Quantitative Assessment of Cortical Excitability in Alzheimer's Dementia and Its Association with Clinical Symptoms: A Systematic Review and Meta-Analyses.

BACKGROUND: Alzheimer's disease (AD) is characterized by cognitive and neuropsychiatric symptoms (NPS) due to underlying neurodegenerative pathology. Some studies using electroencephalography (EEG) have shown increased epileptiform and epileptic activity in AD. OBJECTIVE: This review and meta-analyses aims to synthesize the existing evidence for quantitative abnormalities of cortical excitability in AD and their relationship with clinical symptoms. METHODS: We systematically searched and reviewed publications that quantitatively assessed cortical excitability, using transcranial magnetic sti…

Complete but not partial inhibition of glutamate transporters exacerbates cortical excitability in the R6/2 mouse model of Huntington's disease.

AIM: Deficient glutamate reuptake occurs in the cerebral cortex of Huntington's disease (HD) patients and murine models. Here, we examine the effects of partial or complete blockade of glutamate transporters on excitatory postsynaptic currents (EPSCs) of cortical pyramidal neurons (CPNs). METHODS: Whole-cell patch clamp recordings of CPNs in slices from symptomatic R6/2 mice and wild-type (WT) littermates were used to examine the effects of selective or concurrent inhibition of glutamate reuptake transporters. RESULTS: Selective inhibition of the glial glutamate transporter 1 (GLT-1) or the

Reduced Cortical Excitability, Neuroplasticity, and Salivary Cortisol in 11-13-Year-Old Children Born to Women with Gestational Diabetes Mellitus.

BACKGROUND: Children exposed to gestational diabetes mellitus (GDM) in utero are at increased risk of neurodevelopmental difficulties, including autism and impaired motor control. However, the underlying neurophysiology is unknown. METHODS: Using transcranial magnetic stimulation, we assessed cortical excitability, long-term depression (LTD)-like neuroplasticity in 45 GDM-exposed and 12 control children aged 11-13 years. Data were analysed against salivary cortisol and maternal diabetes severity and treatment (insulin [N = 22] or metformin [N = 23]) during pregnancy. FINDINGS: GDM-exposed ch…

Acute and Chronic Effects of Aerobic Exercise on Motor Inhibition and Cortical Excitability in Adolescents With Attention Deficit Hyperactivity Disorder.

INTRODUCTION: Accumulating evidence suggests that acute aerobic exercise facilitates improvement in motor inhibition among adolescents with attention-deficit hyperactivity disorder (ADHD), potentially through exercise-induced modulation of cortical excitability. The present study aimed to investigate the effects of both acute and chronic aerobic exercise on motor inhibition and cortical excitability, and to elucidate the relationship between these variables in adolescents diagnosed with ADHD. METHODS: A partial cross-over experimental design was employed. Twenty-four adolescents with ADHD und…

Cortical excitability and brain function in patients with insomnia: a pilot transcranial magnetic stimulation and functional near-infrared spectroscopy study.

BACKGROUND: Insomnia significantly impairs well-being, cognitive function, and social functioning, yet subjective psychological assessments often yield equivocal results regarding the extent of this impairment. Neural function may underlie this discrepancy and offer a more precise foundation for guiding treatment. This study therefore employed non-invasive transcranial magnetic stimulation (TMS) and functional near-infrared spectroscopy (fNIRS) to investigate cortical excitability and brain activity patterns in patients with short-term insomnia disorder (SID) and chronic insomnia disorder (CID…

Transcranial magneto-acoustic stimulation enhances motor function and modulates cortical excitability of motor cortex in a Parkinson's disease mouse model.

Parkinson's disease (PD) is a neurodegenerative disorder characterized primarily by motor dysfunction. Transcranial magneto-acoustic stimulation (TMAS), an emerging non-invasive brain neuromodulation technology, is increasingly being applied in the treatment of brain diseases. However, the effects of TMAS on PD are unknown, which is not well studied. Here, we utilized TMAS on PD model mice induced by MPTP to investigate the underlying mechanism of therapy. Our study found that TMAS improved the behavioral performance of PD model mice, enhancing the motor function and motivation for movement. B

Spontaneous and perturbation-based EEG cortical excitability markers are associated with plasma p-tau181 concentration in healthy middle-aged adults.

In early-stage Alzheimer's disease (AD) amyloid-β (Aβ) deposition can induce neuronal hyperactivity, thereby potentially triggering activity-dependent neuronal secretion of phosphorylated tau (p-tau), ensuing tau aggregation and spread. Therefore, cortical excitability is a candidate biomarker for early AD detection. Moreover, lowering neuronal excitability could potentially complement strategies to reduce Aβ and tau buildup. There is, however, a lack of understanding of the relationship between cortical excitability and p-tau increase in vivo. Therefore, in a sample of 658 healthy middle-aged…

Motor cortical excitability behavior in chronic spinal cord injury neuropathic pain individuals submitted to transcranial direct current stimulation-case reports.

