The concepts of disorders of body image are reviewed from three aspects: sociocultural, neurological and psychiatric disorders. Particular attention is paid to the construct of body dysmorphic disorder as defined in the DSM III-R and the separation of this from somatic delusional disorder. The recognition of associated psychopathology will frequently lead to successful treatment before too much time is spent on fruitless attempts at treatment of the presenting complaint.

Bernstein and Putnam's Dissociative Experiences Scale is a standarized, often validated instrument, useful in the screening of dissociative symptoms. An adolescent was implied in a car accident in which he suffered a head trauma without any substantial loss of consciousness. The posttraumatic syndrome was characterized by headaches and mental symptoms: nervousness, feelings of depersonalization, impaired memory, difficulty in concentration. Neurological examination and laboratory tests have always been normal but for a large perduring asymmetry at the Cortical Auditory Evoked Response. This finding is commented in the light of a review of the literature and of some neuropsychological findings.

We report the clinical case of a 12-years' intoxication by doxycycline. A patient with a depersonalization and derealization syndrome took 1 g doxycycline per day. In addition to hepatocellular necrosis with cholestasis, nephrotoxicity, leukopenia, anaemia and skin hyperpigmentation he suffered from hitherto unreported adverse cardiac events as intermittent supraventricular tachycardia and sporadic Wenckebach heart block. Despite a long period of self-medication these side-effects were reversible.
