The irritable bowel syndrome is a common chronic disorder having a broad clinical spectrum of severity. Although only a small proportion of those afflicted seek medical help for their symptoms, a subset have severe and intractable symptoms. A positive diagnosis should be established from the history and physical examination; endoscopic and radiologic investigations should be minimized. We suggest that the physician also assess the severity of the illness based on its symptomatic and functional features and the patient's behavioral response. Classifying the disorder in this manner permits a gra

Spastic Colitis (SC) of the fifties has been deleted and included in the Irritable Bowel Syndrome, as inflammation is not present, and the disturbed intestinal motility may involve the small and large intestine. With the advent of colonoscopy, we could investigate this generalized common term. Between 1983 and 1988 we have studied 120 documented cases of SC by colonoscopy with multiple biopsies. In every patient, we relied on consecutive procedures to confirm the diagnosis. They included: Stool examination, barium enema, small bowels series, colonoscopy with multiple biopsies. Serological diag

The human gastrointestinal tract is a complex system of digestive pathways aided by mechanical processes, enzymes, transport molecules, and colonic bacteria. Occasionally, these bacterial components transplant to atypical locations due to various gastrointestinal imbalances or anatomical structural issues. This may lead to bacterial overgrowth of the small intestine, where minimal or no bacteria are normally found. Symptoms of small intestinal bacterial overgrowth may mimic those of various functional gastrointestinal diseases. Small intestinal bacterial overgrowth is typically diagnosed throu

Functional gastrointestinal disorder (FGID) is common and may affect any part of the digestive tract from the esophagus to the rectum. Functional dyspepsia and the irritable bowel syndrome (IBS) are the most commonly recognized and until recently were considered distinct entities. In recent years, however, new observations and studies of the afferent nervous system have extended our concepts of both IBS and dyspepsia and suggest that these conditions may have common triggers and expression from similar pathophysiological processes.

Neutrophils are important innate immune cells with plasticity, heterogenicity, and functional ambivalency. While bone marrow is often regarded as the primary source of neutrophil production, the roles of extramedullary production in regulating neutrophil plasticity and heterogenicity in autoimmune diseases remain poorly understood. Here, we report that the lack of wingless-type MMTV integration site family member 5 (WNT5) unleashes anti-inflammatory protection against colitis in mice, accompanied by reduced colonic CD8+ T cell activation and enhanced splenic extramedullary myelopoiesis. In add

The alkaline pH-activated, two-pore domain K(+) channel K2P5.1 (also known as TASK2/KCNK5) plays an important role in maintaining the resting membrane potential, and contributes to the control of Ca(2+) signaling in several types of cells. Recent studies highlighted the potential role of the K2P5.1 K(+) channel in the pathogenesis of autoimmune diseases such as rheumatoid arthritis and multiple sclerosis. The aim of the present study was to elucidate the pathological significance of the K2P5.1 K(+) channel in inflammatory bowel disease (IBD). The degrees of colitis, colonic epithelial damage,

Concentrations of the candidate endocrine and paracrine peptide, peptide YY, were measured by specific radioimmunoassay in tissue extracts prepared from normal ileum, normal colon, Crohn's disease, and ulcerative colitis. In both the ascending and descending colon, there were significantly decreased mean concentrations of peptide YY in Crohn's colitis and ulcerative colitis, compared to mean concentrations in normal colon. There was no age-related decrease of concentrations of peptide YY in normal colon. The decrease in concentrations of peptide YY in colon obtained from patients with inflamma

The gastrointestinal (GI) tract constitutes a sophisticated system integral to digestion, nutrient absorption, and overall health, with its functionality predominantly hinging on the distinctive properties of diverse stem cell types. This review systematically investigates the pivotal roles of stem cells across the esophagus, stomach, small intestine, and colon, emphasizing their crucial contributions to tissue homeostasis, repair mechanisms, and regeneration. Each segment of the GI tract is characterized by specialized stem cell populations that exhibit distinct functional attributes, highlig

