On the mechanisms of brain blood flow regulation during hypoxia.

The brain requires an uninterrupted supply of oxygen and nutrients to support the high metabolic needs of billions of nerve cells processing information. In low oxygen conditions, increases in cerebral blood flow maintain brain oxygen delivery, but the cellular and molecular mechanisms responsible for dilation of cerebral blood vessels in response to hypoxia are not fully understood. This article presents a systematic review and analysis of data reported in studies of these mechanisms. Our primary outcome measure was the percent reduction of the cerebrovascular response to hypoxia in condition

Recent insights into mechanisms of hypoxia-induced vasodilatation in the human brain.

The cerebral vasculature manages oxygen delivery by adjusting arterial blood in-flow in the face of reductions in oxygen availability. Hypoxic cerebral vasodilatation, and the associated hypoxic cerebral blood flow reactivity, involve many vascular, erythrocytic and cerebral tissue mechanisms that mediate elevations in cerebral blood flow via micro- and macrovascular dilatation. This contemporary review focuses on in vivo human work - with reference to seminal preclinical work where necessary - on hypoxic cerebrovascular reactivity, particularly where recent advancements have been made. We pro

Mathematical Modelling in Hydrocephalus.

BACKGROUND: Various studies highlight the significance of alterations in cerebrospinal fluid (CSF) and cerebral blood flow (CBF) dynamics in the pathogenesis of hydrocephalus and suggest the role of mathematical modeling in studying these complex interactions. OBJECTIVE: This narrative review discusses mathematical models of CSF and CBF dynamics, including Marmarou's compartmental model of CSF spaces and a model of cardiac changes in cerebral arterial blood volume. The diagnostic utility of CSF compensatory parameters is described along with current information on secondary model-based indice

Impairments of cerebral blood flow microcirculation in rats brought on by cardiac cessation and respiratory arrest.

The impairments of cerebral blood flow microcirculation brought on by cardiac and respiratory arrest were assessed with multi-modal diagnostic facilities, utilising laser speckle contrast imaging, fluorescence spectroscopy and diffuse reflectance spectroscopy. The results of laser speckle contrast imaging show a notable reduction of cerebral blood flow in small and medium size vessels during a few minutes of respiratory arrest, while the same effect was observed in large sinuses and their branches during the circulatory cessation. Concurrently, the redox ratio assessed with fluorescence spectr

Predictive Value of Cerebral Autoregulation Impairment for Delayed Cerebral Ischemia in Aneurysmal Subarachnoid Hemorrhage: A Meta-Analysis.

OBJECTIVE: Delayed cerebral ischemia (DCI) happens in about 30% of patients with aneurysmal subarachnoid hemorrhage (SAH) and is related to higher mortality and disability. Some studies have shown cerebral autoregulation impairment can be a predictor of DCI in aneurysmal SAH. We conducted this meta-analysis to evaluate the predictive value of cerebral autoregulation impairment for DCI based on the current literature. METHODS: A systematic literature search was performed in PubMed and Embase. According to inclusion and exclusion criteria, 2 authors screened the records and extracted data from

Do acute stroke patients develop hypocapnia? A systematic review and meta-analysis.

PURPOSE: Carbon dioxide (CO2) is a potent cerebral vasomotor agent. Despite reduction in CO2 levels (hypocapnia) being described in several acute diseases, there is no clear data on baseline CO2 values in acute stroke. The aim of the study was to systematically assess CO2 levels in acute stroke. MATERIAL AND METHODS: Four online databases, Web of Science, MEDLINE, EMBASE and CENTRAL, were searched for articles that described either partial pressure of arterial CO2 (PaCO2) and end-tidal CO2 (EtCO2) in acute stroke. RESULTS: After screening, based on predefined inclusion and exclusion criteria

Cognitive problems related to vertebrobasilar circulation.

Neurodegenerative disorders are characterized by decreased regional cerebral blood flow. Supporting this concept, both cognitive training exercises and physical activity promote blood flow increase and correlate with healthy cognitive aging. The terminal branches of the posterior circulation supply blood to areas of the brain, such as the thalamus, hippocampus, occipital lobe, and cerebellum, involved with important intellectual functions, particularly recent memory, visual-spatial functioning, and visuomotor adaptations. Amnesia and visual agnosia may be a complication of not only posterior c

Regulation of cerebral blood flow during exercise.

Constant cerebral blood flow (CBF) is vital to human survival. Originally thought to receive steady blood flow, the brain has shown to experience increases in blood flow during exercise. Although increases have not consistently been documented, the overwhelming evidence supporting an increase may be a result of an increase in brain metabolism. While an increase in metabolism may be the underlying causative factor for the increase in CBF during exercise, there are many modulating variables. Arterial blood gas tensions, most specifically the partial pressure of carbon dioxide, strongly regulate

Cerebral protective effects of etomidate: experimental and clinical aspects.