INTRODUCTION: Increased excitability of the motor cortex through transcranial direct current stimulation (tDCS) has been described as a non-pharmacological strategy for the treatment of Spinal Cord Injury neuropathic pain (SCINP). It is also believed that the ability to enhance motor cortex excitability (MCE) could be impaired within chronic SCINP individuals. The following case reports describe the MCE behavior in individuals with chronic SCINP submitted to electrical non-invasive neuromodulation. CASE PRESENTATIONS: This article reports 11 cases with chronic SCINP in which each individual w

Transcranial Magnetic Stimulation Combined With Electroencephalography Analysis Suggest Time-Dependent and Region-Specific Modulation of Cortical Excitability During Acute Alcohol Intoxication.

BACKGROUND: Longitudinal excessive alcohol consumption has adverse effects on brain structure and functions. The immediate effects of alcoho … l brain regions. CONCLUSIONS: The results suggest time-dependent and region-specific modulation of cortical excitability during acute alcohol intoxication. Alcohol's effects are dynamic, regionally heterogeneous, and not limited to simple global suppression. TMS-EEG offers a promising tool for assessing alcohol-related changes in cortical functions.

Time-dependent enhancement of corticospinal excitability during cortico-cortical paired associative stimulation of the hV6A-M1 network in the human brain.

Cortico-cortical paired associative stimulation (ccPAS) is a powerful transcranial magnetic stimulation (TMS) protocol thought to rely on Hebbian plasticity and known to strengthen effective connectivity, mainly within frontal lobe networks. Here, we expand on previous work by exploring the effects of ccPAS on the pathway linking the medial posterior parietal area hV6A with the primary motor cortex (M1), whose plasticity mechanisms remain largely unexplored. To assess the effective connectivity of the hV6A-M1 network, we measured motor-evoked potentials (MEPs) in 30 right-handed volunteers at

Alcohol consumption confers lasting impacts on prefrontal cortical neuron intrinsic excitability and spontaneous neurotransmitter signaling in the aging brain in mice.

Both alcohol use disorder (AUD) and cognitive decline include disruption in the balance of excitation and inhibition in the cortex, but the potential role of alcohol use on excitation and inhibition on the aging brain is unclear. We examined the effect of moderate voluntary binge alcohol consumption on the aged, pre-disease neuronal environment by measuring intrinsic excitability and spontaneous neurotransmission on prefrontal cortical pyramidal (excitatory, glutamatergic) and non-pyramidal (inhibitory, GABAergic) neurons following a prolonged period of abstinence from alcohol in mice. Insights into familial adult myoclonus epilepsy pathogenesis: How the same repeat expansion in six unrelated genes may lead to cortical excitability.

Familial adult myoclonus epilepsy (FAME) results from the same pathogenic TTTTA/TTTCA pentanucleotide repeat expansion in six distinct genes encoding proteins with different subcellular localizations and very different functions, which poses the issue of what causes the neurobiological disturbances that lead to the clinical phenotype. Postmortem and electrophysiological studies have pointed to cortical hyperexcitability as well as dysfunction and neurodegeneration of both the cortex and cerebellum of FAME subjects. FAME expansions, contrary to the same expansion in DAB1 causing spinocerebellar

Impaired motor cortical excitability and synaptic plasticity were more prominent in AD and DLB than in FTD. This aligns with the evidence that cortical motor networks are usually spared in FTDs in early-to-middle stages.

Long-Term Modulation of Cortical Excitability by Repeated Anodal Transcranial Direct Current Stimulation Highlights Neurobiological Constraints in a Neurodevelopmental Disorder Model.

INTRODUCTION: Transcranial direct current stimulation (tDCS) modulates neuronal excitability, but its long-term effects on intrinsic and synaptic properties remain largely underinvestigated, as are its effects in the case of a malfunctioning brain, such as in neurodevelopmental disorders. MATERIALS AND METHODS: We applied a repeated sham or anodal tDCS for five consecutive days to wild-type (WT) and Angelman syndrome (AS) model mice over the parietal cortex. Behavioral assessments began on the third day of stimulation. Ninety minutes after the final fifth session, we extracted the brains and

TMS and EEG Pharmacodynamic Effects of a Selective Sphingosine-1-Phosphate Subtype 1 Receptor Agonist on Cortical Excitability in Healthy Subjects.

Current anti-epileptic drugs lack efficacy, cause many side effects and one third of all patients are treatment-resistant. Drugs targeting the sphingosine-1-phosphate receptor show potential anti-convulsant effects in animal models and decrease cortical excitability in patients with multiple sclerosis, but available compounds alter lymphocyte trafficking and cause immunosuppression, limiting their clinical anti-epileptic potential. TRV045 is a selective sphingosine-1-phosphate subtype 1 receptor agonist without effects on lymphocyte trafficking, demonstrating efficacy in animal models of epile…

The Impact of Sex Hormones on Transcranial Magnetic Stimulation Measures of Cortical Excitability: A Systematic Review and Considerations for Clinical Practice.

Repetitive transcranial magnetic stimulation (rTMS) has emerged as a promising alternative for the treatment of major depressive disorder (MDD), although its clinical effectiveness varies substantially. The effects of sex hormone fluctuations on cortical excitability have been identified as potential factors that can explain this variability. However, data on how sex hormone changes affect clinical response to rTMS is limited. To address this gap, we reviewed the literature examining the effects of sex hormones and hormonal treatments on transcranial magnetic stimulation (TMS) measures of cort…
