old woman with a low-grade MALT lymphoma involving the lung, stomach, lingual tonsil, and bone marrow at the time of diagnosis. The clonal origin of the pulmonary and bone marrow neoplastic infiltrates was assessed by means of gene rearrangement analysis. All of the involved sites were infiltrated by centrocyte- and monocytoid-like cells expressing the B-cell-associated antigens CD19 and CD20 and showed IgM lambda chai

Synchronous Presentation of Chronic Myelomonocytic Leukemia and Multiple Myeloma in a Treatment-Naïve Patient.

BackgroundChronic myelomonocytic leukemia (CMML) is a clonal myelodysplastic/myeloproliferative neoplasm characterized by persistent peripheral blood monocytosis and dysplastic bone marrow morphology. Multiple myeloma (MM) is a plasma cell malignancy defined by clonal plasma cell proliferation and myeloma-defining events. The synchronous occurrence of CMML and MM is exceptionally rare, particularly in treatment-naïve and relatively young patients.Case presentationWe present a 42-year-old-male presenting with progressive fatigue and leukocytosis. Laboratory evaluation revealed marked absolute m

Myeloid sarcoma presenting as a colon polyp and harbinger of chronic myelogenous leukemia.

Myeloid sarcoma, also known as granulocytic sarcoma or chloroma is an unusual accumulation of malignant myeloid precursor cells in an extramedullary site, which disrupts the normal architecture of the involved tissue. It is known to occur more commonly in patients with acute myelogenous leukemia and less commonly in those with myelodysplastic syndrome and myeloproliferative neoplasm, such as chronic myelogenous leukemia. The most common sites of involvement include bone, skin and lymph nodes. However, rare cases have been reported in the gastrointestinal tract, genitourinary tract, or breast.

Smoothened Inhibitor, PF-04449913 Inhibits the Development of Myelofibrosis in a JAK2V617F Transgenic Mouse Model by Reducing TGF-β and MAPK Signaling Pathways.

Treatment of JAK2V617F driven myeloproliferative neoplasms (MPNs) with Ruxolitinib (JAK inhibitor, JAKi) has shown limited disease-modifying benefits and has led to the search for other pathways as potential therapeutic targets for this disease. We investigated the effects of Smoothened inhibition (SMOi) using the small-molecule inhibitor PF-04449913 (PF-13) in a JAK2V617F transgenic mouse model that recapitulates many of the phenotypes of MPNs including bone marrow fibrosis and splenomegaly. We show both that hedgehog (Hh) signaling pathway is activated in JAK2V617F cells and that SMOi reduce

Report of a Rare Case of Intracranial Extramedullary Hematopoiesis Mimicking a Brain Tumor.

Failure of the bone marrow to maintain adequate blood cell production to match blood metabolic demand incites the production of cell lines outside the bone marrow, which is known as extramedullary hematopoiesis. Herein, we are reporting an 80-year-old male patient who presented with two weeks of worsening headaches and behavioral changes. Labs showed thrombocytosis and imaging showed a large right-sided hemorrhagic brain mass. No evidence of malignancy was seen elsewhere. Brain mass biopsy showed intracranial extramedullary hematopoiesis (IEMH) and bone marrow biopsy confirmed the diagnosis of
