BACKGROUND: Infective endocarditis (IE) is commonly complicated by cerebral embolization and hemorrhage secondary to intracranial mycotic aneurysms (ICMAs). These complications are associated with poor outcome and may require diagnostic and therapeutic plans to be modified. However, routine screening by brain CT and CT angiography (CTA) is not standard practice. We aimed to study the impact of routine cerebral CTA on treatment decisions for patients with IE.
METHODS: From July 2007 to December 2012, we prospectively recruited 81 consecutive patients with definite left-sided IE according to mo

INTRODUCTION: Fenestration, early bifurcation, and duplication of the posterior cerebral artery (PCA) and the so-called hyperplastic anterior choroidal artery (AChA), considered a variation of the PCA, are rare. We evaluated the prevalence and characteristic features of these PCA variations on magnetic resonance (MR) angiography.
METHODS: We reviewed intracranial MR angiographic images of 2402 patients examined using a 3-tesla scanner. Images from the skull base to the intracranial region were obtained using the standard time-of-flight technique. We excluded images of 52 patients with insuffi

Intracranial atherosclerotic disease may constitute the most common cause of ischemic stroke worldwide; yet, in the developed world, imaging research has largely focused on extracranial atherosclerosis. Many studies in populations of Asian, African, and Hispanic descent demonstrate the preponderance of intracranial stenosis compared with carotid stenosis. This review examines the clinical presentations of MCA atherosclerosis and stenosis and the use of noninvasive MR imaging in the assessment of intracranial vasculature. MRA is a well-validated technique that offers great advantage over tradit

Computerized tomography(CT) angiography is a reliable alternative to digital subtraction angiography for the detection of aneurysms. The authors report an aneurysm of anterior temporal artery(ATA) misinterpreted as being in the bifurcation of middle cerebral artery(MCA) treated by aneurysm excision followed by primary anastomosis.

Visualization of the cerebral vascular tree is important in experimental stroke and cerebral vascular malformation research. We describe a simple method, nuclear contrast angiography, that enables simultaneous visualization of the arterial tree and cerebral endothelial cells in rodent brain whole mounts. A mixture of latex and black ink was injected into the arterial system of rodents, resulting in high contrast demarcation of the arterial tree of the brain. This method clearly differentiates arteries from veins. We applied this method to demonstrate that 14 days of unilateral carotid artery o

OBJECTIVE: Spontaneous intracranial haemorrhage constitutes 18-40% of all stroke cases. Indications for cerebral angiography to find underlying potentially treatable vascular abnormalities are not clear. This study determined which intracranial haemorrhage patients need cerebral angiography by correlating computed tomography (CT) findings, age and hypertension history with cerebral angiography findings.
METHODS: A total of 54 patients (8-79 years) with intracranial haemorrhage who underwent both CT examination and six-vessel cerebral angiography were studied over a 2-year period. Cerebral ang…

We aimed to establish a method for identifying the ascending convexity branches of the middle cerebral artery on digital subtraction arteriography. We reviewed 100 randomly chosen lateral internal carotid arteriograms of patients with aneurysms. After identifying the Sylvian triangle, the posterior internal frontal, paracentral, superior internal parietal and pericallosal arteries, we formulated templates in order to assess the position of the angular, posterior parietal, central, precentral and prefrontal arteries. Using the Sylvian triangle as a template, we identified the angular and poster

BACKGROUND AND PURPOSE: Digital subtraction angiography (DSA) is the standard of reference for detecting cerebral vasospasm after subarachnoid hemorrhage (SAH). CT angiography (CTA) is a relatively recent method for depicting the intracranial arterial vasculature. The purpose of this study was to compare CTA and DSA in the detection and quantification of cerebral vasospasm.
METHODS: Seventeen patients with SAH underwent initial CTA with or without DSA and follow-up CTA and DSA. The follow-up CTA and DSA studies were performed within 24 hours of each other and 5 to 10 days after SAH. Maximum i

In moyamoya disease, progression of carotid occlusive lesion in an adult patient is very rare. We report a case of adult moyamoya disease with acute angiographical stage progression and hemodynamic deterioration. A 56-year-old female complaining of left motor weakness visited our department. On MRI, infarct lesion was found in the right white matter. On cerebral angiography, occlusive lesions in the bilateral internal carotid arterial siphons and moyamoya vessels were found. The right side was stage V and the left side was stage III. On IMP-SPECT, decreased cerebral hemodynamic reserve of the …

Plain and contrast enhanced computed tomography (CT) scan of brain and cerebral angiography were done in 20 subjects of subarachnoid haemorrhage (SAH), diagnosed on the basis of clinical parameters and proved by cerebrospinal fluid (CSF) examination. Plain CT scan of brain confirmed the diagnosis of SAH in all the 20 (100%) cases, but was able to demonstrate the cause of SAH in 11(55%) cases only. Contrast CT scan provided additional information in 7 more cases. CT was also able to demonstrate the site and extent of bleeding, evidence of intraventricular bleeding and hydrocephalus. Cerebral an…

Time-of-flight MR-angiography of large volumes is limited by the occurrence of saturation effects, which lead to low signal in both veins and arteries. Alternatively to a number of other MRA-techniques, intravenous application of paramagnetic contrast media in combination with 3D-pulse sequences with or without flow refocussing allows the depiction of slow vessels in large volumes without technical extra expenses. The main intracranial indication is anatomical 3D-imaging of normal and dysplastic cerebral veins with high spatial resolution, and the additional depiction of the venous drainage in

OBJECTIVE: To determine the frequency of unexpected major arterial occlusion and incomplete aneurysm clipping on intraoperative angiography after cerebral aneurysm clipping and to determine factors that predict these unexpected findings.
METHODS: Data was collected prospectively on 100 consecutive craniotomies for the clipping of 107 aneurysms in 92 patients. Patient age and sex, aneurysm location and size, how the aneurysm presented, day of surgery after hemorrhage, intraoperative rupture, and postoperative course were recorded. After clipping, the surgeon recorded whether he thought the ane

Standard cervical carotid arteriography was performed on 36 patients and compared with results of noninvasive Doppler arterial imaging and intravenous angiography (IVA). The 72 carotid arteries were anatomically classified by standard angiograms as follows; group 1 (normal), 13 arteries; group 2 (wall disease [50% stenosis]), 14 arteries; group 3 (moderate stenosis [5-% to 75% stenosis]) 12 arteries; group 4 (severe stenosis [greater than 75%]), 15 arteries; and group 5 (complete occlusion), 18 arteries.

The cerebral angiograms of 11 patients suffering from collagen disease are presented. Panarteriitis nodosa was diagnosed in 4 cases, Lupus erythematodes in 2 cases. With 5 patients immunovasculitis with cerebral affection was found, which was, however, not to be classified in detail. More or less characteristic features are to be expected in the angiogram; they might harden the suspicion of collagen disease, although they are not likely to prove its diagnosis. An interpretation of the radiological findings should--in addition to the morphology--primarily take into account the distribution type

PURPOSE: To develop a time-resolved extension of three-dimensional time-of-flight MR angiography, termed Dynamic TOF (DynaTOF), that exploits intrinsic RF saturation behavior to enable noncontrast visualization of cerebrovascular hemodynamics.
METHODS: DynaTOF was implemented by varying flip angle, repetition time (TR), and slab-selective RF excitation profile to modulate longitudinal magnetization saturation and encode relative inflow timing. Data were acquired using a short-TR protocol combined with deep-learning reconstruction to ensure scan efficiency. Five healthy volunteers underwent Dy
