[Post-traumatic corrective osteotomy of the distal radius with a new plate].

PURPOSE OF THE STUDY: Mal-union of distal radius fractures cause functional limitation and progressive degenerative changes of the wrist. The authors evaluate both radiograph and functional outcomes in a group of patients with a post-traumatic deformation of the distal radius treated by corrective osteotomy and fixation by a newly developed T-plate. MATERIAL: A retrospective evaluation covered a group of 13 patients of the average age of 51 years (range, 35-60 years) operated on in 2001 for post-trauma symptomatic mal-union of the distal radius of the Colles type. The group included only pati

Modified Desmanet's intramedullary pinning for fractures of the distal radius.

The purpose of this article is to present the results of Desmanet's intramedullary pinning for distal radius fractures, and also to present … ons that occur with Desmanet's original method. The present method is also good for extra-articular Colles' fracture. Concerning intraarticular Colles' fractures, the undisplaced type and the ulnar split type are good indications for this method. This method can also be used for the dorsal split depression type, but the ulnodorsal split type is not a good indicati …

Ulnar tunnel syndrome caused by anomalous muscles. Case report.

Two cases of ulnar tunnel syndrome at the wrist are described. In both cases an anomalous hypothenar muscle, flexor digiti minimi brevis accessorius, was in part the causative factor. Contributing to the symptoms of nerve compression was swelling associated with rheumatoid disease and Colles' fracture, respectively. Decompression of the nerve including subtotal removal of the anomalous muscle effected complete recovery. Its phylogeny, morphology and clinical significance are discussed in relation to previously described anomalies of the hypothenar muscles.

Cineradiographic study of wrist motion after fracture of the distal radius.

Residual dorsal tilt is one of the main deformative factors in the malunion of fractures of the distal radius. We investigated the effect of such tilt on the range of wrist motion and on carpal alignment in a cineradiographic study of wrist motion in 30 patients with extra-articular Colles' fracture. With increased dorsal tilt, the range of wrist motion became more restricted and abnormalities of carpal alignment during wrist motion became apparent. When the dorsal tilt was less than 10 degrees, the effects on the range of motion and on the dynamic carpal alignment were small.

An evaluation of the United Kingdom National Osteoporosis Society position statement on the use of peripheral dual-energy X-ray absorptiometry.

A recent position statement issued by the UK National Osteoporosis Society recommends a triage approach to the use of peripheral dual-energy X-ray absorptiometry (pDXA) devices. Patients with a forearm T-score greater than -1 or less than -2.5 are regarded as normal or osteoporotic, respectively, while those with a T-score between -1 and -2.5 are sent for further assessment with spine and hip DXA. We have evaluated the NOS pDXA algorithm by comparing it with the alternative strategies of relying on forearm BMD alone, or performing spine and hip DXA in every patient. The evaluation was carried

The radio-radial external fixator in the treatment of fractures of the distal radius.

The technique of radio-radial monobloc-fixation with the small AO external fixator device has been applied to 17 consecutive Colles' fractures. The fracture types were mainly A3 and C2, according to the AO classification. We found this technique to be easy and quick in application and stable in fixation. Direct, precise and atraumatic reduction can be achieved by using the distal pins as joy-sticks. Furthermore, disimpaction of the fracture to regain length is possible without bone grafting. Normal carpal mobility and load transfer is preserved during fracture healing and the injured hand can …

Epidemiology of vertebral osteoporosis.

Whereas the extent, morbidity and costs of hip and Colles' fracture are well recognised from epidemiological studies, those arising from vertebral fracture are less secure. Reasons relate to the uncertain definition of vertebral fracture and its variable clinical expression, and hence its incidence is not known. Utilising radiological criteria 50% or more of vertebral fractures may be asymptomatic. In the remainder morbidity is significant, particularly in the presence of multiple vertebral fractures. Although the true incidence of vertebral fracture is unknown there is evidence that it increa…

Upper extremity disorders in women.

