Middle Fossa Craniotomy With Hydroxyapatite Cement as a Structural Repair for Noniatrogenic Cranial Cerebrospinal Fluid Leaks: A Case Series.

BACKGROUND AND OBJECTIVES: Cranial cerebrospinal fluid (CSF) leaks due to tegmen defects can be repaired through transmastoid or transcranial surgical approaches with different repair substrates, including autologous grafts or nonimmunogenic cements. Here, we describe a case series of patients who underwent middle fossa craniotomy for repair of noniatrogenic cranial CSF leak with the use of hydroxyapatite (HA) bone cement. METHODS: A retrospective analysis of all patients who underwent craniotomy with HA cement for repair of noniatrogenic cranial CSF leak at Henry Ford Hospital in Detroit bet

Biportal Endoscopic TransOrbital and transMaxillary Approach to the Cranio-Orbital Region and Middle Cranial Fossa: A Preliminary Analysis of Maneuverability.

BACKGROUND AND OBJECTIVES: Traditional and well-established transcranial approaches to the spheno-orbital region and middle cranial fossa guarantee optimal intracranial exposure, and additional orbital and zygomatic osteotomies provide further control over extracranial components to be resected; however, these techniques come at the cost of additional morbidity. The introduction of minimally invasive endoscopic approaches and the conceptualization of the so-called "multiportal" paradigm might provide an alternative route. This preliminary study investigates the feasibility of the combined Bipo

Endoscopic anterior transmaxillary "transalisphenoid" approach to Meckel's cave and the middle cranial fossa: an anatomical study and clinical application.

OBJECTIVE Multiple approaches have been designed to reach the medial middle fossa (for lesions in Meckel's cave, in particular), but an anterior approach through the greater wing of the sphenoid (transalisphenoid) has not been explored. In this study, the authors sought to assess the feasibility of and define the anatomical landmarks for an endoscopic anterior transmaxillary transalisphenoid (EATT) approach to Meckel's cave and the middle cranial fossa. METHODS Endoscopic dissection was performed on 5 cadaver heads injected intravascularly with colored silicone bilaterally to develop the appro

Endoscope-assisted keyhole surgery via an eyebrow incision for removal of large meningiomas of the anterior and middle cranial fossa.

BACKGROUND: Conventional open surgery of large meningiomas has proven to be challenging even in experienced hands. Intense retraction and dissection around neurovascular structures increase morbidity and mortality. In the present study, we retrospectively analyzed the surgical technique, and outcome in 40 patients with large anterior cranial fossa meningiomas extending to the middle fossa. All patients were approached via a supraorbital mini craniotomy. METHODS: It is a retrospective study of 40 patients (12 males, 28 females) who underwent surgery for large anterior cranial fossa meningiomas

Postoperative Cognitive Alterations and Functional Brain Reorganization in Children With Middle Cranial Fossa Arachnoid Cysts: A Pilot fMRI Study.

BACKGROUND AND OBJECTIVES: Middle fossa arachnoid cysts (MFACs) are congenital benign lesions for which the indication for surgical intervention remains controversial. Although numerous studies have demonstrated that surgery can improve cognitive function in affected children, investigations into the neural mechanisms underlying these improvements are scarce. Our aim was to quantify neurocognitive outcomes and functional brain network alterations before and after microsurgical fenestration of MFAC in children. METHODS: We acquired resting-state functional MRI data from 18 pediatric MFAC patie

Neuroendoscopic cysto-cisternostomy for middle cranial fossa arachnoid cysts: a systematic review of the practice principles from 169 cases.

INTRODUCTION: Arachnoid cysts are primarily dysembryogenetic splitting or duplication of the embryonic meningeal mesenchyme, hence the paediatric preponderance. Neuroendoscopic cysto-cisternostomy is now the favoured treatment option. We pooled data on middle fossa arachnoid cysts (MCFAC) demographics, clinical presentations, cyst characteristics, neuro-endoscopic cysto-cisternostomy and its outcomes. EVIDENCE ACQUISITION: Using search words (from the keywords; 'endoscopic treatment' and 'middle fossa arachnoid cysts') combined using Boolean operators, a systematic review of the PubMed and Co

Modified Lateral Orbitotomy Approach to Lesions of the Orbital Apex, Superior Orbital Fissure, Cavernous Sinus, and Middle Cranial Fossa.

