INTRODUCTION: Clostridioides difficile infection (CDI) is a clinical and laboratory diagnosis. Populations at higher risk of developing disease require a high clinical index of suspicion for laboratory testing to avoid incorrect assumptions of colonization. Common risk factors include recent antibiotic use, elderly (>65 years old), and immunocompromised patients. C. difficile assays should be ordered in an algorithm approach to diagnose an infection rather than colonization. Screening tests are widely available in hospital systems, but novel molecular testing may aid in diagnosis in patients w

Clostridioides difficile, a Gram-positive anaerobic bacterium, is the leading cause of hospital-acquired antibiotic-associated diarrhea worldwide. The severity of C. difficile infection (CDI) varies, ranging from mild diarrhea to life-threatening conditions such as pseudomembranous colitis and toxic megacolon. Central to the pathogenesis of the infection are toxins produced by C. difficile, with toxin A (TcdA) and toxin B (TcdB) as the main virulence factors. Additionally, some strains produce a third toxin known as C. difficile transferase (CDT). Toxins damage the colonic epithelium, initiati

The role of Clostridioides (C.) difficile as an enteropathogen in dogs is controversial. In humans, intestinal bile acid-dysmetabolism is associated with C. difficile prevalence. The relationship between fecal qPCR-based dysbiosis index (DI) and especially the abundance of bile acid-converting Clostridium hiranonis with the presence of C. difficile in dogs was explored across the following 4 cohorts: 358 fecal samples submitted for routine diagnostic work-up, 33 dogs with chronic enteropathy, 14 dogs with acute diarrhea, and 116 healthy dogs. Dogs that tested positive for C. difficile had sign

The updated Czech guidelines differ in some aspects from the 2021 guidelines issued by the ESCMID Study Group for Clostridium difficile. The key points of these Czech recommendations may be summarized as follows: • The drug of choice for hospitalized patients is orally administered fidaxomicin or vancomycin. In outpatients with a mild first episode of C. difficile infection, metronidazole can also be used. • If the patient's response to treatment is good and there are no complications, the duration of antibiotic treatment can be reduced (e.g. to 5 days in case of fidaxomicin or to 6-7 days in

Clostridioides difficile is a Gram positive spore-forming rod and mainly responsible for nosocomial diarrhea in developed nations. Molecular and antimicrobial surveillance is important for monitoring the strain composition including genotypes of high epidemiological importance such as ribotype 027 (RT027) and corresponding resistance patterns. 1535 isolates obtained from samples sent between 2014 and 2019 to the German National Reference Center (NRC) for diagnostic reasons (NRC strain set), and 1143 isolates from a Tertiary Care University Center in Saarland, Germany (non-NRC strain set), were

BACKGROUND: The intestinal microbiota plays an important role in the pathogenesis of Clostridioides difficile-associated diarrhea, and regional and racial characteristics influence the microbiome composition and diversity. We investigated the intestinal microbiome characteristics of patients with C. difficile colitis (CD+) compared to those of patients with colitis not due to C. difficile (CD-), patients with vancomycin-resistant enterococci (VRE) colonization, and healthy controls, in Korea.
MATERIALS AND METHODS: We collected stool samples from 24, 18, 11 and 13 subjects within CD+, CD-, VR

The Clostridioides difficile infection (CDI) is one of the most common nosocomial infections around. The key pathogenicity factors of this bacterium include the toxins A and B that cause the disease symptoms. Neutralization of these toxins is one of the promising strategies for CDI treatment. Here, we isolated a panel of single-domain antibodies to the toxin B CROPs domain. Two antibodies, TB5A7 and TB4A8, exhibiting potent neutralizing activity were modified to produce homodimeric forms. The TB5A7 and TB4A8 dimers were characterized by enhanced neutralizing activity and protected animals from

Peritoneal dialysis (PD) associated peritonitis is a common complication and main cause of PD failure among end-stage chronic kidney disease (CKD) patients, presenting with abdominal pain, diarrhea and cloudy dialysate. On the other side, these patients may have an increased risk for Clostridioides difficile infection (CDI), with a clinical presentation indistinguishable from PD associated peritonitis. We present the case of a female patient who presented with multiple episodes of watery diarrhea days after PD catheter insertion, treated initially as a suspected PD associated peritonitis but l

UNLABELLED: Clostridioides difficile is a leading cause of healthcare-associated infections and remains an urgent global public health concern. Current FDA-approved therapies for C. difficile infection (CDI) are limited and are frequently associated with treatment failure and high rates of disease recurrence. To address this unmet clinical need, we employed a drug repurposing strategy focused on FDA-approved compounds with established safety profiles and limited systemic absorption characteristics, which are desirable for localized gastrointestinal infections. Through this approach, we identif

OBJECTIVES: The molecular detection of toxigenic Clostridioides difficile (C. difficile) infections mostly relies on the use of unpreserved stool specimens. However, recently the feasibility of sampling systems for stool pathogen detection comprising flocked swabs and preservation media has been highlighted. Hence, we evaluated the Copan FecalSwab stool sampling systems for the detection of toxigenic C. difficile by the Cepheid Xpert C. difficile BT PCR assay.
METHODS: Unpreserved stool samples from patients with suspected toxigenic C. difficile infections were tested for C. difficile using t

Clostridioides difficile infection is a prevalent healthcare-associated infection that can lead to severe gastrointestinal complications, including gastrointestinal bleeding. This case report presents a 77-year-old female with multiple comorbidities, including a history of cerebral infarction, who was on long-term aspirin therapy. She initially presented with melena, suggestive of upper gastrointestinal bleeding. Preliminary treatment with pantoprazole and somatostatin resolved the melena. However, she later developed dark red blood per rectum, indicating a shift to lower gastrointestinal blee

Clostridioides (C.) difficile is a spore-forming, toxin-producing nosocomial human gut pathogen and a causative agent of gastrointestinal infections, leading to mild to severe diarrhea. Severe C. difficile infections (CDI) can cause life-threatening conditions, such as pseudomembranous colitis, colonic perforation, or toxic megacolon. The main virulence factors of C. difficile and responsible for CDI symptoms are two AB-type protein toxins, toxin A (TcdA) and toxin B (TcdB). TcdA and TcdB are large, single-chain proteins with multiple domains and glucosyltransferase activity. After receptor-me

Clostridioides difficile infection (CDI) is a major healthcare-associated concern, particularly in the elderly. While CDI typically manifests as toxin-mediated colitis, hematogenous dissemination leading to bacteremia is relatively rare and associated with high mortality. We report an unusual case of C. difficile bacteremia in an octogenarian female patient occurring shortly after right hemicolectomy for ascending colon adenocarcinoma. This critically ill patient presented with bloodstream infection in the ICU following surgery. Multiple risk factors converged, including advanced age, extensiv

Antimicrobial resistance is emerging in clinical strains of Clostridioides difficile. Ibezapolstat (IBZ) is a DNA polymerase IIIC inhibitor that has completed phase II clinical trials. IBZ has potent in vitro activity against wild-type, susceptible strains but its effect on C. difficile strains with reduced susceptibility to metronidazole (MTZ), vancomycin (VAN), or fidaxomicin (FDX) has not been tested. The primary objective of this study was to test the antibacterial properties of IBZ against multidrug-resistant C. difficile strains. The in vitro activity, bactericidal, and time-kill activit
