Child abuse and neglect: a global phenomenon.

Child abuse and neglect are worldwide phenomena which seem to be assuming epidemic proportions in the U.S.A. Four conditions are usually considered under child abuse and these include physical abuse, physical neglect, sexual abuse and emotional maltreatment. The various predisposing factors to abuse or neglect of children in developed and developing countries are considered. The apparent low incidence of cases of child abuse in Nigeria may not be unrelated to our inability to recognize and report cases as they occur. The primary health physician therefore has a challenge to suspect, detect, tr…

The medical evaluation of the sexually abused child.

The diagnosis of suspected sexual abuse is nearly always based on a description of abuse provided by the child. The physician should invite a child to describe his or her sexual victimization in detail if such a "disclosure" interview has not already occurred, if the child seems ready to describe the abuse, and if a child protective services worker has not yet been notified of the suspected abuse. If an allegedly abused child has already been carefully interviewed, however, the physician should instead obtain information from the child's parents or other appropriate adults to determine how bes

Diagnostic and surgical implications of child abuse.

One hundred fifty-six children younger than 13 years of age with blunt abdominal injuries were reviewed, and those injured in accidents (89%) were compared with those injured by child abuse (11%). Abused children were younger (mean age, 2 1/2 years) and all presented late to medical attention with a history that was inconsistent with their physical findings. Only 65% of abused children had physical or roentgenographic signs of prior abuse, while 35% had no signs of prior abuse. The abdominal organs injured were different; 61% of accidentally injured children suffered injuries to a single, soli…

[Diagnosis and sequelae of sexual child abuse].

This paper reviews briefly epidemiological and clinical data on prevalence and incidence of child-sexual-abuse. Individual symptoms and family factors of intra-familial child sexual abuse are described and some possible psychopathological factors and vulnerability factors are listed. Two different forms of parasuicide and self-mutilating behaviour are distinguished and complicating factors of secondary traumatisation are described.

Individual psychotherapy for the sexually abused child.

This paper describes one Center's experience using individual psychotherapy to help the sexually abused child. The approach described rests upon a five-part conceptualization of the traumatic experience: trauma, threat to ontogeny, neglect and emotional unavailability by the caregiver, child's feeling of exploitation, and the child's adaptation. The process of treatment is divided into three phases: starting, middle and closing. Some common issues which occur are guilt, loss and anger, as well as alterations in the child's sexual feelings. The psychotherapeutic approach to these and other issu

Evaluation of child abuse reporting by physicians.

There is little information concerning child abuse reporting by physicians. The present study, a questionnaire survey, was therefore designed to examine several aspects of child abuse reporting by practicing physicians in Virginia. Physicians diagnosed few abused or neglected children in their practices. More than 90% saw five or fewer cases, and 26% encountered no abuse or neglect at all during the preceding year. Most physicians were inclined to report all diagnosed cases of physical abuse (91%) and sexual abuse (92%), but fewer reported all cases of physical neglect (58%), emotional abuse (…

Rhabdomyolysis and myoglobinuria as manifestations of child abuse.

Rhabdomyolysis should be suspected in cases of physical child abuse in which there is extensive soft tissue injury. It is easily investigated using the urinalysis and serum CPK levels. Renal failure is the most common complication and manifests itself as acute tubular necrosis, sometimes accompanied by the following specific laboratory abnormalities: elevated creatinine-to-BUN ratio, hyperkalemia, and myoglobinuria. Treatment is aimed at the preservation of renal function and the prevention of complications caused by electrolyte abnormalities. A full recovery can be expected for adults with th…

The range of factors identified as related to child maltreatment has expanded over the years. The literature clearly calls for an ecological … concerning interaction patterns in abusive, neglectful, and normal families. Advantages of viewing child abuse in the general context of family interaction are illustrated by the work of Patterson and his colleagues and by Gelles.(ABSTRACT TRUNCATED AT 400 WORDS)

Fractures in infants: a sign of child abuse.

Fractures occurring in children under one year of age were reviewed retrospectively. Forty-nine such patients presented to the emergency department at Children's Hospital of Michigan in 1978. Twenty-two of these fractures were thought to represent physical abuse. Physical abuse was considered when: 1) the history was incompatible, discrepant, variable or unexplained; 2) associated physical findings were present; 3) unusual fractures were present; 4) there was a known history of previous abuse/neglect; 5) there had been delay in seeking medical aid. A suggested method of dealing with this parti

Retinopathy in the sexually abused battered child.

Two cases of Purtscher's retinopathy in sexually abused battered children were observed. The major ocular findings were severe diffuse subretinal, retinal, and preretinal hemorrhages. Sexual trauma in conjunction with shaking may have produced sufficient changes in intraabdominal and intrathoracic pressure to account for retinal and intracranial hemorrhage. A prompt examination by an ophthalmologist is recommended for all children who are victims of physical abuse.

Life crisis as a precursor to child abuse.

A number of theories have been proposed on the causation of child abuse, and many of these theories assign some role to stress. Stress is variously conceptualized as a continuous state resulting from poverty, poor housing, and the like, or as an immediate crisis such as an argument or the breakdown of an appliance. An alternate perspective on stress identifies it with life change events which require readjustment in the lifestyle of a person. When an excessive number or magnitude of such life change events occur, the person affected may be said to be in a state of life crisis. Such states of l…

Radiographers' emotional reactions to suspected child abuse.

INTRODUCTION: In cases of suspected physical abuse (SPA) in children, a full skeletal survey is performed. Radiographers performing the survey may experience emotional challenges during the examination. The purpose of the study was to explore the emotional aspects of SPA imaging among radiographers and how they respond to potential challenges. METHODS: Data were collected from 8 paediatric radiographers with responsibility for SPA imaging working in three Danish university hospitals using semi-structured in-depth interviews. Interviews were carried out by an experienced radiographer and a soc

Identifying Potential Child Abuse through Oral Examination.

Limited reports of oropharyngeal trauma exist in the literature even though this type of injury is extremely common in pediatric populations. There are no widely agreed upon diagnostic and management tools for such injuries in abuse cases, emphasizing the importance of reporting rare cases of orofacial trauma. This case report of a soft palate laceration demonstrates an instance of initially unrecognized potential child abuse. We aim to clarify understanding of such injuries. Furthermore, the report highlights the need for recognition of oral signs of child abuse in order to promote early dete…

School Nurses Avoid Addressing Child Sexual Abuse.

Child sexual abuse (CSA) is a global public health problem with major consequences for the individual child and society. An earlier Swedish study showed that the school nurses did not initially talk about nor mention CSA as one form of child abuse. For the child to receive adequate support, the disclosure is a precondition and is dependent on an available person prepared to listen. The aim of the study was to explore the ability of the school nurses to detect and support sexually abused children. It is a secondary analysis of focus group interviews with school nurses. Thematic analysis was per…

Parent-Focused Prevention of Child Sexual Abuse.

Child sexual abuse (CSA) is a serious public health issue. Current after-the-fact approaches to treating victims and punishing offenders are not adequate to address a problem of this magnitude; development and rigorous evaluation of CSA prevention strategies are critical. We propose that CSA prevention efforts should target parents of young children. Parents have been neglected as a focus of CSA prevention; they merit attention given their potential to improve children's safety via effective communication and monitoring. This paper provides an overview of current strategies for reducing CSA pr
