Biomechanical, psychosocial, and organizational risk factors for WRMSD: population-based estimates from the Connecticut upper-extremity surveillance project (CUSP).

In a case-control study of a population-based telephone survey of 3,798 working adults, symptoms of work-related musculoskeletal disorder (WRMSD) were associated with biomechanical, psychosocial, and organizational factors. In several models, biomechanical exposures with strong associations were static postures (odds ratios [ORs] = 2.00-5.45); repeated pushing, pulling, lifting (ORs = 1.86-12.75); and repeated neck bending (ORs = 1.07-12.8). Psychosocial and organizational factors consistently retained in these models were demands (ORs = 1.26-1.59) and organizational support (ORs = 0.53-0.79).

Using Data-To-Care Strategies to Optimize the HIV Care Continuum in Connecticut: Results From a Randomized Controlled Trial.

BACKGROUND: Re-engaging people with HIV who are newly out-of-care remains challenging. Data-to-care (D2C) is a potential strategy to re-engage such individuals. METHODS: A prospective randomized controlled trial compared a D2C strategy using a disease intervention specialist (DIS) vs standard of care where 23 HIV clinics in 3 counties in Connecticut could re-engage clients using existing methods. Using a data reconciliation process to confirm being newly out-of-care, 655 participants were randomized to DIS (N = 333) or standard of care (N = 322). HIV care continuum outcomes included re-engage…

Notes from the field: Fatal gastrointestinal mucormycosis in a premature infant associated with a contaminated dietary supplement--Connecticut, 2014.

In October 2014, a hospital in Connecticut notified CDC and the Connecticut Department of Public Health of a fatal case of gastrointestinal mucormycosis in a preterm infant. The infant, born at 29 weeks' gestation and weighing 1,400 grams (about 3 pounds), had developed signs and symptoms initially consistent with necrotizing enterocolitis approximately 1 week after birth. Exploratory laparotomy revealed complete ischemia of the gastrointestinal tract from the esophagus to the rectum; a portion of necrotic cecum was sent for microscopic examination. Following surgery, the infant developed mult…

The association between history of violence and HIV risk: a cross-sectional study of HIV-negative incarcerated women in Connecticut.

PURPOSE: We examine the association between history of violence and risk for HIV infection among incarcerated women. Specifically, we consider physical violence and rape as they relate to unprotected sex with male primary and nonprimary (male or female) sexual partners among a sample of HIV negative female inmates (n = 1,588) housed in Connecticut's sole correctional facility for women between November 1994 and October 1996. METHODS: A supplement to the mandatory Connecticut Department of Correction Inmate Medical Screening/Health History was used to collect information on each woman's backgr…

Genomic diversity and novel sequence types of Pasteurella multocida from diverse animal hosts in Connecticut, United States, 2022-2023.

Pasteurella multocida is a zoonotic pathogen infecting a wide range of domestic and wild animals. Although genomic data are available for several domestic hosts, isolates from wildlife remain underrepresented. Here, we performed whole-genome sequencing of nine P. multocida isolates recovered in Connecticut between 2022 and 2023 from cows, a deer, a rabbit, a cat, and a pheasant to characterize their genetic features and enhance our understanding of the host-associated diversity. All isolates except one feline strain shared a common genotype across multiple host species, being classified as cap…

Utilization of extended-spectrum beta-lactamase (ESBL) detection systems in microbiology laboratories: survey of Connecticut hospitals from 1998-2002.

There has been recent concern raised over the detection, prevalence, and clinical implications of infection with Escherichia coli and Klebsi … his phenomenon, additional testing for suspected isolates is recommended nationally. We surveyed 28 Connecticut hospitals from 1998-2002 to determine if these institutions utilized screening and confirmation methods for suspected isolates. The number of hospitals which have implemented ESBL detection systems doubled from 11 to 22 over the study period. Curren …

Isolation of eastern equine encephalitis virus and West Nile virus from crows during increased arbovirus surveillance in Connecticut, 2000.

The emergence of the West Nile virus (WNV) in the northeastern United States has drawn emphasis to the need for expanded arbovirus surveillance in Connecticut. Although the state of Connecticut began a comprehensive mosquito-screening program in 1997, only since 1999 have there been efforts to determine the prevalence of arboviruses in bird populations in this state. Herein, we report on our results of an arbovirus survey of 1,704 bird brains. Included in this report are the first known isolations of eastern equine encephalitis virus (EEEV) from crows and data on the geographic and temporal di…

Population-based incidence and characteristics of community-onset Staphylococcus aureus infections with bacteremia in 4 metropolitan Connecticut areas, 1998.