Reduction of cerebral blood flow below a critical threshold for a protracted time interval results in irreversible metabolic events culminating in cell death. The development of agents capable of extending the tolerable ischemic interval is of great importance as such agents may allow time for therapeutic measures to be accomplished which could restore cerebral perfusion. This issue is of particular pertinence in the treatment of complex cerebrovascular diseases when local (or global) cerebral blood flow must be interrupted during vascular reconstruction. Thiopental achieved great popularity o

[Criteria for brain death: cessation of cerebral circulation demonstrated by Doppler ultrasonography of the carotid arteries].

If human death is defined by brain death, its diagnosis needs medicolegal criteria based on clinical examination and EEG. However, this evaluation could be difficult because technical or physiological limitations might impair the interpretation, especially after barbiturates and/or hypothermia. Since brain death is characterized by an intracranial circulatory arrest, methods assessing this phenomenon are warranted. Among these methods, conventional or isotopic cerebral angiography appears the most promising, but it cannot be easily performed everywhere. Because superficial blood flow in arteri

Cerebral blood flow in schizophrenia.

The regional cerebral blood flow of 27 young (mean age 24 years) schizophrenic patients was measured using the intravenous 133Xe method. The results were compared with those obtained from a group of age- and sex-matched control subjects. The main findings of the study were as follows: (a) the gray matter mean values of cerebral blood flow for both hemispheres were slightly but significantly lower in the schizophrenic group, and the statistical variance was significantly greater in the patients than in the control group; (b) the mean value for each hemisphere was found to be significantly lower

[Measurement of total cerebral blood flow in man by means of the hydrogen clearance method].

The authors discuss a new method for measuring the overall cerebral blood flow in man from analysis of the hydrogen clearance curves recorded in the internal jugular vein with a sounding platinum electrode. The hydrogen clearance curves are polyexponential in character and for calculating the blood flow it is therefore necessary to use the stochastic method. The use of the "initial inclination" method is also admissible in proximate curve analysis. The values of the overall cerebral blood flow in patients with craniocerebral injury measured by the method suggested are in agreement with the dat

Internal carotid artery agenesis: A narrative review.

BACKGROUND: Congenital anomalies of the internal carotid artery (ICA), including hypoplasia, aplasia, and agenesis, are rare vascular conditions with a prevalence below 0.01%. Most patients remain asymptomatic because of collateral cerebral circulation, yet these anomalies may cause cerebrovascular complications, particularly ischemic events and intracranial aneurysm formation. METHODS: A narrative literature review was performed to synthesize available data on demographics, embryology, pathophysiology, clinical presentation, imaging evaluation, classification systems, management, and outcome

Evaluation of the Intracranial Flow Alteration during Manual Syringe and Continuous Pump Aspiration.

GOALS: While mechanical thrombectomy (MT) has been shown to be effective in the treatment of acute large vessel occlusions, adjunctive measures, such as balloon guide catheters (BGC) and aspiration techniques, are utilized heterogeneously. Clarifying the effects of aspiration applied to the anterior cerebral circulation with proximal flow arrest can shed light on embolic protection during MT. MATERIALS AND METHODS: Manual and pump aspiration were applied through a BGC in a synthetic cerebrovascular model with a 60 ml syringe and a Penumbra pump, respectively. Flow direction was observed durin

Cerebral embolism from atrial myxoma in pediatric patients.

We describe 2 children with cerebrovascular events caused by emboli from left atrial myxomas and review 7 other pediatric cases from the literature. Transient cutaneous eruptions involving the extremities preceded the cerebrovascular events and were most likely attributable to fragmentation of the atrial tumor with peripheral embolization. Our first case demonstrates the more common presentation with acute hemiplegia caused by cerebral infarction and the second case a transient ischemic attack manifested by more subtle features as a result of involvement of the vertebrobasilar circulation. The involvement of cerebrovascular reactivity in pathogenesis of space motion sickness.

The aim of the presented work is to investigate the mechanisms of involvement of cerebral circulation in pathogenesis of space motion sickness. In experiments on chronic rabbits the 2-hours rocking and antiorthostasis served as a model of influences of the zero-gravity conditions intracranial hemo- and liquorodynamics. Cerebravascular reactivity was evaluated as a response of cerebral blood flow and intracerebral rheoencephalography to functional loads (photostimulation, CO2-inhalation, Stookey test). Shifts of water balance in the brain tissue has been assessed by means of three-frequency bra

Pathogenesis of Parkinson's disease.

Nucleolar volume and melanin pigment were measured in nerve cells of the substantia nigra in cases of idiopathic Parkinson's disease and in other cases of parkinsonism associated with cerebrovascular disease. In cerebrovascular parkinsonism, nucleolar volume is reduced by 16%, whereas melanin content is unchanged, reflecting local deficiencies in circulation on function of nerve cells, otherwise normal for age. In idiopathic parkinsonism, remaining cells are atrophied, and nucleolar volume is reduced by 16%. Melanin content is decreased by 22% because of preferential loss of the highly pigment
