Novel Surgical Approach for Decompression of the Scalene Triangle in Neurogenic Thoracic Outlet Syndrome.

Thoracic outlet syndrome (TOS) is a cause of upper extremity and shoulder dysfunction. TOS can present with a wide range of symptoms due to compression of the brachial plexus or its branches during their passage through the cervicothoracobrachial region or scalene triangle. There are three types of TOS: arterial, venous, and neurogenic. Neurogenic TOS (nTOS) is by far the most frequent type and represents more than 95% of all cases. Historically, surgical intervention for all types of TOS has evolved based on the treatment for a vascular etiology and has typically included a first rib resectio

Surgical outcomes of decompressive laminectomy by transspinous approach for degenerative lumbar spinal stenosis.

BACKGROUND: Recently, porous fusion/fixation implants (PFFIs) with a 3D-printed porous, fenestrated, bone self-harvesting design were developed for S2-alar-iliac (S2AI) spinopelvic fixation. Prior studies have demonstrated the safety and feasibility of using PFFI for S2AI spinopelvic fixation, but their impact on outcomes remains unknown. This study compared 1-year outcomes of PFFI vs standard threaded screws. METHODS: This is a single-center retrospective cohort study evaluating consecutive patients who underwent S2AI spinopelvic fixation between 2019 and 2024. Patients who underwent S2AI fi

Application of Ion-beam Implanted Expanded Polytetrafluoroethylene to Microvascular Decompression and the Surgical Outcome.

Accurate and long-term transposition of offending vessels is required in microvascular decompression (MVD) for the treatment of hemifacial spasm (HFS) and trigeminal neuralgia (TN). We created ion-beam implanted of an expanded-polytetrafluoroethylene (i-ePTFE) surface to transpose offending vessels in MVD. In 13 patients with MVD, we concealed and transposed offending vessels with tape-shaped i-ePTFE, and relieved facial and trigeminal nerve compression by attaching the i-ePTFE to the dura with fibrin glue. After surgery, none of the patients reported further symptoms or experienced recurrence

The urgency of surgical decompression in acute central cord injuries with spondylosis and without instability.

STUDY DESIGN: Systematic review, ambispective analysis of observational data. OBJECTIVE: To make recommendations as to whether or not urgent surgical decompression is ever indicated as the optimal treatment for enhancing neurologic recovery in a patient with acute central cord injury without fracture or instability. SUMMARY OF BACKGROUND DATA: There are currently no standards regarding the role and timing of decompression in acute traumatic central cord syndrome. In the setting of TCCS without spinal column instability, much controversy exists. METHODS: We have performed a thorough literatu

Safety and efficacy of early surgical decompression of the thoracic outlet for Paget-Schroetter syndrome.

The surgical treatment of Paget-Schroetter syndrome has evolved to include early thrombolytic therapy and an interval period of anticoagulation, followed by late surgical decompression of the thoracic outlet. More recently, we have developed an abbreviated course of therapy in which the thrombolytic therapy is followed by early surgical decompression during the same admission, then a period of anticoagulation. We compared early surgical decompression with the standard management protocol to determine safety and efficacy of the early treatment algorithm. Nine patients were treated with lysis an

Surgical treatment of spasmodic torticollis: is there a role for microvascular decompression? With an illustrative case report.

Spasmodic torticollis (ST) is a puzzling movement disorder, characterized by involuntary tonic or clonic contractions of various neck muscles. From time to time, psychogenic, extrapyramidal and neuroperipheral origins have been postulated and reflecting the variety of theories proposed, as many different treatments have been attempted, none of which has shown absolute effectiveness. Surgery of ST classically includes destructive procedures such as myotomies, stereotactically placed lesions, rhizotomies and neurectomies. The recent application of the concept of "neurovascular conflict" to ST in

Surgical treatment of degenerative spondylolisthesis in the lumbar spine: no reposition without prior decompression.

In the present note, the cause of root compression in cases with degenerative lumbar spondylolisthesis is discussed in order to clarify the best method of surgery and to avoid iatrogenic damage of the cauda equina if a transpedicular fusion is going to be performed. The root compression is due to osteophytes growing out from the upper articular facets and from the upper ridge of the lamina of the lower vertebra. Therefore, the repositioning of the anteriorly displaced vertebra by the instrumentation could lead to the squeezing of the cauda equina between the withdrawn vertebral body and the de

[Remote results of the surgical decompression of the posterior tibial nerve in the neuropathies of Hansen's disease].

Results are reported of surgery to the posterior tibial nerve affected by leprosy in 50 patients. A total of 90 neurolyses were performed using a similar technique. Study of the effect of surgery on the course of plantar sensitivity showed that 3 groups of nerves can be distinguished. Nerves in group 1 (21 totally paralyzed nerves) responded poorly to surgery (38% useful results) while on the contrary nerves in group 3 (non-paralyzed nerves) represented the ideal indication, surgery being truly prophylactic. In group 2 nerves a useful result was obtained in 82%. Indications for use of this sur

The influence of large decompressive craniectomy on the outcome of surgical treatment in spontaneous intracerebral haematomas.

The outcome of a series of 73 patients with spontaneous intraparenchymatous haematomas treated by surgical evacuation of the clot and decompressive craniectomy has been described. Comparing postoperative mortality of this series with another series of patients (54 cases) treated only with surgical removal of the clot without decompressive craniectomy a statistically significant improvement in the mortality rate after craniectomy could be observed in acute cases demanding surgical intervention for preservation of life in the first 24 hours. If signs of brain-stem suffering appear surgical morta

Comparison of percutaneous radiofrequency gangliolysis and microvascular decompression for the surgical management of tic douloureux.

Forty-two patients with tic douloureux underwent posterior fossa craniectomy and microvascular decompression (MVD) or partial rhizotomy of the trigeminal nerve and were followed an average of 25 months after operation. Thirty-six patients were found to have anatomical distortions of the nerve by an artery, vein, bony prominence, or a combination of factors, and 30 patients (83%) of this groups hav remained pain-free postoperatively. Six patients had no discernible pathological condition at the time of operation and underwent partial trigeminal rhizotomy. No patient underwent repeated MVD or rh

Surgical outcomes between posterior decompression alone and posterior decompression with fusion surgery among patients with Meyerding grade 2 degenerative spondylolisthesis: a multicenter cohort study.

BACKGROUND: Whether lumbar decompression with fusion surgery is effective against Meyerding grade 2 degenerative spondylolisthesis (DS) is unknown. Therefore, the current study aimed to compare the surgical outcomes between posterior decompression alone and posterior decompression with fusion surgery among patients with grade 2 DS with central canal stenosis. METHODS: This retrospective cohort study included prospectively registered patients (n = 3863) who underwent surgery for degenerative lumbar spinal canal stenosis at nine high-volume spine centers from April 2017 to July 2019. Aggressive thoracic vertebral hemangioma managed with preoperative embolization and surgical decompression: A case report.

A 24-year-old male presented with a 3-month history of progressive spastic paraparesis (Frankel grade B) secondary to a T9 aggressive vertebral hemangioma (AVH) with epidural extension and spinal cord compression, displaying the characteristic polka-dot and corduroy signs on computed tomography. Spinal digital subtraction angiography confirmed marked hypervascularity predominantly supplied by the right T9 intercostal artery. Selective injection of the left T9 intercostal artery opacified an ipsilateral posterior spinal artery, contraindicating embolization on that side. Preoperative embolizati