This study retrospectively analyzed the magnitude and epidemiology of community-onset Staphylococcus aureus (COSA) infections and methicillin-resistant S. aureus (MRSA) infections in 4 Connecticut metropolitan areas (population, 1.1 million). The study looked at hospital medical records of persons admitted with S. aureus bacteremia in 1998. COSA was categorized as "health care associated," "with underlying medical condition," or "no underlying medical condition." Overall, 48% of S. aureus bacteremic infections were COSA (incidence, 17 cases/100,000 persons). Incidence increased with age and hi…

Demographic and social determinants of the interval from symptom onset to diagnosis of pediatric brain tumors: a study of the Connecticut Tumor Registry.

OBJECTIVE: Pediatric patients frequently experience long intervals between the initial onset of symptoms and the diagnosis of a brain tumor. Understanding the determinants of the prediagnostic symptomatic interval (PSI) has the potential to facilitate earlier initiation of treatment. Here, the authors studied the impact of demographic factors and social determinants of health on the PSI among pediatric brain tumor patients. METHODS: All pediatric patients with brain tumors included in the Connecticut Department of Public Health's Tumor Registry from 2004 to 2018 were reviewed. Demographic, cl…

Prevalence of extended spectrum beta-lactamase producing Escherichia coli and Klebsiella isolates in a large community teaching hospital in Connecticut.

The prevalence of extended-spectrum beta-lactamase (ESBL)-producing Escherichia coli (ESC) and Klebsiella (KS) among consecutive non-urine isolates were evaluated. Twenty-four of 392 isolates produced ESBLs. Among these half were from respiratory sites and all were susceptible to meropenem. Pulse field-gel electrophoresis (PFGE) showed 12 clonally distinct ESBLs and iso-electric focusing (IEF) revealed that most (83%) produced multiple enzymes.

Infection with agents of human granulocytic ehrlichiosis, lyme disease, and babesiosis in wild white-footed mice (Peromyscus leucopus) in Connecticut.

White-footed mice, Peromyscus leucopus, were captured in southern Connecticut during 1997 and 1998 to determine the prevalence of infections caused by granulocytic Ehrlichia sp., Borrelia burgdorferi, and Babesia microti. Of the 50 mice captured and recaptured, 25 of 47 (53.2%) and 23 of 48 (47.9%) contained antibodies to the BDS or NCH-1 Ehrlichia strains, respectively, as determined by indirect fluorescent antibody (IFA) staining methods. The majority (83.3%) of 48 mice also contained antibodies to B. burgdorferi, as determined by enzyme-linked immunosorbent assay. Moreover, 20 of 26 (76.9%)…

Rate of intrusion of maxillary incisors in Class II Div 1 malocclusion using skeletal anchorage device and Connecticut intrusion arch.

BACKGROUND: Nonsurgical correction of deep bite involves either extrusion of posterior teeth, intrusion of incisors, or combination of both. The introduction of skeletal anchorage device with microimplant provides near absolute anchorage without producing any untoward effects on anchor unit. Connecticut Intrusion Arch (CIA) provided an efficient system of intruding anterior segment without producing much adverse affects on anchor teeth. METHODS: The study comprised of 30 patients of Class II Div 1 malocclusion with overbite of >6 mm and required therapeutic extractions of all first premolars,…

Allocative efficiency analysis of strategies to reduce overdose deaths among people with opioid use disorder and history of incarceration in Connecticut.

BACKGROUND: While Connecticut has successfully slowed overdose death (ODD) rates, additional progress is necessary. We examined policies that allocate resources with maximal efficiency to reduce ODDs. METHODS: We developed a mechanistic simulation of overdose policies, including medications for opioid use disorder (OUD) among people involved in the criminal justice system both during incarceration (MOUD-INC) and post-release in the community (MOUD-COM), and naloxone in the community (NLX) to determine how maximally scaling all permutations compared to current MOUD and NLX levels (i.e., status…

Underestimation of Chrysotile Health Risk due to Under-ascertainment of Mesothelioma: Evidence from a Century of Connecticut's Experience with the "Magic Mineral".

Over a century ago, Connecticut industry began using chrysotile asbestos. Chrysotile found a home in several factories that used it exclusively or predominantly. The occurrence of mesothelioma in 4 of those factories is the subject of this paper-2 have been reported previously and are updated here with new information; one was the subject of a prior internal corporate study that was never published; one is reported here for the first time. Twenty-four cases of mesothelioma have been identified among these workers, including several who had no known amphibole exposure. It is likely that additio…

West Nile virus is predicted to be more geographically widespread in New York State and Connecticut under future climate change.

The effects of climate change on infectious diseases are a topic of considerable interest and discussion. We studied West Nile virus (WNV) in New York (NY) and Connecticut (CT) using a Weather Research and Forecasting (WRF) model climate change scenario, which allows us to examine the effects of climate change and variability on WNV risk at county level. We chose WNV because it is well studied, has caused over 50,000 reported human cases, and over 2200 deaths in the United States. The ecological impacts have been substantial (e.g., millions of avian deaths), and economic impacts include livest…
