Conformational flexibility and biological activity of salmon calcitonin.

We have assessed the biological activity of salmon calcitonin I (sCT) using an in vivo biological assay of hypocalcemic activity in rats. The changes in biological activity observed are explained on the basis of changes in the conformational properties of the hormone analogues. Helical content in the presence and absence of lipids and detergents was assessed by using circular dichroism, and the section of the molecule that folds into a helix was predicted on the basis of the helix-coil transition theory of Mattice and co-workers. In the amino acid sequence of sCT, residue 8 is valine and resid…

Immunoreactive somatostatin and calcitonin in pulmonary neuroendocrine tumor.

A well-differentiated neuroendocrine carcinoma of the lung that secreted immunoreactive somatostatin (IR-SRIF) and IR-calcitonin (CT) in a 72-year-old women is described. The plasma concentrations of IR-SRIF (57.5 pg/ml) and IR-CT (340 pg/ml) before operation were significantly higher than the respective normal ranges. After resection of the tumor, the plasma CT level (105 pg/ml) decreased to within the normal range, and the SRIF level (32.7 pg/ml) also decreased, but was still abnormally higher, which suggested the presence of an unidentified remnant of the tumor. Abnormal accumulation of tec…

Thyroglobulin and calcitonin immunoreactivity in canine thyroid carcinomas.

Although thyroid gland neoplasms are well-recognized entities in dogs, the diagnosis and classification of these tumors often is difficult. In contrast to human thyroid carcinomas, which are predominantly of the papillary or follicular types, a relatively high proportion of the canine tumors contain compact cellular areas and resemble, to some extent, medullary thyroid carcinomas. In order to assess the value of immunohistochemical techniques in the identification and classification of these neoplasms, 21 canine thyroid carcinomas were examined for the presence of thyroglobulin and calcitonin …

General principles, problems and interpretation in the radioimmunoassay of calcitonin.

Although radioimmunoassay (RIA) methods for calcitonin (CT) have been available for ten years, there remain problems in achieving high sensitivity, controlling plasma and other protein artifacts, and interpreting results in light of circulating CT heterogeneity and tumor products having CT-like immunoreactivity. The development of sensitive radioimmunoassays meeting these criteria remains challenging. We will review some of the approaches to these problems.

A stimulation test with calcitonin for differential diagnosis of hypercalcaemia.

Experimental studies have suggested that in primary hyperparathyroidism (HPT) the cells of the hyperfunctioning parathyroid tissue retain some capacity for stimulation and that an increase in secretion of parathyroid hormone (PTH) can occur when the extracellular calcium concentration is lowered within the hypercalcaemic range. We have tested this hypothesis in 23 patients with HPT, 10 patients with hypercalcaemia of other origin (7 of whom had disseminated malignant disease) and 17 normal subjects. In all three groups a single injection of 100 MRC units of salmon calcitonin caused a reduction…

Ectopic secretion of calcitonin as a tumor marker (review).

The significance of the hormone calcitonin, ectopically produced, is evaluated as a tumour marker. Since 40 percent of an unselected sample of tumours is hypercalcitoninaemic, calcitonin may be useful as a tumour marker, though absence of specificity limits its diagnostic value.

Hypersecretion of calcitonin in patients with the Verner-Morrison syndrome.

Three patients with the Verner-Morrison syndrome due to pancreatic tumours are reported; all had an overproduction of calcitonin and hypercalcemia. In two cases the hypercalcitoninaemia was due to an ectopic secretion from the pancreatic tumour, but in one the source could not be determined with certainty. Hypercalcemia in the Verner-Morrison syndrome might be due to simultaneous hyper-parathyroidism (MEA I) or secretion from the tumour of a substance or substances with calcium-elevating properties. Streptozotocin was of beneficial value in treating these patients. None of the patients became …

Pheochromocytoma associated with hypercalcemia and ectopic secretion of calcitonin.

