Validation of a brain-damage vs. schizophrenia MMPI key.

Since neuropsychologic tests largely have failed to separate schizophrenic from brain-damaged patients, an MMPI key was constructed to make such a separation. When cross-validated, this key provided a 76% correct separation and achi2 significant beyond the .001 level for 50 schizophrenic and 50 brain-damaged profiles. Neuropsychologic tests should be used to separate normals from brain-damaged patients and the MMPI to separate schizophrenic from brain-damaged patients.

Halstead-Reitan Test Battery: a problem of differential diagnosis.

The Halstead-Reitan Test Battery is one of the most widely recognized neuropsychological test batteries. Many claims have been made as to its validity. Despite these claims, doubts persist. A critical review of the literature shows that the battery can separate brain-damaged patients from normal patients, general medical patients, and patients with certain psychiatric disorders. However, the battery cannot separate brain-damaged patients as a group from schizophrenics as a group, though in individual cases there may exist pathognomonic signs indicating brain damage. The impairment index, as a

Neonatal encephalopathies as classified by EEG-sleep criteria: severity and timing based on clinical/pathologic correlations.

Neonatal encephalopathies can be characterized in functional terms using electroencephalography. Severity of an encephalopathic state can also be estimated by electrographic interpretation independent of the time of disease process onset. Moderately or markedly abnormal electroencephalographic patterns on serial studies are highly correlated with neurologic sequelae in survivors. Electroencephalography is rarely pathognomonic or specific in determining when a condition initially occurred. However, electroencephalographic abnormalities are associated with different clinical situations, and brai

The influence of growth retardation on perinatal hypoxic-ischemic brain damage.

The effect of growth retardation on the extent of brain damage produced by hypoxia-ischemia was assessed in immature rats. Newborn rats were raised in litters of 6 or 14 pups from day 2 to 7. On postnatal day 7, those immature rats raised in litters of 14 weighed 18% less than animals raised in litters of 6 (P less than 0.001). They then were subjected to cerebral hypoxia-ischemia by unilateral common carotid artery ligation followed by 3 h of exposure to 8% oxygen-92% nitrogen at 37 degrees C. Upon return to their dams, all litters were culled to 6 pups. At 30 days of age, the animals underwe

[Hypoxic-ischemic brain damage in the newborn].

The condition referred to as "birth asphyxia" occurs as the result of a hypoxic-ischaemic insult during the process of labour and delivery. There is no standard clinical definition for birth asphyxia, but its incidence in term infants has been differently reported to be between 2.9 and 9.0 cases per 1000 deliveries. In term infants the risk of death appeared to be closely related to the duration that the Apgar score is severely depressed. The best predictor of disability in surviving infants is abnormal neurological behavior in the neonatal period referred to as hypoxic-ischaemic encephalopath

MRI of fetal acquired brain lesions.

Acquired fetal brain damage is suspected in cases of destruction of previously normally formed tissue, the primary cause of which is hypoxia. Fetal brain damage may occur as a consequence of acute or chronic maternal diseases, with acute diseases causing impairment of oxygen delivery to the fetal brain, and chronic diseases interfering with normal, placental development. Infections, metabolic diseases, feto-fetal transfusion syndrome, toxic agents, mechanical traumatic events, iatrogenic accidents, and space-occupying lesions may also qualify as pathologic conditions that initiate intrauterine

A systematic review of neuroimaging for cerebral palsy.

The American Academy of Neurology now recommends that all cases of cerebral palsy of unknown origin undergo neuroimaging. Controversy surrounds this recommendation because of concerns about the adequacy of the supporting evidence. This article reviews the evidence provided by magnetic resonance imaging (MRI) and computed tomography (CT) imaging studies in cerebral palsy and discusses the potential benefits of imaging, techniques in current use, and future directions, with a focus on improving etiologic understanding. Most (83%) children with cerebral palsy have abnormal neuroradiological findi

Frontal lobe lesions, diffuse damage, and neuropsychological functioning in traumatic brain-injured patients.

