Laparoscopic gastrointestinal surgery.

The growth and scope of laparoscopic gastrointestinal surgery are outstripping the ability of medical journals to print series and reports of experience, and much data remain anectodal and preliminary. Laparoscopic cholecystectomy has become established, although, unfortunately, peroperative cholangiography has not yet become normal practice, despite convincing published evidence that it is likely to reduce the increased number of bile ducts that are damaged by laparoscopic surgeons. Imaging the bile ducts and the overall management of choledocholithiasis need re-evaluation, and the approach w

[Candidiasis in the field of gastroenterological surgery].

Postoperative candidiasis in the gastroenterological surgery has an increasing tendency with poor prognosis particularly in generalized cases. Candida is difficult to be specified as infectious agent only by culture findings, and has a problem on rapidity and sensitivity. Furthermore, also as for the time starting antifungal treatment, the start after the blood culture has given positive result is problematic. We analyzed the present state of candidiasis in our department and studied its background factors. Totaling of 4,424 samples from all of bacterial and fungal culture tests performed at o

Minimally invasive management of epiphrenic esophageal diverticula.

The purpose of this study is to review our initial experience with a minimally invasive approach to manage symptomatic epiphrenic esophageal diverticula. Five patients with symptomatic epiphrenic esophageal diverticula underwent surgical management between August 1997 and December 2002. All patients complained of dysphagia; had experienced symptoms for at least 12 months; and were evaluated preoperatively by a barium esophagram, esophagogastroduodenoscopy, and esophageal manometry. The epiphrenic esophageal diverticula measured 5 cm or less in all patients. Manometry demonstrated esophageal dy

[The "grasping tie". I. Introduction to and technical notes of an innovative mechanical tool aimed at formation of "purse string" in digestive surgery].

The grasping tie is an original and mechanical tool, conceived and planned to be used in digestive surgery, to temporary grasp and fasten at the axis of the head or the anvil of a circular stapler the tract of the gut chosen for anastomosis. Its use permit to fix the lower oesophageal stump in total gastrectomy, or that of the rectal stump in the lower rectal resection, respectively to the axis of the circular stapler component for the time necessary to perform the mechanical anastomosis. That is performed by the employment of a thin nylon ribbon, which will be progressively tightened as reque

Artificial intelligence and robotic surgery in emergency gastrointestinal procedures: a review of current evidence and future directions.

Gastrointestinal emergency surgeries have a high rate of morbidity and mortality due to diagnostic inaccuracies, urgent decision-making, and physiological instability of patients. While robotic surgery and artificial intelligence (AI) have become well-established in elective procedures, their application in emergency gastrointestinal surgery remains an emerging field. This study is presented as a narrative review due to the heterogeneity of available study designs, study populations, the used technologies and reported outcomes. A literature search was performed across relevant databases. Data

Compliance Assessment to Enhanced Recovery Protocols: A Three-Tier Donabedian Framework-Based Approach.

Enhanced recovery pathways are multi-component perioperative care bundles that have been associated with reduced length of hospitalization, overall morbidity and hospital costs. The challenges faced with the implementation of enhanced recovery pathways in hepato-pancreato-biliary (HPB) surgery have been two-fold, namely, the inherent higher procedural complexity with an attendant risk of complications that may necessitate a case-by-case deviation from protocols, and the challenges of incomplete measurement of adherence, even when the pathways have been developed based on evidence. The counter

[Comparison of robot-assisted and conventional endoscopic abdominal surgeries in the Russian Federation: a systematic review and meta-analysis].

OBJECTIVE: To compare the results of robot-assisted (RA) and laparoscopic (LA) technologies in abdominal surgery in Russian surgical practice. MATERIAL AND METHODS: A systematic review and meta-analysis were conducted in accordance with methodological recommendations of the Russian Ministry of Health and the Cochrane guidelines. Considering heterogeneity of primary study designs, we did not determine the overall effect of meta-analysis. Differences were presented for each study. The review continues the 2021 study and updates data using similar inclusion criteria and approaches to meta-analys

[Structured Implementation and Modular In-house Training as Key Success Factors in Robotically Assisted Surgery - Evaluation Using the Example of Colorectal Surgery].

OBJECTIVES: To demonstrate the applicability of structured implementation of robotic assisted surgery (RAS) and to evaluate a modular training procedure during the implementation phase in in-house mentoring. METHOD: Execution of a self-defined PDCA (PDCA: Plan-Do-Check-Act) implementation cycle accompanied by prospective data collection of patient characteristics, operation times, complications, conversion rates and postoperative length of stay of a modularly defined training operation (robotic assisted rectosigmoid resection - RARSR). RESULTS: Evaluation of 100 consecutive cases distributed
