BACKGROUND: Airway hyperresponsiveness (AHR) to direct stimuli, such as methacholine (MCh), is observed not only in asthma but other diseases. AHR to indirect stimuli is suggested to be more specific for asthma. The purpose of this study was to determine whether asthmatic airway inflammation is more closely related to AHR to hypertonic saline (HS), an indirect stimulus, than to MCh.
METHODS: Sixty-four consecutive adult patients with suspected asthma (45 asthma and 19 non-asthma) performed a combined bronchial challenge and sputum induction with 4.5% saline, and MCh challenge on the next day.

BACKGROUND: The methacholine challenge test is a common investigation used to identify airway hyperresponsiveness. There has been recent debate as to whether the airway effect is due to the concentration or the dose of methacholine.
OBJECTIVE: To determine whether the airway response during a methacholine challenge test is due to the concentration or the dose of methacholine.
METHODS: Ten subjects with stable, mild asthma were enrolled in this study. Two methacholine challenge tests, one with 30-second inhalations of methacholine and the other with two minute inhalations of methacholine, wer

PURPOSE OF REVIEW: To review the diagnostic accuracy of lung function measurements made using spirometry for childhood asthma, recent guidel … , needs clarification. The diagnostic profiles of measurements of bronchodilator responsiveness and bronchial hyperreactivity made using spirometry, although reasonable, are not perfect. In schoolchildren, they are no better than knowledge of aeroallergen sensitization when considering a diagnosis of asthma.

Histopathological studies of asthmatic airways removed postmortem or by bronchial biopsy show marked inflammatory changes, notably epithelial cell disruption and damage, and the presence of large numbers of eosinophils. The epithelial damage is seen in mild, asymptomatic asthmatics as well as in patients who have died in status asthmaticus. Damage to the epithelium may also correlate with bronchial hyperreactivity. The epithelium has been suggested to be a target for inflammatory cell mediators and cytokines. Recently, the airway epithelium has itself been shown to produce and release several …

Twenty-two patients with only chronic nonproductive cough lasting for more than 2 months were prospectively examined to determine whether airway cough receptor sensitivity and bronchial responsiveness relate to the efficacy of bronchodilator therapy on the cough. Clenbuterol (10 micrograms, 4 times a day for 1 week) was effective on the cough in 10 patients (group 2) but not in the other 12 patients (group 1). Cough threshold to inhaled capsaicin was significantly less in group 1 than in group 2 at the first visit but not after the therapy when the cough stopped. Bronchial responsiveness to me

The number of atopic patients presenting only chronic non-productive cough appears to be increasing. This study was conducted to confirm the existence of non-asthmatic cough associated with atopy. We prospectively examined atopic findings, therapeutic effects of inhaled procaterol, azelastin, and/or glucocorticoids, improvement of FEV1 by bronchodilator therapy and bronchial responsiveness to methacholine in 20 patients. The cough was relieved by inhaled procaterol in 10 patients (Group 2) but not in the other 10 patients (Group 1). The increase in FEV1 by inhaled salbutamol following aminophy

Views on mechanisms of increased airway responsiveness in patients with bronchial asthma and other respiratory tract are presented. The data have been divided into three parts: mucosal and submucosal changes, changes in airway regulations and effector cells. In the first part the etiopathological mechanisms of inflammatory processes in airways are explained. The second part views the neurogenic and other airway disorders (vagus reflex arc, local axon reflex, epithelial relaxing factor) while the third focuses on the receptor activity changes and cellular mechanisms known to cause airway hypers

Feline asthma (FA) is a common inflammatory airway disease with an unclear, multifactorial origin, involving inhaled aeroallergens triggering a type-1 hypersensitivity response and eosinophil activation. Clinical signs include chronic cough, wheezing, tachypnea, and respiratory distress. Diagnosis involves physical examinations, radiography, computed tomography, bronchoscopy, bronchoalveolar lavage, and allergy testing. Key features are bronchial hyperreactivity, mucus production, wall thickening, and airway remodeling. Main treatment includes inhaled corticosteroids, often combined with bronc…