A 17-year-old woman manifested fever, abdominal pain, headache, and hypertension caused by a solitary, benign pheochromocytoma. She also had hypercalcemia and elevated plasma immunoreactive calcitonin levels. After removal of the pheochromocytoma, calcium and calcitonin levels returned to normal. Studies of peripheral and tumor venous blood showed no excess or ectopic parathyroid hormone secretion, but the tumor contained and secreted calcitonin. Sporadic pheochromocytoma may secrete calcitonin and cause hypercalcemia by non-parathyroid hormone-mediated mechanisms. The potential is clearly pre…

Therapy of osteogenesis imperfecta with synthetic salmon calcitonin.

We evaluated the long-term use of synthetic salmon calcitonin in the management of osteogenesis imperfecta tarda and congenita. Forty-eight children, ranging in age from 6 months to 15 years, and two young adults, received synthetic salmon calcitonin 2 MRC units/kg three days a week and a daily oral calcium supplement of 230 to 345 mg. The annual fracture rate was decreased during calcitonin therapy as compared to the period preceding therapy. There was an increase in the ability of the patient to stand and move and in the subjective feeling of strength in the lower extremities during calciton…

Relative deficiency of plasma-calcitonin in normal women.

A new radioimmunoassay technique was used to measure plasma-calcitonin in normal subjects. Plasma-calcitonin concentrations in women were less than 25% of those in men, and were often undetectable. However, during pregnancy or administration of the oestrogen-progestagen contraceptive pill, plasma-calcitonin equalled or exceeded male concentrations. These findings are consistent with an effect on calcitonin production of normal male testosterone concentrations and concentrations of the sex steroids during pregnancy. Calcitonin may protect the skeleton during calcium stress. Further, although it…

The ectopic secretion of calcitonin by lung and breast carcinomas.

Many patients with advanced non-thyroid malignancies have elevated plasma immunoreactive calcitonin concentrations. Breast and bronchial carcinomas contain immunoreactive calcitonin and an epidermoid bronchial carcinoma has been shown to produce immunoreactive calcitonin in vitro. We have established monolayer cultures of breast carcinomas and eight out of fifteen consecutive carcinomas released immunoreactive calcitonin; some released HCG (human chorionic gonadotrophin) or CEA (carcinoembryonic antigen). In addition, a primary human breast carcinoma has been shown to release and contain calci…

Secretion of immunoreactive calcitonin by human breast carcinomas.

Twenty-three out of 28 patients with metastatic breast carcinoma and one out of 13 patients with localised disease had raised levels of plasma immunoreactive calcitonin. Monolayer cultures of breast carcinomas maintained for up to 10 weeks released immunoreactive calcitonin, and a primary breast carcinoma passaged in "nude" mice for over a year contained material immunologically and chromatographically resembling the monomeric form of human calcitonin. These studies indicate that breast carcinomas can produce calcitonin and that plasma calcitonin measurements may be useful in staging patients …

Calcitonin from ultimobranchial glands of dogfish and chickens.

Acid extracts of thyroid glands from a small shark Squalus suckleyi and domestic fowl Gallus domestica contained no detectable calcitonin activity, while very potent hypocalcemic responses were obtained in rats with similar extracts from the ultimobranchial glands of these two species. The calcitonin concentration was 4 to 40 times that present in hog thyroid, which, as in most other mammals, contains ultimobranchial tissue. The evidence suggests that calcitonin is a fundamental calcium-regulating hormone present in all higher vertebrates and that it is an ultimobranchial rather than a thyroid…

Calcitonin gene-related peptide-targeted therapies for migraine.

Calcitonin gene-related peptide (CGRP)-targeted therapies are the first medications developed specifically for migraine prevention. They block the actions of CGRP, a neuropeptide with a key role in migraine pathophysiology. There are 2 categories of drugs: monoclonal antibodies directed against either the CGRP ligand or receptor, and small-molecule CGRP receptor antagonists. CGRP monoclonal antibodies are available as self-administered subcutaneous injections or as an intravenous infusion, and are administered monthly or quarterly. Clinical trial and real-world data over the past 10 years supp…
