Influences of protein levels on the cerebrospinal fluid distribution of ceftazidime & ceftriaxone in the cerebrospinal fluid of patients with inflamed meningitis.

INTRODUCTION: Ceftazidime and ceftriaxone are used to treat various gram-negative pathogens, such as Streptococcus pneumoniae and Pseudomonas aeruginosa, and have shown excellent therapeutic efficacy against bacterial meningitis. However, there is insufficient information on the pharmacokinetic characteristics of their cerebrospinal distribution. Here, we investigated the association of clinical laboratory data in cerebrospinal fluid with ceftazidime and ceftriaxone concentration in the cerebrospinal fluid of patients with inflamed meningitis. METHODS: Cerebrospinal fluid samples were collect…

Anti-VDRL antibodies by ELISA in cerebrospinal fluid and its value in the differential diagnosis of abnormalities of the cerebrospinal fluid.

Enzyme-linked immunoabsorbent assay (ELISA) techniques for the detection of antibodies to the Venereal Disease Research Laboratories (VDRL) antigen as well as for the estimation of antibodies to cardiolipin (aCL) in cerebrospinal fluid (CSF) were performed in several groups of patients, including those with definite paretic neurosyphilis (DPNS, 10 patients), probable paretic neurosyphilis (PPNS, 19 patients), systemic lupus erythematosus (SLE, 71 patients), and miscellaneous neurologic disorders (30 patients), and normal subjects (11 patients). In the DPNS group, all demonstrated positive VDRL…

[Blood-cerebrospinal fluid barrier function and cholinesterase activity in cerebrospinal fluid in degenerative changes of the lumbar spine].

The objective of this study was to determine whether spinal compression associated with degenerative changes of the lumbar vertebral column induces significant alteration in defined cerebrospinal fluid parameters. Serum and lumbar cerebrospinal fluid were studied in 62 patients and 47 age-matched controls. Patients were grouped according to neurologic status, duration of symptoms, and findings in magnetic resonance imaging. Statistically significant elevations in cerebrospinal fluid/serum albumin ratio, acetylcholinesterase, butyrylcholinesterase activity, and total protein concentration were …

Population pharmacokinetics of vancomycin in serum and cerebrospinal fluid: variability in cerebrospinal fluid concentrations despite targeted serum exposure.

Vancomycin is a key component of empirical therapy for central nervous system (CNS) infections, yet dosing is guided by systemic pharmacokinetic/pharmacodynamic (PK/PD) targets that may not reflect drug exposure at the site of infection. Cerebrospinal fluid (CSF) penetration is variable, and the relationship between serum and CSF concentrations remains poorly defined. This study aimed to develop a population pharmacokinetic model simultaneously describing vancomycin concentrations in serum and CSF using real-world therapeutic drug monitoring (TDM) data and to characterize the relationship betw…

Effects of increased intracranial pressure on cerebrospinal fluid influx, cerebral vascular hemodynamic indexes, and cerebrospinal fluid lymphatic efflux.

The glymphatic-lymphatic fluid transport system (GLFTS) consists of glymphatic pathway and cerebrospinal fluid (CSF) lymphatic outflow routes, allowing biological liquids from the brain parenchyma to access the CSF along with perivascular space and to be cleaned out of the skull through lymphatic vessels. It is known that increased local pressure due to physical compression of tissue improves lymphatic transport in peripheral organs, but little is known about the exact relationship between increased intracranial pressure (IICP) and GLFTS. In this study, we verify our hypothesis that IICP signi…

Syndrome of cerebrospinal fluid hypovolemia following lumbar puncture cerebrospinal fluid leak in a patient with idiopathic intracranial hypertension.

An 11-year-old girl presented with headache of 3 months' duration. There was bilateral disc edema. The cerebrospinal fluid pressure was 50 cm of water with normal cerebrospinal fluid cytology and biochemistry. She developed severe headache (different and disabling), dizziness, vomiting, and backache on sitting up 6 hours after lumbar puncture, and lying supine relieved all of her symptoms. Intravenous fluids, analgesics, and complete bed rest did not relieve her symptoms over the next 72 hours. She was completely relieved of her symptoms on receiving two tablets of Caffergot containing 200 mg …

Collagen sponge repair of small cerebrospinal fluid leaks obviates tissue grafts and cerebrospinal fluid diversion after pituitary surgery.