Asthma may result from excessive Th-2 response in children not previously exposed to Th-1-inducing infections. We tested the hypothesis that BCG vaccination in Th-2-susceptible newborn BP2 mice blocks allergic inflammation and bronchial hyperreactivity (BHR). Ten day-old BP2 mice received 10(5) CFU of BCG 1173P2 intranasally (IN), and 6, 10 or 14 weeks thereafter were sensitized with 100 microg ovalbumin (OVA) in aluminium hydroxide twice subcutaneously (SC) at 1 week interval, and challenged 1 week after the second sensitization with 10 microg OVA IN. Compared to OVA-challenged unvaccinated m…

To elucidate the contribution of magnesium to bronchial hyperreactivity in patients with stable bronchial asthma, magnesium concentrations in serum (S-Mg), erythrocytes (R-Mg), and lymphocytes (L-Mg) were measured in 25 patients with bronchial asthma (BA group) and 9 age-matched healthy subjects (control group). A parenteral magnesium loading test, a continuous low-dose magnesium infusion of 0.2 mEq/kg over 4 hr, was performed in 10 of 25 asthmatic patients and in the control group. R-Mg was significantly lower in the BA group than in the control group (4.96 +/- 0.47, 6.13 +/- 0.62 mEq/L, p < …

Although atopic asthma symptoms often seem to disappear around puberty, subjects in this age group may experience unexpected, often severe, asthma attacks. This may be related to persistence of untreated airway hyperresponsiveness/inflammation in a life period characterized by low perceptiveness of disease-related symptoms. This study was designed to evaluate the prevalence and the severity of bronchial hyperreactivity and the exhaled nitric oxide (FENO) levels in a group of steroid-naive asthmatic adolescents. Fifty-two patients with mild-intermittent asthma were studied, ages 12 to 16, sensi…

Exposure to particulate matter (PM10) can induce respiratory diseases that are closely related to bronchial hyperresponsiveness. However, th … riction, elevated CHRM3 mRNA and protein expression levels on bronchial smooth muscle, then induced bronchial hyperreactivity. Additionally, 4 months of PM10 exposure caused an increase in ERK1/2 phosphorylation and increased the secretion of inflammatory factors in bronchoalveolar lavage fluid. Treatment with the MEK1/2 inhibitor, U0126 inhibited the PM10 exposure-induced …

OBJECTIVE: Bronchial challenge test (BCT) measures current airways-hyperreactivity, however, its predictive role in pre-school children (<6 years) for the diagnosis of asthma at school age is still debatable. We aimed to find whether preschool children with a positive adenosine or methacholine BCT are more prone to asthma at school age.
METHODS: We included children aged 6-13 years with respiratory symptoms that were previously referred to our pulmonary function laboratory for BCT (methacholine or adenosine, depending on the question asked) at age 10 months to 6 years (baseline). BCT was cons

The basic features of bronchial asthma are dyspnea with wheezing and objectively confirmed obstructive respiratory disorder reversible after inhalation of bronchodilators. In stable intermittent bronchial asthma, these features are not present; therefore confirmation of asthma consists of the presence of bronchial hyperresponsiveness (BHR). In the present study, there were 902 bronchoprovocation tests performed for the verification of BHR. A significant criterium for BHR is a decrease of FEV(1) of 20% from the baseline level. Every test either positive or negative was finished with inhalation

BACKGROUND: Smoking may influence the response of the lungs to other inhaled substances. A study was undertaken to assess the effect of the interaction between smoking and the immunoresponse to common aeroallergens (atopy) on bronchial responsiveness.
METHODS: A random sample was selected from the general population census of five areas of Spain (Albacete, Barcelona, Galdakao, Huelva, and Oviedo). A total of 1169 (35%) subjects completed a face-to-face respiratory questionnaire, a methacholine bronchial responsiveness challenge, and underwent measurements of total and specific serum IgE level
