Core open science practices in major medical journals: development of an automatized tool based on a Large Language Model.

BACKGROUND: Open science practices are increasingly promoted to improve the transparency and reproducibility of biomedical research, and their large-scale monitoring has been identified as a priority by initiatives such as the UNESCO Recommendation on Open Science. Existing automated tools cover only narrow subsets of practices. We developed and validated an automated tool, based on locally run open-weights large language models (LLMs), to extract a core set of 13 open science practices from published medical research articles. METHODS: We built a validation database of 303 research articles

Improving Patient Access to New Drugs in South Korea: Evaluation of the National Drug Formulary System.

This study reviews and evaluates the national drug formulary system used to improve patient access to new drugs by making reimbursement decisions for new drugs as part of the South Korean national health insurance system. The national health insurance utilizes three methods for improving patient access to costly drugs: risk-sharing agreements, designation of essential drugs, and a waiver of cost-effectiveness analysis. Patients want reimbursement for new drugs to be processed quickly to improve their access to these drugs, whereas payers are careful about listing them given the associated fina

Cost-effectiveness of molecularly matched off-label therapies for end-stage cancer - the MetAction precision medicine study.

BACKGROUND: Precision cancer medicine (PCM), frequently used for the expensive and often modestly efficacious off-label treatment with medications matched to the tumour genome of end-stage cancer, challenges healthcare resources. We compared the health effects, costs and cost-effectiveness of our MetAction PCM study with corresponding data from comparator populations given best supportive care (BSC) in two external randomised controlled trials. METHODS: We designed three partitioned survival models to evaluate the healthcare costs and quality-adjusted life years (QALYs) as the main outcomes.

Universal Free Contraception Coverage Policy, Out-of-Pocket Payments, and Costs.

IMPORTANCE: While most individuals in Canada have some form of drug coverage, many public and private plans leave a substantial role for direct payment by individuals. For contraceptives, cost-sharing (deductibles and copayments) and the exclusion of some contraceptives from formularies often leads to access gaps and inequity. How universal, first-dollar contraceptive coverage policies change patient and public costs compared with a mixed public-private system is unclear. OBJECTIVE: To evaluate whether patient out-of-pocket (OOP) costs and payer type changed after the implementation of a univ

Budget impact analysis of glecaprevir/pibrentasvir treatment for hepatitis C in Iran: a modelling study.

OBJECTIVES: Glecaprevir/pibrentasvir (GLE/PIB), despite being a highly costly medication, is considered a cost-effective approach compared with sofosbuvir/velpatasvir (SOF/VEL) and sofosbuvir/daclatasvir (SOF/DCV) in the treatment of hepatitis C virus (HCV) infection. No study has evaluated the effect of GLE/PIB's introduction into Iran's drug list from a health policy perspective and estimated the budgetary impact change. Therefore, this study was conducted to analyse the fiscal effect of the introduction of GLE/PIB into Iran's drug list. DESIGN: Budget impact analysis. The assumptions and c

Variations in the perceived value of anti-SARS-CoV-2 therapeutics based on physicians' clinical backgrounds.

Although the global emergency phase of the coronavirus disease 2019 (COVID-19) pandemic has ended, antiviral therapy remains crucial for patients at high risk of severe illness. In Japan, the out-of-pocket costs for antiviral drugs shifted from full public coverage to partial patient payment in April 2024. However, the impact of physicians' background characteristics on antiviral cost perceptions and prescribing behavior remains underexplored. This study involved a secondary analysis of a nationwide, web-based interventional survey of 1,500 physicians treating COVID-19. Participants reported w

Delivery Timing, Out-Of-Pocket Maternity Costs, and Postpartum Care Utilization: An Instrumental Variable Analysis Among Commercially Insured Women.

OBJECTIVE: To examine whether out-of-pocket costs during pregnancy and delivery affect use of postpartum care. STUDY SETTING AND DESIGN: Because health insurance deductibles and limits reset annually, the timing of childbirth within that year quasi-randomly assigns people to different levels of cost-sharing during pregnancy+delivery versus postpartum. We use a novel instrumental variable approach that leverages this variation to analyze whether higher maternity spending due to delivering early in the plan year affects postpartum care utilization. The exposure is maternity out-of-pocket spendi

Sacrum magnetic resonance imaging for low back and tail bone pain: A quality initiative to evaluate and improve imaging utility.

As quality and cost effectiveness become essential in clinical practice, an evidence-based evaluation of the utility of imaging orders becomes an important consideration for radiology's value in patient care. We report an institutional quality improvement project including a retrospective review of utility of sacrum magnetic resonance (MR) imaging for low back pain at our institution over a four-year period and follow-up results after physician education intervention. Sacral MR imaging for low back pain and tailbone pain were only positive for major findings in 2/98 (2%) cases, and no major ch

Willingness to Pay for Colorectal Cancer Screening and Effect of Copayment in Southern Thailand.

Background: The incidence rate of colorectal cancer in Thailand is increasing. Hence, the nationwide screening programme with copayment is being considered. There are two proposed screening alternatives: annual fecal immunochemical test (FIT) and once-in-10-year colonoscopy. A copayment for FIT is 60 Thai baht (THB) per test (≈ 1.7 USD); a copayment for colonoscopy is 2,300 THB per test (≈ 65.5 USD). Methods: The willingness to pay (WTP) technique, which is theoretically founded on a cost-benefit analysis, was used to assess an effect of copayment on the uptake. Subjects were patients aged 50-

Evaluating the feasibility of study methods for a future trial-based economic evaluation of a multistage shared decision-making program for type 2 diabetes mellitus: Protocol for a cluster-randomized controlled pilot study.

INTRODUCTION: We developed a multistage shared decision-making program for type 2 diabetes that aims to support person-centered type 2 diabetes management in primary care. The program consists of an online patient decision aid, a preparatory consult for patients, and interprofessional training for healthcare professionals. The short- and long-term effectiveness of the multistage shared decision-making program needs to be researched in a trial-based economic evaluation. To evaluate the feasibility of study methods for such an evaluation, we will conduct a pilot study. In this article, we outlin