This study quantified lesion volume in relation to damage location and executive functioning in traumatic brain-injured (TBI) patients. Magnetic resonance (MR) scans of 68 TBI patients were analyzed by taking volumetric measures of lesion sites. Patients were grouped according to the presence/absence of frontal lobe lesions. Measures of frontal lesion volume were studied as predictors for outcome on designated tests of executive functioning (Halstead Category Test and Wisconsin Card Sorting Test). Results showed no significant differences in level of deficit between groups. In addition, no sig

Correlates of left-handedness in a large sample of schizophrenic patients.

Questionnaire data concerning hand preference from the parents of 686 schizophrenic patients were used to test predictions relevant to neurodevelopmental models of schizophrenia. In line with predictions, left-handed patients (n = 94) had a more frequent history of difficult birth, more childhood cognitive and behavioural abnormalities and more persistent auditory hallucinations than did right-handed patients.

Time- and temperature-dependent forebrain ischemic damage in Mongolian gerbils.

The influence of transient bilateral carotid artery occlusion time, core body temperature regulation and post-operative survival time was examined on delayed neuronal death in the hippocampal subregions of Mongolian gerbils. A simple and rapid histologically based scoring system was used. A carotid artery occlusion time of 5 min with stabile core body temperature at 37.5-38.0 degrees C and a post-operative survival time of 4 days was sufficient to induce a reproducible and almost total neuronal damage in the hippocampal subfields. Furthermore, the use of gerbils with ages varying from 50 to 10

Neurology of developmental dyslexia.

Developmental dyslexia has until recently been considered to belong solely in the domain of educational psychology. With the advent of better theories on language and reading, and better methods for assessing the structure and function of living human brains and for determining genetic transmission, dyslexia is now poised to become a focal concern of cognitive neuroscience and genetic research. Recent findings from neuroanatomy, neurophysiology, neuropsychology, and genetics research are reviewed.

Associative agnosias and the functions of the left hemisphere.

Associative agnosias are traditionally regarded as perceptual, and ideational apraxia as motor, deficits, but they can be understood as amnesias for generic knowledge, caused by bilateral or unilateral left-hemispheric cortical lesions. Current theories of hemispheric specialization explain these syndromes' mandatory link with left-hemispheric damage, and are validated by this link. This link reflects the multiple nature of generic, categorical representational systems, not all reducible to natural language, and the left hemisphere's principal role as their substrate, regardless of their depen

Diagnostic and prognostic value of CT brain scan in term neonates with moderate and severe birth asphyxia.

Twenty term neonates with moderate (stage II) and 5 with severe (stage III) hypoxic ischemic encephalopathy (HIE) were prospectively studied to determine diagnostic and prognostic value of CT brain scan. Three neonates expired, 4 were lost to follow up while 18 were followed up to 18 months of age. Cerebral hypodensities were noted in 20 and intracranial hemorrhage (ICH) in 8, of which 6 had both ICH and hypodensity. Twelve of 14 infants with hypodensities and 5 of 6 with ICH who were followed up were handicapped at 18 months. Thirteen of 18 babies followed up were subjected to repeat CT scans

Sonographic diagnosis of subcortical cystic leukomalacia.

The real-time sonographic diagnosis of subcortical cystic leukomalacia has been described only recently. We report on a severely asphyxiated term infant who went on to develop this condition, and discuss the usefulness of ultrasound scanning in term babies with hypoxic-ischaemic disease.

[Criteria of brain death: 3d generation. Interpretation of criteria established by the Federal Chamber of Physicians in determining brain death].

The criteria of brain death published by the Federal Chamber of Physicians in 1982 in their second generation have stood the test of time and remain valid. The most important amendments in issuing a third generation are the following: In primary cerebral lesions supratentorial lesions have to be distinguished from infratentorial processes. In primarily the infratentorial processes EEG activities of the cortex max outlast apneic brain stem areflexia. Therefore in infratentorial processes controls of the EEG are indispensable. The extinction of evoked potentials, certain conditions being observe

Imitation of facial movements in brain damaged patients.

The imitation of a large repertoire of upper and lower facial actions was requested from a group of left, right brain damaged and control subjects in order to explore: (1) if a left or right hemispheric focal lesion produces a similar pattern of impairment on this task; (2) if the impairment is associated with oral apraxia. The results show that left and right brain damaged patients score significantly lower than controls but the two pathologic groups do not differ from each other. The imitation of facial movements is not related in any way to oral apraxia and no specific localization of the l
