BACKGROUND: Brain abscess remains a serious intracranial infection associated with substantial mortality and neurological disability, despite advances in neuroimaging, neurosurgery, and antimicrobial therapy. Recent data from the Middle East are limited, and the prognostic impact of readily available clinical and radiologic variables in this region remains poorly defined.
METHODS: We retrospectively analyzed data from consecutive patients with radiologically and clinically confirmed brain abscesses treated at two tertiary centers in Jeddah, Saudi Arabia, between 2010 and 2020. Demographic, cl…

Brain abscess is rarely caused by coinfection with different pathogens. A middle-aged immunocompetent woman developed right-sided focal motor seizures with Todd's palsy. Brain imaging identified a frontal ring-enhancing lesion for which she started antitubercular therapy and corticosteroids. However, on tapering the corticosteroids, she developed a right-sided hemiparesis with increased lesion size. Excision biopsy identified coinfection with Mycobacterium tuberculosis and Aspergillus spp She recovered well with antitubercular and antifungal agents. Coinfections with multiple pathogens pose di…

A brain abscess (BA) is a purulent infection of the central nervous system and can be associated with dental procedures. The paper presents a case of a patient diagnosed with an odontogenic BA. On admission, the patient was awake, alert, and oriented (Glasgow Coma Scale 15). The patient reported undergoing excision and drainage of an abscess in the submandibular area three years earlier. An MRI of the head showed a mass lesion (34x19x25 mm) located in the right parieto-occipital region, and an abscess was suspected. During the procedure, puncture and evacuation of the contents of the abscess i…

We present the first reported case of a pediatric central nervous system (CNS) abscess caused by Actinomyces israelii in the context of advanced HIV disease. A three-year-old girl from São Tomé and Príncipe presented with progressive neurological deficits, including gait instability and language delay. Brain MRI revealed a right temporal lobulated lesion with surrounding edema and mass effect. Chronic superior sagittal sinus thrombosis and hydrocephalus were also identified. Neurosurgical intervention included ventriculocisternostomy and microsurgical resection of the lesion, which revealed a

A 63-year-old right-handed man was admitted to our hospital after sustaining a traumatic right-sided subdural hematoma, subarachnoid hemorrhage, and temporal lobe hemorrhagic contusion. He was managed non-operatively and discharged without any neurologic deficits. Two weeks later he presented with worsening headaches and altered sensorium. Imaging studies showed that the patient had developed a cerebral abscess at the site of his intracerebral hemorrhage. The abscess was surgically resected, and the patient was treated with antibiotics with complete resolution of symptoms. This case illustrate

Nocardia paucivorans brain abscesses are unusual in humans. Sixteen cases of this infection have been reported in the world medical literature. There is precise clinical information available from nine patients. All of these patients recovered or were cured from their brain disease with long-term antimicrobial treatment. Surgical drainage was performed in four patients.

BACKGROUND: Central nervous system nocardial infection is a rarely reported disease that usually affects patients with predisposing and debilitating conditions but also the immunocompetent population. The most common variant affecting the brain is Nocardia farcinica. Management of brain nocardiosis is troublesome and requires consideration of the severity of the underlying systemic disease, the difficulties in identifying the bacterium, and the frequent delay in initiating adequate therapy.
CASE DESCRIPTION: We present 3 cases of N. farcinica brain abscess (single, multiloculated, and multifo…

We report a case of brain abscess following the percutaneous treatment for trigeminal neuralgia. This procedure envisages the access with a needle into the middle cranial fossa through the oral cavity. Thus, in this case, the bacterial infection can be more likely ascribed to the possible contamination of the needle inside the oral cavity rather than to other frequent and more controllable causes of infection like an imperfect sterilization of surgical instruments or an inadequate antiseptic preparation of both operator's hands and patient's skin. The subsequent brain abscess was treated with …

Toxocariasis in man is associated with three syndromes which are visceral larva migrans, ocular larva migrans and covert toxocariasis. Although neurotoxocariasis is defined as the fourth syndrome of toxocariasis, it is usually considered as a neurological disease which is usually concomitant with visceral larva migrans. In this report, a case of brain abscess caused by toxocariasis was presented. A 56 years-old female patient was admitted to our hospital with headache, pain referring to right side of her face and teeth, numbness of forth and fifth finger of her right hand. Cranial diffusion we…

Nocardia brain abscesses typically occur in immunocompromised patients. Most cases of nocardiosis are caused by the Nocardia asteroides complex and Nocardia brasiliensis. Here, we present a patient with a Nocardia abscessus brain abscess. The diagnosis was confirmed by DNA sequencing, and the organism was susceptible to linezolid, clarithromycin, ceftriaxone, imipenem, tobramycin, amikacin, minocycline and sulfamethoxazole. The patient was successfully treated medically in combination with surgical excision.

A case of a 38-year-old male farmer with a brain abscess caused by Cladophialophora bantiana is described. He had a 2 year history of non-insulin-dependent diabetes and myelodysplastic syndrome. A cranial computed tomography scan demonstrated a hypodense ring lesion with peripheral oedema and a midline shift in the left frontal lobe. A darkly pigmented mould was isolated from the brain abscess. The isolate was identified as C. bantiana based on its morphological features and DNA sequence analysis. The patient was unresponsive to burr hole aspiration and irrigation, as well as liposomal amphote…

Persistent left superior vena cava (PLSVC) is the most common congenital cardiac anomaly occurring in around 0.4% of the general population, and 1.3-4.5% in those with additional cardiac defects. Eighty-two percent of PLSVC coexists with a right superior vena cava. Usually PLSVC is asymptomatic, but can cause difficulties during central venous cannulation from the left side. There are a few cases of brain abscess in the literature associated with PLSVC. Here, we describe a patient of brain abscess with PLSVC in the intensive care unit of King Faisal Hospital, Makkah, Kingdom of Saudi Arabia.

A 63-year-old man with a history of chronic lymphocytic leukemia and a prosthetic aortic valve was hospitalized because of a mastoiditis, complicated by meningitis and epileptic seizures. Two weeks later he developed a lesion in the right temporal lobe. A brain abscess was suspected. However after treatment his clinical condition failed to improve. 99mTc-Tetrofosmin brain SPECT was performed and revealed substantially increased tracer uptake. Due to the patient's clinical deterioration, surgery was considered most appropriate. Histopathology established the diagnosis of glioblastoma multiforme…

Nocardial infections in an immunocompromised host have been increasingly reported. Nocardial brain abscess, the most common presentation of nocardiosis in the central nervous system, is associated with a high mortality rate because of its delayed diagnosis and its unresponsiveness to the usual antibiotic therapy. We report four patients who experienced a long-term cure of nocardial brain abscess due to treatment by a combination of surgery and postoperative antibiotic therapy; 1 man and 3 women, ages ranging from 43 to 67 years old. Two patients were associated with systemic lupus erythematosu…

Nocardial brain abscess is rare and accounts for about 1-2% of all cerebral abscesses. However, the early detection and treatment of it is very important because the mortality of it is three times higher than that of other bacterial brain abscesses. We report a case of nocardial brain abscess in a renal transplant recipient. Enhanced brain CT and MR scans showed multiple conglomerated ring-enhancing lesions with budding appearance and distinct surrounding edema.

Though positive visual symptoms can be psychological in nature, or can result from a perceptive or anxious patients recognizing optical principals in the eye itself, this case illustrates how a thorough history is required to delineate those rarer signs which accompany serious macular or neuro-ophthalmic pathology.