BACKGROUND: The lateral orbitotomy approach (LOA) provides minimally invasive access to the orbit, cavernous sinus region, and middle cranial fossa. Orbital retraction with this approach can nonetheless injure orbital structures, causing unnecessary morbidity. OBJECTIVE: To describe our clinical experience with the modified LOA (mLOA), wherein the medial aspect of the lateral orbital wall posterior to the orbital rim is preserved. METHODS: This is a retrospective, single-institution case series of patients undergoing a mLOA for lesions of the orbital apex, superior orbital fissure, cavernous

Ruptured middle cranial fossa arachnoid cysts after minor trauma in adolescent boys presenting with subdural hygroma: two case reports.

BACKGROUND: Intracranial arachnoid cysts are common, cerebrospinal fluid-filled, innocent lesions that are usually detected incidentally on brain imaging. They may rupture and complicate due to subdural hematoma or hygroma after minor trauma. Authors present two cases of ruptured middle cranial fossa arachnoid cysts in adolescent (12-year-old and 15-year-old) Afghan boys presenting with subdural hygroma after minor trauma. CONCLUSION: Imaging work-up is necessary for symptomatic patients following minor head trauma as incidentally detected ruptured intracranial arachnoid cysts can be responsi

Endonasal, supraorbital, and transorbital approaches: minimal access endoscope-assisted surgical approaches for meningiomas in the anterior and middle cranial fossae.

OBJECTIVE: Minimally invasive endoscope-assisted approaches to the anterior skull base offer an alternative to traditional open craniotomies. Given the restrictive operative corridor, appropriate case selection is critical for success. In this paper, the authors present the results of three different minimal access approaches to meningiomas of the anterior and middle fossae and examine the differences in the target areas considered appropriate for each approach, as well as the outcomes, to determine whether the surgical goals were achieved. METHODS: A consecutive series of the endoscopic endo

Conservative management of a large temporal epidural hematoma associated with an incidental middle cranial fossa arachnoid cyst: A case report.

BACKGROUND: Epidural hematoma (EDH) is a potentially life-threatening complication of traumatic brain injury typically associated with skull fractures and injury to meningeal vessels. Arachnoid cysts are benign cerebrospinal fluid-filled lesions most frequently located in the middle cranial fossa and are usually discovered incidentally during neuroimaging. The coexistence of an EDH with an underlying arachnoid cyst is uncommon and may influence both the radiological appearance and clinical course. CASE DESCRIPTION: A 27-year-old male football player presented with headache and dizziness after

Factors influencing the efficacy of surgical repair for spontaneous middle cranial fossa CSF leaks: a systematic review and meta-analyses.

Spontaneous middle cranial fossa cerebrospinal fluid (CSF) leaks can be treated surgically by middle cranial fossa (MCF), transmastoid (TM), or combined approaches. Efficacy of these approaches and the factors influencing CSF leak recurrence rates need further clarity. A literature search was conducted across EMBASE, MEDLINE, Scopus, and the Cochrane Library. Case reports, non-English articles, and studies with pediatric patients alone were excluded. Proportional meta-analysis was used to estimate the CSF leak recurrence rates. Influence of associated comorbidities and adjuvant treatment measu

Surgical management of spontaneous middle cranial fossa defects: a systematic review and meta-analysis of available reconstructive techniques and materials.

Different materials and techniques have been proposed for surgical repair of spontaneous middle cranial fossa (MCF) defects. However, conclusive evidence supporting their selection and impact on clinical outcomes is lacking. The study aims to conduct a systematic review and meta-analysis on materials and techniques employed to repair MCF defects and evaluate complications and rates of recurrent cerebrospinal fluid (CSF) leaks. A PRISMA-guided systematic review and meta-analysis were performed using MESH terms and specific keywords including studies published before May 2022. Primary outcomes i
