Cardiothoracic emergencies associated with subclavian hemodialysis catheters.

Three adults have had life-threatening perforations of the vena cava or right atrium associated with the use of subclavian hemodialysis catheters. Subclavian hemodialysis catheters should be meticulously positioned in the superior vena-caval lumen so the tip does not tent the vena-caval wall. If a patient with a subclavian hemodialysis catheter experiences unexplained and refractory hypotension, especially during hemodialysis, cardiac or vena-caval perforation should be suspected.

Knotting of central venous catheters: nonsurgical correction.

A complete knot occurred in a central venous catheter inserted through the subclavian vein. By passing a spring guidewire into the catheter under fluoroscopic control, the knot was easily untied leaving the catheter correctly in place.

The injury potential of Fogarty balloon catheters.

The problem of arterial injury following embolectomy using a balloon catheter is reviewed. Three illustrative clinical cases are presented. The cause of the arterial injury in these cases was due to rupture of the vessel. The hypotheses were that either the vessel was ruptured by direct penetration or bursting by overdistension of the balloon. The linear appearances of the arterial tear in two patients and a false aneurysm in the third suggested that bursting was the correct explanation. Laboratory investigation was carried out on animals and human cadavers and these confirmed that arterial in

Systematic culture of central catheters and infections related to catheters in a neonatal intensive care unit: an observational study.

Systematic culture of the tip of central lines is performed in many neonatal intensive care units (NICUs) to guide any subsequent antibiotic therapy. The clinical relevance of this procedure is debated, given the significant bacterial contamination during its removal. We aimed to describe infections related to catheters and assess the usefulness of central catheter systematic cultures for probabilistic antibiotic therapy in cases of suspicion of catheter-related infections in a NICU. A retrospective study in a NICU included all newborn patients hospitalized with a central catheter, between Jan…

Thermal profiles of pulsed field ablation catheters.

BACKGROUND: Pulsed field ablation (PFA) induces cell death predominantly through irreversible electroporation (IRE), a non-thermal mechanism. However, Joule heating during PFA delivery may result in clinically relevant temperature rises. OBJECTIVES: This study aimed to characterize the thermal profiles, peak surface temperatures, and ablation lesions produced by 6 proprietary PFA catheters using an ex vivo vegetal model. METHODS: The thermal profile of 6 PFA catheters-Farawave, PulseSelect, Volt, Varipulse, Electropulse, and Sphere-9-was evaluated in Solanum Tuberosum at recommended energy s…

Flushing peripheral intravenous catheters: A scoping review.

BACKGROUND: Peripheral intravascular catheters (PIVCs) are indispensable vascular access devices in healthcare, facilitating the administration of intravenous therapies. Despite their vital role, PIVCs are frequently associated with complications such as occlusion, infection, and thrombosis, which contribute to catheter failure. Flushing catheters is one of the most common practices during PIVC maintenance, as it cleans the internal catheter lumen, ensuring patency and reducing the risk of complications. However, inconsistencies in flushing practices such as flushing technique, volume to use, …

Long-term antifouling surfaces for urinary catheters.

The presence of a variety of bacteria is an inevitable/indispensable part of human life. In particular, for patients, the existence and spreading of bacteria lead to prolonged treatment period with many more complications. The widespread use of urinary catheters is one of the main causes for the prevalence of infections. The necessity of long-term use of indwelling catheters is unavoidable in terms of the development of bacteriuria and blockage. As is known, since a permanent solution to this problem has not yet been found, research and development activities continue actively. Herein, polyeth…

Indwelling pleural catheters: evidence for and management.

The incidence of pleural disease is increasing, and pleural medicine is increasingly recognised as a subspecialty within respiratory medicine. This often requires additional training time. Once underresearched, the last decade has seen an explosion in evidence related to the management of pleural disease. One of the cornerstones of pleural effusion management is the insertion of an indwelling pleural catheter. This allows patient-centred outpatient management and now has a robust evidence base. This article summarises evidence as well serves as a practical guide to the management of any compli

Bedside Clinician's Guide to Pulmonary Artery Catheters.

BACKGROUND: Pulmonary artery catheters provide important information about cardiac function, mixed venous oxygenation, and right-sided pressures and potentially provide temporary pacing ability. OBJECTIVE: To provide bedside clinicians with guidance for techniques to insert right heart monitors and devices, describe risk factors for difficult insertion and contraindications to placement, and provide updates on new technologies that may be encountered in the intensive care unit. METHODS: An extensive literature review was performed. Experienced clinicians were asked to identify topics not add…

["Naked" exit-site for peritoneal dialysis catheters].

Exit site infections (ESI) and peritoneal catheter tunnel infections are strongly associated with peritonitis. Alternative exit-site dressings can include the use of water and soap and the absence of sterile gauze. This article reports our experience with "naked" exit-sites, meaning without any kind of gauze to cover them. From January 2017 to October 2020, we enrolled 38 patients of the Nephrology and Dialysis Unit of the "San Martino" Hospital in Belluno. Nine of these patients had a "naked" exit-site. At the end of the study, no significant differences were found in the percentage of ESI-fr

Imaging Approach to Misplaced Central Venous Catheters.

Central venous catheters (CVCs) are commonly used in patients in a variety of clinical settings, including the intensive care unit, general ward, and outpatient settings. After placement, the radiologist is frequently requested to evaluate the location of CVCs and deem them suitable for use. An understanding of the ideal location of catheter tips as well as the approach to identifying malpositioned catheter tips is essential to prevent improper use, recognize and/or prevent further injury, and direct potential lifesaving care. An approach to CVC placement based on tip location can be helpful i…

Avoiding Bladder Catheters During Atrial Fibrillation Ablation.

OBJECTIVES: This study sought to determine if atrial fibrillation (AF) ablation can be performed safely without bladder catheterization. BACKGROUND: Patients undergoing AF ablation often receive bladder catheters. Catheterization is associated with potential complications. The ABCD-AF (Avoiding Bladder Catheters During Atrial Fibrillation) ablation study evaluates the advantages of performing AF ablation without routine catheterization. METHODS: In this single-center, prospective, randomized controlled trial, 80 patients received bladder catheterization (group A), and 80 patients received on…

The Esophageal Manometry with Gas-perfused Catheters.

BACKGROUND: The well-established methods for esophageal manometry have some disadvantages: the-water-perfused catheters needs calibration by gravity and measuring in supine position, and the solid-state catheters are very expensive. Manometry using gas-perfused catheters is a suitable alternative. There have been only a few publications about this. OBJECTIVES AND METHODS: The results for esophageal manometry in 1700 patients were retrospectively analyzed based on the clinical reports and the manometry data. The gas-perfusion manometry was critically assessed. RESULTS: The mean age was 54 yea…