OBJECTIVE: Repair of a cerebrospinal fluid (CSF) leak created at the time of transsphenoidal surgery typically involves placement of a fat, fascial, or muscle graft and sellar floor reconstruction. In this report, a simplified repair for small, "weeping" CSF leaks using collagen sponge is described. METHODS: All patients underwent an endonasal transsphenoidal procedure using the operating microscope. At the completion of tumor removal, if a small CSF leak was noted but no obvious large arachnoidal defect was present, a piece of collagen sponge was fashioned to cover the exposed diaphragma sel…

Agarose electrophoresis of cerebrospinal fluid in multiple sclerosis. A simplified method for demonstrating cerebrospinal fluid oligoclonal immunoglobulin bands.

The gamma globulins in cerebrospinal fluid from almost all patients with multiple sclerosis migrate in agarose electrophoresis as abnormal discrete populations, so-called oligoclonal bands. Such bands have also appeared in cerebrospinal fluid from patients with other types of inflammatory pathology such as neurosyphilis, acute idiopathic polyneuropathy, and subacute sclerosing panencephalitis. The demonstration of cerebrospinal fluid oligoclonal bands may aid in the evaluation of patients with early or atypical multiple sclerosis. This report describes a simple method for demonstrating cerebro…

Myelin basic protein in cerebrospinal fluid and other body fluids.

Myelin basic protein (MBP) or a fragment thereof may enter cerebrospinal fluid (CSF) and other body fluids in an etiologically nonspecific fashion to provide information about the status of central nervous system (CNS) myelin damage. MBP immunochemically detected is referred to as MBP-like material (MBPLM). The clinical utility of the assay for MBPLM in CSF is to document the presence, continuation, or resolution of CNS myelin injury. The analysis of CSF for MBPLM is subject to many variables, among which are the antisera and the form of the assay utilized. The dominant epitope of CSF MBPLM is…

Cerebrospinal fluid dynamics and cerebrospinal fluid infusion in children. Part II: Clinical application of lumbar cerebrospinal fluid infusion in children with macrocephaly and normal growth rate of the head circumference.

In fourteen children with macrocephaly and a normal growth rate of the head circumference, the CSF dynamics were studied by means of the lumbar CSF infusion test with constant flow. We discuss the significance of the outflow resistance together with the advantage of the Pressure Volume Index, which constitute the major parameters of the test. On the basis of computed tomography, we were able to show a differentiation in macrocephaly between megalencephaly, so-called external ventricular obstructive hydrocephalus and communicating hydrocephalus. We were not able to establish a disturbance of th

Cerebrospinal fluid is a significant fluid source for anoxic cerebral oedema.

Cerebral oedema develops after anoxic brain injury. In two models of asphyxial and asystolic cardiac arrest without resuscitation, we found that oedema develops shortly after anoxia secondary to terminal depolarizations and the abnormal entry of CSF. Oedema severity correlated with the availability of CSF with the age-dependent increase in CSF volume worsening the severity of oedema. Oedema was identified primarily in brain regions bordering CSF compartments in mice and humans. The degree of ex vivo tissue swelling was predicted by an osmotic model suggesting that anoxic brain tissue possesses

Imaging of Cerebrospinal Fluid Shunts and Their Complications.

Hydrocephalus, characterized by excessive intracranial accumulation of cerebrospinal fluid (CSF) in the ventricular system as a result of excessive production, decreased absorption, or impaired circulation of CSF, is a common condition affecting 1%-2% of the U.S. population. More than 40 000 CSF shunts are placed every year in the United States, the majority of which are for treatment of hydrocephalus. Temporary options for CSF diversion include serial lumbar or ventricular punctures, ventriculosubgaleal shunt, external ventricular drain, and ventricular access device. Permanent CSF diversion …

Cerebrospinal fluid flushing as a means of neuroprotection.

Central nervous system (CNS) injury or disease states are often difficult to treat due to the closed system of the dura mater/blood-brain barrier and the bony skull and vertebrae. The closed system results in at least partial containment of any pro-inflammatory molecules, pathogens, or toxic byproducts in the case of brain or spinal cord lesions, which can result in a destructive feedback loop. Cervical-approach access techniques (lateral C1-C2, suboccipital and lateral atlanto-occipital space punctures) are less-common methods of cerebrospinal fluid (CSF) sampling due to the relative ease and…

Molecular forms of neurogranin in cerebrospinal fluid.

Neurogranin (Ng) is a 78 amino acid neuronal protein and a biomarker candidate for Alzheimer's disease (AD). Ng has been suggested to bind to calmodulin and phosphatidic acid via its centrally located IQ domain. Ng is cleaved within this functionally important domain, yielding the majority of fragments identified in cerebrospinal fluid (CSF), suggesting that cleavage of Ng may be a mechanism to regulate its function. Up to now, Ng has been shown to be present in CSF as both C-terminal fragments as well as full-length protein. To obtain an overview of the different molecular forms of Ng present…
