BACKGROUND: Echocardiography is widely used for non-invasive measurements of stroke volume and cardiac output, while Ultrasonic Cardiac Output Monitor (USCOM) offers an alternative Doppler-based approach. Whether these two modalities can be used interchangeably in clinical practice remains unclear. This study aims to evaluate the correlation and agreement between echocardiography and USCOM for hemodynamic assessment.
METHODS: A total of 44 pediatric patients underwent hemodynamic assessment using echocardiography and sequential USCOM measurement during the same evaluation. Spearman's rank cor

In critically ill patients, fluid infusion is aimed at increasing cardiac output and tissue perfusion. However, it may contribute to fluid overload which may be harmful. Thus, volume status, risks and potential efficacy of fluid administration and/or removal should be carefully evaluated, and monitoring techniques help for this purpose. Central venous pressure is a marker of right ventricular preload. Very low values indicate hypovolemia, while extremely high values suggest fluid harmfulness. The pulmonary artery catheter enables a comprehensive assessment of the hemodynamic profile and is par

Cardiac output, total peripheral vascular resistance, renal, extrarenal, forearm muscle and skin hemodynamics and an indicator of the splanchic vascular resistance were estimated in 20 subjects with chronic renal disease without signs of chronic renal failure and without anemia. The data were compared with a group of subjects with essential hypertension. The high blood pressure of chronic renal disease of mild or moderate severity was maintained in the first place by a high cardiac output, this being due to a rise of the stroke volume, while the heart rate was only slightly increased. The tota

High-flow arteriovenous fistulas (AVFs) in hemodialysis patients can cause significant hemodynamic stress by increasing venous return and right ventricular preload, sometimes leading to pulmonary hypertension (PH) and high-output heart failure. We report two hemodialysis patients who underwent cardiac surgery and unremitting exertional dyspnea and intradialytic hypotension despite improved leftheart function. In case 1, a 74-year-old man demonstrated an AVF flow of 2.91 l/min with a resistance index of 0.47. In case 2, a 74-year-old man had an AVF flow of 3.84 l/min and resistance index of 0.4

BACKGROUND/AIMS: High-output heart failure (HOHF) is an under-recognised and understudied variant form of cardiac failure characterised by increased cardiac output (CO), low systemic vascular resistance, and higher filling pressures. This study aimed to compare mortality, morbidity, and haemodynamic profiles of patients with HOHF with those of patients with heart failure with preserved ejection fraction (HFpEF).
METHOD: Patients diagnosed with HFpEF who underwent invasive haemodynamic assessment in the United States Veterans Affairs (VA) health system between 2007 and 2022 were divided into t

Preeclampsia is associated with cardiac dysfunction, not only during the clinical phase of the disease, but also after delivery, with long term implications for both maternal and neonatal cardiovascular health. An abnormal cardiovascular phenotype also precedes conception, indicating that pre-existing cardiovascular dysfunction is associated with the development of preeclampsia. This review summarises the changes in cardiovascular function in preeclampsia, examining the evidence for when cardiovascular dysfunction develops and presenting the evidence for two phenotypes - one associated with fe

Glomerular hyperfiltration alone or associated with albuminuria is a well-known feature of sickle cell associated nephropathy. Though, glomerular hyperfiltration is currently considered to be related to a high renal plasma flow and chronic hemolysis, cardiac output influence on measured glomerular filtration rate (mGFR) have not been investigated so far. Thirty seven homozygous sickle cell patients (SCA) from the RAND study investigated before and under angiotensin converting enzyme inhibitor (ACEI) were included. Both mGFR and cardiac index (CI) were high (> 110 ml/min/1.73 m2 and > 3.5 l/m2

BACKGROUND: Current guidelines identify pulmonary hypertension (PH) secondary to tachycardia-driven high-output conditions (eg, anemia, thyrotoxicosis), whereas bradycardia-induced PH-a critical low-flow phenotype-has not been addressed.
CASE SUMMARY: We present a case of atrial fibrillation with slow ventricular response and PH. Pacemaker implantation was performed, and hemodynamic evaluation revealed that as the heart rate increased, pulmonary artery pressure progressively decreased. Postoperative cardiac function indices also improved.
DISCUSSION: This case, along with existing evidence,

Congenital hemangiomas (CHs) are unusual and diverse tumors distinguished from infantile hemangiomas by being largely developed at birth and glucose transporter (GLUT1)-negative. We describe three infants who presented in utero or at birth with segmentally distributed vascular tumors that were GLUT1-negative, had histology compatible with congenital hemangioma, and exhibited spontaneous clinical involution. One of the three patients had high-output cardiac failure and was found to have a mutation in GNAQ (c.626A>c, p.Gln209Pro); another had high-output cardiac failure, heterotaxy, and transien

