Making Genomic Data Actionable in Electronic Health Records: Practical Viewpoint on Standards, Governance, and Implementation.

Electronic health records (EHRs) are central to patient-centered care. However, many current EHR systems do not represent, exchange, or reuse genomic findings in structured, interoperable, and clinically actionable forms. Instead, genomic results are often retained in static reports. This limits their use in routine clinical care and clinical decision support. It also restricts their future reinterpretation as scientific knowledge evolves, as well as their reuse for research within appropriate governance frameworks. To support precision and personalized medicine, we advocate that clinically re

An Affordable Artificial Intelligence Solution for Intelligent Document Processing of Faxed Documents.

Despite widespread adoption of electronic health records (EHRs), health systems remain heavily dependent on faxed documents for critical patient information. At New York University Langone Health, this represents nearly 20 million document-pages per year - laboratory results, consult notes, imaging prescriptions, refill requests, and prior authorizations - each requiring manual review and indexing. These workflows are time consuming, involve multiple staff touchpoints, can be prone to error, and may create delays for patients awaiting follow-up care. To provide the highest quality of care to p

How to make a specific diagnosis of panniculitis on clinical grounds alone: an integrated pathway of general criteria and specific findings.

A clinical approach to the vexing problem of diagnosis of panniculitis is traced in this paper, in order to obtain from the clinical findings, history and laboratory data of the patient useful, detailed and precise information, essential to address dermatologists to a specific clinical diagnosis of panniculitis. This approach is created in the same way as when a dermatologist faces any other dermatological disease, be it inflammatory or neoplastic. A common behavior in case of panniculitis is in fact just to take an adequate biopsy and wait for the pathologist report. This is indeed a limitati

Rapid bacterial diagnostics at the point of care: emerging electrochemical and Raman-based approaches.

Bacterial infections impose a major clinical and economic burden, which is likely to increase further as antimicrobial resistance rises, reinforcing the need for early detection to accelerate and better target therapy. Conventional culture-based diagnostics are well established, but they are too slow to inform early therapeutic decisions. This review frames the growing need for rapid bacterial diagnostics in the context of acute infection, antimicrobial stewardship, and point-of-care testing. We focus on emerging electrochemical biosensing and Raman-based approaches and compare them with estab

[Clinical reasoning and decision making in the practice. A woman with abdominal complaints].

A female patient aged 28 years suffered from recurrent episodes of diarrhoea. Giardia lamblia was isolated once. By rigorous application of basic clinical skills, such as thorough history taking and physical examination, together with laboratory tests the correct diagnosis was established (in this patient systemic lupus erythematosus). Two important aspects of clinical reasoning, viz. returning to the patient when new information becomes available (cyclic way of working) and pattern recognition ensured a fruitful diagnostic process.

Automated Monkeypox Identification from Electronic Medical Records Using Large Language Models - Shenzhen City, Guangdong Province, China, 2023-2025.

INTRODUCTION: Monkeypox (mpox) is an emerging infectious disease with diverse clinical manifestations that complicate early case identification. Traditional diagnostic methods relying on laboratory confirmation may delay detection during the early stages of infection. Clinical free-text data in electronic medical records (EMRs), such as chief complaints and histories of present illness, contain valuable information for early identification yet remain underutilized in routine surveillance. This study explored an automated approach to mpox identification from EMRs using large language models (LL

Practical Recommendations on Laboratory Monitoring in Patients with Atopic Dermatitis on Oral JAK Inhibitors.

Oral Janus kinase inhibitors (JAKi), a class of advanced targeted systemic therapy, have demonstrated efficacy and safety in the treatment of moderate-to-severe atopic dermatitis (AD). Like other small molecules, oral JAKi have the potential for off-target effects including laboratory-related adverse events (AEs). Product labels for oral JAKi recommend an initial laboratory assessment and follow-up 4-12 weeks later to monitor for potential changes, based on evidence from clinical trials across therapeutic indications for oral JAKi, which may not reflect a population of moderate-to-severe AD pa

Plasmodium knowlesi - Clinical Isolate Genome Sequencing to Inform Translational Same-Species Model System for Severe Malaria.

Malaria is responsible for unacceptably high morbidity and mortality, especially in Sub-Saharan African Nations. Malaria is caused by member species' of the genus Plasmodium and despite concerted and at times valiant efforts, the underlying pathophysiological processes leading to severe disease are poorly understood. Here we describe zoonotic malaria caused by Plasmodium knowlesi and the utility of this parasite as a model system for severe malaria. We present a method to generate long-read third-generation Plasmodium genome sequence data from archived clinical samples using the MinION platfor

Guide to Utilization of the Microbiology Laboratory for Diagnosis of Infectious Diseases: 2024 Update by the Infectious Diseases Society of America (IDSA) and the American Society for Microbiology (ASM).

The critical nature of the microbiology laboratory in infectious disease diagnosis calls for a close, positive working relationship between the physician and the microbiologists who provide enormous value to the health care team. This document, developed by experts in both adult and pediatric laboratory and clinical medicine, provides information on which tests are valuable and in which contexts, and on tests that add little or no value for diagnostic decisions. Sections are divided into anatomic systems, including Bloodstream Infections and Infections of the Cardiovascular System, Central Ner

A Reproducible Admission Biochemical-Coagulation Scoring Protocol for Multiple Organ Dysfunction Syndrome in Acute Pancreatitis.

