OBJECTIVE: This study examined patterns of diagnostic test use for patients with low back pain. Three specific questions were addressed: 1) What tests do physicians recommend for patients with 3 common types of low back pain? 2) Do physicians in various specialties differ in the tests they would order? and 3) How appropriate are physicians' choices of tests, based on current medical knowledge and expert recommendations?
METHODS: A stratified national random sample of 2,604 physicians in 8 specialties was mailed questionnaires asking about the tests they would order for hypothetical patients w

Routine obstetric screening for all patients should include a Pap smear, urinalysis and urine culture, complete blood count, blood type, Rh factor and direct Coombs' test. Patients should also be screened for syphilis, rubella, gonorrhea and diabetes. For patients at high risk for certain conditions, additional studies of value include obstetric ultrasound, amniocentesis, serum alpha-fetoprotein level and screening for sickle cell anemia, herpes, hepatitis B, Chlamydia and human immunodeficiency virus.

BACKGROUND: Rapid diagnostic test (RDT) results are the foundation of both case management and malaria surveillance across sub-Saharan Africa. However, RDT results may be misrecorded in health facility registers by healthcare workers (HCWs), either unintentionally or deliberately, for example, to justify treatment based on clinical judgment. A multi-country evaluation was conducted to quantify the extent of RDT misrecording and identify factors associated with recording and reporting accuracy. This report summarizes the study's methods, key findings, and implications for improving malaria RDT

BACKGROUND: With increasing clinical use of Electrocardiographic Imaging (ECGI), it is imperative to understand the limits of this technique. The objective of this study is to evaluate a potential-based ECGI approach for activation and repolarization mapping in sinus rhythm.
METHOD: Langendorff-perfused pig hearts were suspended in a human-shaped torso tank. Electrograms were recorded with a 108-electrode sock and ECGs with 256 electrodes embedded in the tank surface. Left bundle branch block (LBBB) was developed in 4 hearts through ablation, and repolarization abnormalities in another 4 hear

Glucose oxidase paper strips for semiquantitative determination of glucose in urine are commercially available but details of their preparation are not published. The purpose of this study was to produce such strips, using a coupled enzyme glucose oxidase-peroxidase reaction which would yield purple color with ortho-tolidine, and safranin upon dipping into urine containing glucose. In this present study, without any special equipment, special humidity and temperature control, the new reagent strip named R-strip could be successfully prepared in the atmospheric conditions of Thailand. R-strips

BACKGROUND AND OBJECTIVES: Diagnostic criteria play an important role in informing clinical decision-making, particularly for conditions lacking objective tests, biomarkers, or reference standards. Despite their importance, there is no established methodological guidance for developing diagnostic criteria. This scoping review aimed to identify and describe the methodological approaches used to develop diagnostic criteria in the absence of objective tests, biomarkers, or reference standards.
STUDY DESIGN AND SETTING: We conducted a scoping review in accordance with JBI methodology and the PRIS

Tuberculosis encompasses a spectrum of characteristics-including bacillary burden, clinical severity, and access to care-that are relevant to clinical and epidemiological outcomes and the performance of diagnostic assays. The value of diagnostic assays depends not only on their numerical accuracy, which can vary substantially between populations, but also on which individuals with and without tuberculosis the assays identify. Moreover, detectable features of tuberculosis, such as pathogen burden or host responses, are often correlated, making it difficult to predict the accuracy and impact of

Leishmanias is a parasitic infection caused by a protozoan belonging to the genus Leishmania and transmitted by sand fly, Phlebotomus fly in the old world and Lutzomyia in the New world. The disease is prevalent in the tropics, subtropics, and Southern Europe, where it affects about 1.5 million to 2 million people annually. India, Bangladesh, Sudan, South Sudan, Brazil and Ethiopia account for up to 90% of all the VL cases. While Leishmania cases in Asia are declining, cases in East Africa especially in Somalia, Sudan, South Sudan, Ethiopia and Kenya have been increasing. The rise in East Afri

Nasopharyngeal swab is the most widely used diagnostic test for COVID-19 detection. However, enormous tests have posed a high risk of infection to medical professionals due to close contact with patients and substantial health burden. While automation of the nasopharyngeal swab is regarded as a potential solution to address these problems, the quantitative study of force for safe and effective control has not been widely performed yet. Hence, this study presents applied force during the standard nasopharyngeal swab sampling procedure using a handheld sensorized instrument. The sensorized instr

BACKGROUND: To overcome the limitations of conventional malaria rapid diagnostic tests (cRDTs) in diagnosing malaria in patients with low parasitaemia, ultrasensitive malaria rapid diagnostic tests (uRDTs) have recently been developed, with promising results under laboratory conditions. The current study is the first meta-analysis comparing the overall sensitivity, and specificity of newly developed ultrasensitive Plasmodium falciparum malaria RDT (Alere™ Ultra-sensitive Malaria Ag P. falciparum RDT) with the cRDT conducted in the same field conditions.

Histoplasmosis is a worldwide-distributed systemic mycosis caused by the dimorphic fungus Histoplasma capsulatum. Its clinical manifestations range from subclinical or mild respiratory illness to progressive disseminated histoplasmosis (PDH), a life-threatening disease, whose accurate diagnosis is still challenging and limited in many countries, where this disease is highly endemic. In this regard, Histoplasma antigen testing is now included in the WHO Essential Diagnostics List. The final diagnosis of histoplasmosis is established by culture and/or visualization of the yeast cells by cytology

BACKGROUND: Sickle cell disease (SCD) is an important cause of under-five mortality. Tanzania is the 5th country in the world with the highest births prevalence of SCD individuals. Significant advances in the neonatal diagnosis of SCD using rapid point-of-care testing have been made. However genetic confirmation is still required for positive cases, in uncertain cases, in multiply transfused patients, to resolve compound heterozygosity (Hb S/ β0 Thal or Hb S/ β+ thal) not uncommon in the coastal regions of East Africa and increasingly also for pre-marital counselling and potentially for future

The 2016-2017 Tanzania HIV Impact Survey (THIS) reported the accomplishments towards the 90-90-90 global HIV targets at 61-94-87, affirming the need to focus on the first 90 (i.e., getting 90% of people living with HIV (PLHIV) tested). We conducted a patient-pathway analysis to understand the gap observed, by assessing the alignment between where PLHIV seek healthcare and where HIV services are available in the Shinyanga region, Tanzania. We used existing and publicly available data from the National AIDS Control program, national surveys, registries, and relevant national reports. Region-wide

False-positive results in the diagnostic of meningitis and encephalitis pose important challenges. This study aimed to determine false-positive rates for Haemophilus influenzae in cerebrospinal fluids evaluated by the BioFire FilmArray® Meningitis/Encephalitis Panel. We conducted a retrospective study of all H. influenzae-positive FilmArray®. Meningitis/Encephalitis Panel results from June 2016 to October 2019 in two Swiss university hospitals. Cases were classified as true positive, likely true-positive, and likely false-positive results according to cerebrospinal fluid culture, H. influenzae