The majority of foreign gastroenterologists have their doubts whether there exist "undifferentiated" colitisis. They consider these to be a variant of the irritable bowel syndrome. We have examined 173 patients. Of these, 67 percent were less than 45 years of age with significant intestinal disorders caused by chronic colitis. Diagnosis of Crohn's disease and nonspecific ulcerative colitis had been excluded. The main complaints of the patients were constipations alternated sometimes with short-time diarrhea (61%), diarrhea (13%), spasmodic pains in the inferior parts of the abdomen (20%), abdo

In the light of three cases of neonatal obstruction related to a small calibre descending colon, the authors review the main clinical and radiological signs and compare them with the data in the literature. This functional disturbance is related to immaturity of the intrinsic innervation of the colon which is especially common in low birth weight neonates or of diabetic mothers. Diagnosis is dependent upon enema using diluted uroangiographic hydrosoluble iodine contrast medium, which shows a disparity in calibre at the left colic angle. The differential diagnosis lies with Hirschsprung's disea

Patients suffering from the irritable bowel syndrome (IBS) tend to have extraintestinal symptoms. The purposes of this study were to compare the nature and severity of these symptoms in IBS patients in relation to patients with inflammatory bowel disease (IBD) and to nonpatients and to clarify the relationship between intestinal and extraintestinal symptoms. A consecutive group of male patients and a control group of age-matched male subjects were studied. Symptoms were graded for severity using a validated, self-administered inventory. There were 53 IBS patients, 55 IBD patients (32 Crohn's d

A 42 year old male that presented with functional gastrointestinal symptoms is presented. X ray examination showed an esophageal diameter of 3 cm, measured 6 cm above the gastroesophageal junction. This finding, according to previous experiences of the authors, is highly suggestive of Chagasic esophageal involvement. The patient resided in a highly endemic area for Chagas disease; hemagglutination, latex agglutination and ELISA serological tests for Chagas disease were positive. The manometric study of the esophagus confirmed the diagnosis showing an elevated lower esophageal sphincter pressur

Hirschsprung disease (HSCR) is a congenital malformation characterized by the absence of the enteric nervous system (ENS) in the distal colon, resulting from defective colonization of enteric neural crest cells (ENCCs). The underlying pathogenesis of HSCR remains incompletely understood. Here, we report that c-Cbl-associated protein (CAP), also known as sorbin and SH3 domain-containing protein 1 (SORBS1), is upregulated in the aganglionic colon tissues of children with HSCR. Functional studies revealed that CAP overexpression suppresses ENCC colonization by binding the lipid raft protein floti

Stroke survivors can experience a plethora of complications throughout their recovery that impair quality of life and impact neurological outcomes. Intestinal dysfunction is reported to occur rapidly following stroke in both humans and animal models and alterations such as reduced barrier integrity, lymphocyte loss and an altered microbiota have been suggested to contribute to increased brain injury. Despite the persistence of gastrointestinal symptoms in many stroke survivors, how the intestinal environment changes over the course of stroke recovery remains poorly understood. Here, we use an

BACKGROUND: The association of Pseudomonas aeruginosa (PsA) and Haemophilus influenzae (HI) colonization with lung function in children with primary ciliary dyskinesia (PCD) remains insufficiently defined. We aimed to compare spirometric parameters between PsA- and HI-colonized children and evaluate longitudinal changes before, at, and after chronic colonization.
METHODS: This retrospective cohort included patients aged ≥ 5 years with definite or highly probable PCD and chronic PsA or HI colonization. Chronic colonization was defined using repeated sputum cultures over 12 months. Pulmonary fu

With the aim of improving end organ treatment, we describe a new system of classifying irritable bowel syndrome (IBS) according to clinical features into four groups, spastic colon syndrome (SCS), functional diarrhea (FD), diarrhea-predominant spastic colon syndrome (DPSCS), and midgut dysmotility (MGD). The aim of the study was to investigate fasting and postprandial distal colonic motility in the four groups of patients and to compare the results with normal controls. Distal colonic motility studies were performed in the unprepared colon. 2.5-hr recordings were made from four channels with a