In 7 hypertensive patients with renal artery stenosis and in 1 patient with hypertension and unilateral pyelonephritic nephrophthisi the influence of the angiotensin II antagonist, saralasin on systemic hemodynamics was studied. In the patients with normal renin infusion of saralasin produced an increase in total peripheral resistance, in patients with elevated renin a decrease in peripheral resistance was observed. In 3 patients who had extremely high renin levels while under sodium saralasin produced a dangerous drop in blood pressure concomitant with a marked decrease in cardiac output and

OBJECTIVES: Extracorporeal life support (ECLS) is the reference rewarming technique of accidental deep hypothermic cardiac arrest (DHCA). This study was designed to examine the impact of different rewarming blood flow rates and temperature setting of ECLS on cardiopulmonary lesions after DHCA in a porcine model of accidental hypothermia.
METHODS: Twenty-four pigs were cannulated for ECLS, cooled until DHCA occurred, and subjected to 30 minutes of cardiac arrest. During the rewarming phase, we compared a low blood flow rate of 1.5 L/min versus a high flow rate of 3.0 L/min as well as two-tempe

PURPOSE OF REVIEW: Maternal haemodynamic changes during spinal anaesthesia for caesarean section have traditionally been evaluated by noninvasive blood pressure and heart rate. Recent publications have addressed the importance of cardiac output measurement in the assessment of the maternal circulation. In this review, a physiological approach is suggested for the prevention and treatment of haemodynamic instability during caesarean section in healthy women and in those with preeclampsia or cardiac disease.
RECENT FINDINGS: A better understanding of the maternal haemodynamic effects of spinal

This article reviews causes of marathon collapse related to physical exhaustion, heat exhaustion and dehydration. During severe exercise-heat stress (high skin and core temperatures), cardiac output can decrease below levels observed during exercise in temperate conditions. This reduced cardiac output and vasodilated skin and muscle can make it difficult to sustain blood pressure and perhaps cerebral blood flow. Dehydration can accentuate this cardiovascular strain. In contrast, excessive heat loss to the environment during cold weather may result in hypothermic collapse. Other factors contrib

A 53-year-old Japanese woman underwent investigation of her heart murmur. A continuous abdominal bruit was heard. Blood gas analysis and chest X-ray showed congestive heart failure. Enhanced computed tomography of the pelvis showed a 10 x 4 cm hypervascular tumor in the retroperitoneal space. Cardiac catheterization disclosed a cardiac output of 13.2 L/min and a step-up of oxygen at the right common iliac vein. Abdominal aortic angiography showed an extremely vascular pelvic tumor and rapid filling of the inferior vena cava. This is a rare case of a highly vascular pelvic tumor causing high-ou

Except in the presence of left to right shunts, slow aspiration from the distal lumen of a catheter placed in the pulmonary artery can provide a sample of mixed venous blood. The oxygen saturation of such a sample (SvO2) is dependent on arterial oxygen saturation, hemoglobin concentration, cardiac output, and tissue oxygen demands. A fall in SvO2 usually implies anemia, arterial oxygen desaturation, and/or decreased cardiac output; however, a normal or high value does not exclude such disturbances. When employed in conjunction with the other indicators of tissue oxygenation available in an int

OBJECTIVE: To determine if hemodynamic assessment in 'low-risk' pregnant women at term with an appropriate-for-gestational age (AGA) fetus can improve the identification of patients who will suffer maternal or fetal/neonatal complications during labor.
METHODS: This was a prospective observational study of 77 women with low-risk term pregnancy and AGA fetus, in the early stages of labor. Hemodynamic indices were obtained using the UltraSonic Cardiac Output Monitor (USCOM® ) system. Patients were followed until the end of labor to identify fetal/neonatal and maternal outcomes, and those which

The mechanisms responsible for maintenance of the high-output state associated with thyrotoxicosis have been investigated by measurement of cardiac-function curves and venous compliance during ganglionic blockade with trimethaphan. Thirteen calves were injected daily with L-thyroxine (200 micrograms/kg) for 12-14 days. Thyroxine treatment increased heart rate (70%), left ventricular systolic pressure (22%), cardiac output (120%), left ventricular maximum rate of pressure development (dP/dt) (56%), and total blood volume (18%) and decreased systemic vascular resistance (39%). These hemodynamic

Twenty-three patients older than 50 years, with systolic hypertension, underwent hemodynamic study. Cardiac output and stroke volume varied widely, with several high values. An elevated systemic vascular resistance, when considered as a function of the cardiac output, was the most prevalent hemodynamic abnormality. After administration of hydrochlorothiazide, 50 mg/day, for one month in 20 patients, 18 had a significant fall in BP, systemic vascular resistance, and stroke volume. After one year of continuous therapy, the hemodynamics, studied in 14 patients, did not change further. There were
