Potential impact and cost-effectiveness of long-acting injectable lenacapavir plus cabotegravir as HIV treatment in Africa.

Although viral suppression is attained for most adults living with diagnosed HIV in East, Central, Southern and West Africa (ECSWA), challenges remain with sustained adherence to daily oral pill taking for some in the population. Here, we evaluate the potential effectiveness and cost-effectiveness of introduction of a new combination of long-acting injectable drugs of lenacapavir + cabotegravir to increase levels of sustained viral suppression. We find there is potential for a significant impact on HIV deaths and disability adjusted life years, including due to a decrease in mother to child tr

The safety of cabotegravir in pregnancy: a systematic review and meta-analysis.

BACKGROUND: HIV remains an important global public health crisis. Antiretroviral treatment has been essential to suppress viral replication, and in at-risk individuals, antiretroviral pre-exposure prophylaxis drugs (PrEP) lower transmission rates. Unfortunately, current oral regimens require stringent daily adherence to be effective. Long-acting intra-muscular cabotegravir potentially averts the issue of poor patient adherence. This formulation has the potential to cause a breakthrough in mitigating the spread of HIV; however, safety in pregnancy is still to be established. This systematic rev

Co-delivery of Plinabulin and Tirapazamine boosts anti-tumor efficacy by simultaneously destroying tumor blood vessels and killing tumor cells.

It is imperative to optimize chemotherapy for heightened anti-tumor therapeutic efficacy. Unrestrained tumor cell proliferation and sustained angiogenesis are pivotal for cancer progression. Plinabulin, a vascular disrupting agent, selectively destroys tumor blood vessels. Tirapazamine (TPZ), a hypoxia-activated prodrug, intensifies cytotoxicity in diminishing oxygen levels within tumor cells. Despite completing Phase III clinical trials, both agents exhibited modest treatment efficiency due to dose-limiting toxicity. In this study, we employed methoxy poly(ethylene glycol)-b-poly(D,L-lactide)

Effect of Obesity on the Exposure of Long-acting Cabotegravir and Rilpivirine: A Modeling Study.

BACKGROUND: Obesity is increasingly prevalent among people with human immunodeficiency virus (HIV, PWH). Obesity can reduce drug exposure; however, limited data are available for long-acting (LA) antiretrovirals. We performed in silico trials using physiologically based pharmacokinetic (PBPK) modeling to determine the effect of obesity on the exposure of LA cabotegravir and rilpivirine after the initial injection and after multiple injections. METHODS: Our PBPK model was verified against available clinical data for LA cabotegravir and rilpivirine in normal weight/ overweight (body mass index

Preformulation stability of Spantide II, a promising topical anti-inflammatory agent for the treatment of psoriasis and contact dermatitis.

Substance P is readily expressed in skin inflammatory disorders such as psoriasis and contact dermatitis. Spantide II is a peptide (MW 1668.76) that specifically binds to neurokinin-1 receptor (NKR-1) and blocks inflammation associated with substance P. The anti-inflammatory property of Spantide II makes it a suitable candidate to be studied as a topical formulation for the treatment of dermal inflammatory disorders. The objective of this study was to investigate the influence of pH, temperature, salt concentration and concentration on the aqueous stability of Spantide II. The stability of Spa

Comparative Effectiveness of Daily Oral and Long-acting Injectable Prep: A Critical Review of Clinical Evidence and Public Health Implications.

The Human Immunodeficiency Virus (HIV) is the cause of Acquired Immunodeficiency Syndrome (AIDS). Preexposure Prophylaxis (PrEP) antiretroviral drugs to prevent HIV-1 infection are currently available in many countries. In Brazil, the recommended and approved regimen for PrEP is a daily Fixed-Dose Combination (FDC) tablet of Tenofovir Disoproxil Fumarate (TDF) with Emtricitabine (FTC) (300/200 mg), marketed as Truvada®. However, adherence to daily oral pill regimens remains one of the greatest challenges for the successful use of PrEP. Therefore, a Long-Acting Injectable (LAI) PrEP regimen, su

Virologic Failure and Emergent Integrase Strand Transfer Inhibitor Drug Resistance With Long-Acting Cabotegravir for HIV Treatment: A Meta-analysis.

BACKGROUND: The long-acting injectable regimen of cabotegravir plus rilpivirine (CAB/RPV) emerged as an alternative to oral standard-of-care integrase strand transfer inhibitor (INSTI)-based regimens for individuals with adherence challenges or preference for reduced dosing schedules. Although oral INSTI regimens have a high barrier to emergent resistance, less is known about the potency and durability of CAB/RPV. METHODS: We reviewed clinical trial registries, PubMed, EMBASE, and conference abstract databases to identify reports of CAB/RPV for HIV therapy. We abstracted data on virologic fai

The effect of long-acting cabotegravir and rilpivirine treatment on drug resistance in South Africa: a modelling study.

BACKGROUND: Long-acting injectable cabotegravir and rilpivirine (LAI cabotegravir and rilpivirine) is recommended as maintenance therapy for people living with HIV who achieved viral suppression on oral antiretroviral therapy (ART). However, its effect on drug resistance evolution in resource-limited settings remains uncertain. We aimed to assess this effect under different roll-out strategies and explore key factors for resistance. METHODS: We extended the Modelling Antiretroviral Drug Resistance In South Africa (MARISA) model to assess the effect of introducing LAI cabotegravir and rilpivir

Valproic Acid Induces Antimicrobial Compound Production in Doratomyces microspores.

One of the biggest challenges in public health is the rising number of antibiotic resistant pathogens and the lack of novel antibiotics. In recent years there is a rising focus on fungi as sources of antimicrobial compounds due to their ability to produce a large variety of bioactive compounds and the observation that virtually every fungus may still contain yet unknown so called "cryptic," often silenced, compounds. These putative metabolites could include novel bioactive compounds. Considerable effort is spent on methods to induce production of these "cryptic" metabolites. One approach is th

A Darwinian model for the evolution of drug resistance to long-acting PrEP during an early HIV infection.

Predicting the evolution of drug resistance remains a central challenge in HIV prevention and treatment. Evolutionary trade-offs, i.e., the fitness costs of resistance, shift as drug-selective pressure changes, a complexity amplified by fluctuating drug concentrations during long-acting prophylaxis. We develop a Darwinian evolutionary game theory model of early HIV infection under bimonthly pre-exposure prophylaxis (PrEP) with long-acting cabotegravir (CAB-LA). This work introduces a mathematical framework that couples within-host viral dynamics with adaptive evolutionary processes in a time-d

HIV-1 non-group M phenotypic susceptibility in vitro to bictegravir and cabotegravir.

OBJECTIVES: HIV-1 group O (HIV-1/O) is one of the four HIV-1 groups and is endemic in Cameroon, representing 1% of HIV-1 infections in the population. Around 50% of the strains of this group naturally show a mutation (Y181C) providing them with resistance to NNRTIs and making therapeutic management more difficult. Today, the WHO recommends the use of integrase strand transfer inhibitors (INSTIs) as first-line treatment. Bictegravir and cabotegravir are the two most recent INSTIs. Because of the genetic polymorphism of HIV-1/O, studies are required to evaluate their phenotypic susceptibility to