Carpal tunnel syndrome, Colles' fracture, and osteoarthritis of the basilar joint of the thumb are only three of the many upper extremity conditions that preferentially affect women. With more and more women entering the workplace, these conditions become more disruptive of patients' lifestyles and even may be increasing in incidence. Orthopaedic surgeons traditionally have focused on the surgical treatment of patients with these conditions, but it is becoming increasingly clear that surgeons also must focus on delineating pathophysiology to better identify those individuals at risk and help p…

Prilocaine leakage during tourniquet inflation in intravenous regional anaesthesia: the influence of fracture manipulation.

Recent evidence has shown that there may be leakage of anaesthetic drugs into the systemic circulation during the tourniquet inflation phase of intravenous regional anaesthesia. This, together with increased pressure in forearm vessels during manipulation of fractures, prompted the measurement of arterial levels of local anaesthetic to assess potential toxicity. Prilocaine levels were measured in nine patients undergoing manipulation of Colles' fractures and six patients undergoing other upper limb procedures. Eleven patients had measurable concentrations of prilocaine in the arterial circulat…

Epidemiology of osteoporosis.

Fragility fractures are now recognised as a major problem of public health. Although the prevalence of all fractures is similar among men and women, the vast majority of osteoporotic fractures occur in elderly women. These comprise vertebral compression fractures, Colles fractures at the wrist, and hip fracture, and to a lesser extent fractures at other sites. The incidence of vertebral and hip fracture increases exponentially with age. The reasons for this relate in part to the lower bone density of women at the time of maturity (peak bone density), and the accelerated bone loss that occurs a

[Conservative treatment of classical radius fractures].

In Colles' fractures of the radius the distal fragment is slanted dorsally and radially, dislocated dorsally and radially and supinated. The radius is shortened. Furthermore a comminution-zone of the dorsal cortex is found. In closed reduction the fracture is unwedged and reduced by recreating the mechanism of trauma in neutral position, rather a light dorsiflection is maintained to check secondary dislocation of the distal fragment and shortening of the radius. The cast should push the carpus slightly palmwards and hold the hand in light ulnar-duction. Immobilization is retained for four to s…

Ulnar shortening osteotomy as a treatment of symptomatic ulnar impaction syndrome after malunited distal radius fractures.

A malunited distal radius fracture can lead to symptomatic ulnar impaction syndrome, which is a common cause for ulnar-sided wrist pain. If conservative treatment fails and symptoms persist after an arthroscopic ulnocarpal debridement, ulnar shortening osteotomy (USO) is the treatment of choice. Since the first USO described by Milch in 1941 after a malunited Colles fracture, many techniques have been described varying in surgical approach, type of osteotomy and osteosynthesis material used. Many studies demonstrated good to very good functional results after USO, reporting, however, a delayed

[What wrist fracture did Pouteau actually describe?].

The so-called " Pouteau-Colles" fracture was never specifically described by Pouteau. This surgeon from Lyons wrote a paper published in his posthumous works in 1783. The author was allowed access to an original edition of the paper at the "Ancienne Ecole de Médecine Navale de Rochefort". When reading this paper, it seems that Pouteau described a great variety of different kinds of forearm fractures. His original contribution was to define the mechanism of injury as occurring during a fall on the outstretched hand as opposed to direct injury. Pouteau presents a hypothetical muscular theory of

Management of fractures of the distal radius: surgeon's perspective.

The new millennium represents a time for expanding our present knowledge of the treatment of distal radius fractures, based on the foundation of information that has been gathered over the past century. Treatment-oriented classifications have replaced the prior generalization applied to all "Colles fractures" and have directed preoperative planning. Newer external fixation frames, improved surgical technique, and superior instrumentation allow for safer reproducible ligamentotaxis. These current concepts of treatment along with a comprehensive therapy plan have provided the basis for enhanced

A model of lifetime osteoporosis impact.

The study goal was to use population-based data to model aspects of lifetime osteoporosis impact not previously studied, specifically: (1) to estimate person-years of fracture-related functional impairment against the trajectory of functional status in the general population; (2) jointly to consider hip, vertebral, and Colles' fractures in estimating the percent of women who will ever fracture; and (3) to estimate the lifetime number of fractures expected in a cohort of 10,000 50-year-old white postmenopausal women. The model estimates that 54% of 50-year-old women will sustain osteoporosis-re…