Early recognition of multiple organ dysfunction syndrome in acute pancreatitis remains difficult because organ deterioration may occur before conventional severity assessments are complete. This article describes a reproducible protocol for developing, validating, and applying an admission biochemical-coagulation composite score based on routine laboratory tests obtained within 24 h of hospital admission. Multiple organ dysfunction syndrome was selected as the primary endpoint because it reflects clinically consequential dysfunction across more than one organ system and directly informs early

[Laboratory logistics for infectious diseases--from clinical laboratory physicians].

The diagnosis of bacterial meningitis rests on examination of the CSF. The gross appearance of the fluid may be cloudy or turbid if the white cell count is elevated. Gram's staining should always be used in examining CSF, as it permits rapid and accurate identification of the etiologic agent in approximately 60 to 90 percent of cases of bacterial meningitis. The CSF culture is positive in approximately 70 to 85 percent of patients with bacterial meningitis. Many other rapid diagnostic tests have been developed to aid in the diagnosis of bacterial meningitis when Gram's staining gives negative

Cohort Profile: The Central Denmark Cancer Cohort.

PURPOSE: Cancer outcome studies frequently utilize registry data, which provide large-scale population-level information. However, these registries often lack detailed clinical information regarding comorbidities, lifestyle factors, and in-hospital treatments. The Central Denmark Cancer Cohort (CDCC) was established to address these limitations by linking medical and administrative registry data with clinical electronic medical records (EMR), to facilitate research to better understand the clinical course of cancer and its complications. PATIENTS AND METHODS: The CDCC includes all patients wi

Informative missingness: What can we learn from patterns in missing laboratory data in the electronic health record?

BACKGROUND: In electronic health records, patterns of missing laboratory test results could capture patients' course of disease as well as reflect clinician's concerns or worries for possible conditions. These patterns are often understudied and overlooked. This study aims to identify informative patterns of missingness among laboratory data collected across 15 healthcare system sites in three countries for COVID-19 inpatients. METHODS: We collected and analyzed demographic, diagnosis, and laboratory data for 69,939 patients with positive COVID-19 PCR tests across three countries from 1 Jan

Human-like conversational agents as social partners: a scoping review of socioaffective mechanisms, well-being outcomes, risks and governance in the post-Turing era.

INTRODUCTION: Large language models have evolved from laboratory demonstrations into mass-market companion-style conversational agents that many users treat as social partners. As these systems produce increasingly human-like conversational behavior, users may attribute mind, form affective bonds, disclose sensitive information, and rely on agents for emotional support, creating both potential benefits and psychosocial risks. METHODS: We conducted a PRISMA-ScR-informed scoping review of socioaffective human-artificial intelligence interaction research published or posted from 2016 to 15 Janua

CRISPR-Cas genome editing system in the diagnosis and therapy of infection caused by herpes simplex virus type 1 (Orthoherpesviridae: Alphaherpesviridae: Simplexvirus: Simplexvirus humanalpha1).

Herpes simplex virus type 1 (HSV-1), newly named as Simplexvirus humanalpha1 is one of the most common pathogens in the human population, which can cause severe disease, often with fatal outcomes. Diagnostic methods currently in use are specific and sensitive, but time-consuming, require expensive laboratory equipment and highly qualified personnel. Existing therapeutic agents have a number of significant drawbacks. To successfully treat and prevent the spread of the infection, new rapid, easy-to-use, and highly sensitive diagnostic tools and effective therapeutic agents are required. One appr

Increase in Vibrio parahaemolyticus infections associated with consumption of Atlantic Coast shellfish--2013.

Vibrio parahaemolyticus (Vp) is found naturally in coastal saltwater. In the United States, Vp causes an estimated 35,000 domestically acquired foodborne infections annually, of which most are attributable to consumption of raw or undercooked shellfish. Illness typically consists of mild to moderate gastroenteritis, although severe infection can occur. Demographic, clinical, and exposure information (including traceback information on implicated seafood) for all laboratory-confirmed illnesses are reported by state health departments to CDC through the Cholera and Other Vibrio Surveillance syst

Clinical manifestations and diagnosis of lyme borreliosis.

Lyme borrelosis is a multi-systemic disease caused byBorrelia burgdorferisensu lato. A complete presentation of the disease is an extremely unusual oberservation, in which a skin lesion follows a tick bite, the lesion itself is followed by heart and nervous system involvement, and later on by arthritis; late involvement of the eye, nervous system, joints and skin may also occur. Information on the relative frequency of individual clinical manifestations of Lyme borreliosis is limited; however, the skin is most frequently involved and skin manifestations frequently represent clues for the diagn

Endocrine complications of AIDS and drug addiction.

The endocrine and metabolic consequences of illicit drug use and HIV disease are extensive and profound. Both narcotic drug use and AIDS have the capacity to cause clinically significant multiglandular derangements. Admittedly, we were not able to focus as much attention on the less frequently occurring disturbances of calcium, phosphorus, or folate metabolism in HIV disease. Similarly, we reported very little information about the endocrinologic significance of the use of classes of narcotics other than opiates and to a far lesser extent cocaine. Even with these limitations, the spectrum of d
