Five patients with slipped capital femoral epiphysis were conclusively diagnosed as being hypothyroid. One was a cretin, who was diagnosed and begun on thyroid medication at four months of age but subsequently many years later developed the slip. Suspicion of hypothyroidism should exist for the clinician who finds himself with a child below age 12 with a delayed bone age, borderline mental status and a slipped capital femoral epiphysis. His attention in addition to the orthopedic management of the slip, should be directed towards laboratory studies to rule out hypothyroidism.

Carpal fractures are exceedingly rare clinical entities and are often associated with concomitant injuries. In this review, we focus on fractures of the carpus, excluding the scaphoid, and provide an update on the current consensus as to mechanism, diagnosis, management, outcomes, and complications after such injuries.

This paper reports a longitudinal study of carpal bone development in 125 children (98 boys, 27 girls) attending a clinic for Perthes' disease. The age at appearance of ossification in each carpal bone was estimated by studying consecutive radiographs obtained at six-months intervals. The results were compared with the findings for normal children published by Stuart et al. Three radiologic groups of carpal bone maturation are defined: not delayed (capitate and hamate); markedly delayed (triquetral and lunate); and slightly delayed (scaphoid, trapezium, and trapezoid). Overall, the bone age de

PURPOSE: To evaluate the diagnostic value of magnetic resonance (MR) imaging in patients with clinical suspicion of scaphoid fractures and n … ures.
RESULTS: MR imaging depicted occult fractures of the scaphoid bone in 14 patients (33%), the capitate bone in four (10%), the trapezium in one (2%), and the distal radius in two (5%). All wrist fractures were detected with a combination of STIR and T1-weighted spin-echo sequences. The sensitivity and specificity for detection of radiographically occult f …

Proximal row carpectomy (PRC) is an effective treatment option for degenerative or posttraumatic osteoarthritis of the wrist in conditions such as scapholunate advanced collapse, scaphoid nonunion advanced collapse, Kienbock disease, and chronic fracture dislocations of the carpus. PRC involves excision of the scaphoid, lunate, and triquetrum, and relies on the articulation of the remaining capitate from the distal row to articulate with the lunate fossa. PRC offers the potential advantage of greater range of motion, technical ease, and decreased immobilization, and eliminates specific complic

STUDY OBJECTIVE: To evaluate the accuracy of day 4 bone scans in predicting the presence or absence of fracture in patients with "clinical scaphoid fracture."
DESIGN: Prospective sensitivity study of ED patients with clinical scaphoid fractures. Each patient was immobilized in a thumb spica cast and had day 4 bone scans of both wrists and hands. Blinded day 4 bone scan results were ultimately compared with the diagnosis on day 14 when patients returned for repeat clinical examination and radiographs. In cases of equivocal radiographic or clinical examination results, a day 14 bone scan was pe

The authors report two cases of bone disorders in children with short stature, with confirmed growth hormone (GH) deficiency treated by GH supplementation. The first patient, aged 15 years, developed avascular necrosis of the femoral head and scoliosis. The second one, aged 17 years, had avascular necrosis of the femoral capital epiphysis on one side and acute slipped capital femoral epiphysis (SCFE) on the other side. All these complications were diagnosed while they were receiving GH-therapy. The exact aetiology and the role of GH in the pathogenesis of these conditions are still unknown.

We report the case of a boy aged 13 years and 9 months, with predominantly right-sided spastic tetraparesis, who could walk with assistance, GMFCS III, phenotype consistent with adiposogenital syndrome. He presented a 4-week history of left-sided limp without pain, radiologically classified as a stable, chronic slipped capital femoral epiphysis (SCFE) with mild slippage (<30º) on the Southwick classification. In situ fixation of the hip was performed using a full-headed screw, followed by six weeks of rest. Twelve years since the intervention, the patient remains asymptomatic with no clinical

Carpal coalition is a rare wrist anomaly. A coalition can be an isolated condition, but it can be observed as a component of a congenital syndrome as well. Carpal coalition is usually asymptomatic and detected incidentally. It is most frequently encountered as lunate-triquetral coalition, followed by capito-hamate coalition. Other coalition types are very rare. In this case report, we present a case in which the patient applied with complaints of pain, and in whom a coalition of capitate-trapezoid- second and third metacarpal was detected.

The Os Styloideum (OS) is an accessory carpal bone located in the dorsal aspect of the wrist between the base of the second and third metacarpal bones and the trapezoid and the capitate. It is frequently partially coalited with the metacarpal bones and is usually asymptomatic. It may become symptomatic after a traumatic fracture, developing swelling, edema, and pain. We report 3 cases with a fracture of an OS partial coalition with the third metacarpal bone with signs of delayed consolidation. They all complain of persistent pain at the dorsal aspect of the hand that increased with activity. O

Current multifactorial age estimation methods are based on radiography, however, in the forensic field there is growing interest in using magnetic resonance imaging (MRI). With regard to the carpal region, MRI provides more information for defining the individual ossification nuclei and the cartilage surrounding single bones. During the phase of bone growth, the progressive reduction of the cartilage layer is accompanied by the development of a cartilage-bone interface. The aim of our study was to create a new model for age estimation, based on the ratio between the area occupied by the nucleu

OBJECTIVE: The aim of this study in clinically active juvenile idiopathic arthritis (JIA) was to assess the frequency and distribution pattern of synovitis as hallmark of disease and additional soft-tissue and bony abnormalities on MRI in the knee and wrist as two target joints.
MATERIALS AND METHODS: MRI datasets of 153 clinically active JIA patients (110 with knee and 43 with wrist involvement) were evaluated independently by two readers for the presence of literature-based imaging features: "synovial hypertrophy," "bone marrow changes," "bone erosions," "tenosynovitis" (only in the wrist),

Stable slipped capital femoral epiphysis (SCFE) is a pediatric hip disorder managed with in situ fixation of the proximal femoral epiphysis, otherwise known as 'pinning the hip'. The objective of this study was to characterize how the choice of implant for in situ fixation of stable SCFE has changed over time. A systematic review of publications concerning in situ fixation of stable SCFE from January 1993 to November 2021 was conducted. The change in the type, diameter, and number of implants used in publications over time and the age of their respective patient cohorts was evaluated. A total

Anatomical variations in bony structures around the wrist have been considered as risk factors for Kienböck's disease: ulnar variance, Nattrass index, ulnar variance/capitate height ratio and presence of a lunohamate joint. This study aimed to assess the order of importance of these variations as risk factors for Kienböck's disease. Two groups were formed: patients (n = 58) and controls (n = 235). On posteroanterior radiographs in the two groups, these risk factors were examined by four raters. After inter-rater correlation analysis, an artificial neural network was used to estimate their rela

The ideal management of an unstable slipped capital femoral epiphysis (SCFE) is still controversial. The currently available options are in-situ screw fixation with delayed corrective osteotomy, closed reduction and screw fixation and anatomic reduction by modified Dunn's procedure (MDP). We present the results of our technique of controlled re-positioning (CRP) with primary osteoplasty in which the epiphysis is repositioned to the preacute slip stage and a mini-open osteoplasty is done in the same sitting to avoid later femoro-acetabular impingement. We had 27 unstable slips which presented t

Combined fracture of the scaphoid and capitate bones with concurrent scapholunate dissociation, but without severe dislocation, is a rare lesion which results in significant carpal instability and requires operative treatment. We report a case of this unusual injury and its functional result after 20 months.
