Lower extremity pain and numbness.

CLINICAL INTRODUCTION: A 45-year-old man with no past medical history presents with bilateral lower extremity pain and perineal numbness after walking 10-15 min. The pain starts in his bilateral buttocks and radiates down into his thigh and into his toes. He has no back pain, fever, difficulty urinating or stooling, incontinence or trauma. Social history is positive for smoking. Physical examination was only remarkable for non-dopplerable lower extremity pulses, and he was unable to ambulate down the ED hallway without becoming symptomatic. Ultrasound of his abdominal aorta (figure 1) was perf

Migration of an Atrial Septal Occluder Device With Formation of Abdominal Aortic Dissection.

Atrial septal defects can be closed surgically or percutaneously. However, percutaneous closure of atrial septal defects carries some risks. Embolization of the atrial septal occluder is the major adverse event. Embolization of the device into the main pulmonary artery, left atrium, right ventricle, aortic arch, descending aorta, abdominal aorta, iliac bifurcation, and iliac arteries can be seen. We report the case of a 19-year-old man with migration of an Amplatzer (St Jude Medical, St Paul, MN) atrial septal occluder into the iliac bifurcation with the development of abdominal aortic dissect

Multiple variations of branches of abdominal aorta.

The abdominal aorta and its major branches supply oxygenated blood to nearly all the organs in the abdominal cavity. During routine dissection (January 2009) of a middle aged male cadaver at Dr. PSIMS, Gannavaram, Krishna Dist. (INDIA), the following variations of branches of abdominal aorta were found. The coeliac trunk gave off three branches. The first branch was left inferior phrenic artery which arose directly from coeliac trunk. The second branch bifurcates into left gastric artery and accessory hepatic artery for left lobe of liver. The second branch gave off splenic artery and common h

Treatment of acute aortic dissection type A with paraplegia and distal limb ischemia within a hybrid operating room.

OBJECTIVE: Acute aortic dissection type A is among the most lethal surgical emergencies. Patients may suffer from occlusion of the aorta or its branches causing end-organ malperfusion complicating the diagnosis and worsening the prognosis. Paraplegia is a rare manifestation that affects less than 5% of patients. If type A aortic dissection and occlusion of the downstream thoraco-abdominal aorta occur simultaneously and require acute treatment, a medical dilemma occurs; what should be treated first? CASE REPORT: We describe a case with an extensive acute type A aortic dissection with signs of

Aortic dissection in a young patient without any predisposing factors.

Aortic dissection is rare in the pediatric and young adult population. We hereby present a case of a 17-year- old male patient, without any predisposing factors, who developed an aortic dissection. The initial presentation was acute abdominal pain with massive retroperitoneal hematoma. His clinical condition deteriorated rapidly, did not respond to surgical hemostasis, and died within 36 hours of admission. The major autopsy finding was dissection of the descending aorta, extending from 2 cm distal from the origin of the left Subclavian artery. Microscopically, the aortic sections showed intim

Adventitial CXCL1/G-CSF expression in response to acute aortic dissection triggers local neutrophil recruitment and activation leading to aortic rupture.

RATIONALE: In-hospital outcomes are generally acceptable in patients with type B dissection; however, some patients present with undesirable complications, such as aortic expansion and rupture. Excessive inflammation is an independent predictor of adverse clinical outcomes. OBJECTIVE: We have investigated the underlying mechanisms of catastrophic complications after acute aortic dissection (AAD) in mice. METHODS AND RESULTS: When angiotensin II was administered in lysyl oxidase inhibitor-preconditioned mice, AAD emerged within 24 hours. The dissection was initiated at the proximal site of th

Fate and Consequences of the False Lumen After Thoracic Endovascular Aortic Repair in Type B Aortic Dissection.

BACKGROUND: Type B aortic dissection (TBAD) occurs due to an entry tear in the intimal layer of the aorta distal to the origin of the left subclavian artery where blood enters the newly formed false lumen (FL) and extends distally or proximally to form a dissection over an indeterminate length of the aorta which, over time, may eventually rupture. Thoracic endovascular aortic repair (TEVAR) aims to seal off the entry tear proximally with the stent-graft, occluding the origin of the dissection and excluding the FL. Nevertheless, in some cases, the perfusion to the FL is maintained, hindering th

Postoperative Changes in the Distal Residual Aorta after Surgery for Acute Type A Aortic Dissection: Impact of False Lumen Patency and Size of Descending Aorta.

Objective In the present study, we retrospectively identified and analyzed the factors that influenced progressive dilatation of the residual distal aorta after surgical repair for acute type A aortic dissection (acute type A). Methods A total of 477 patients underwent surgical repair for acute type A aortic dissection between 1995 and 2012. Postoperative and follow-up computed tomography (CT) scans of the descending aorta were available in 105 patients. We analyzed the maximum aortic diameter, total luminal area, and true luminal area of the descending thoracic and abdominal aorta. Results Th

A Case Report and a Literature Review of Traumatic Abdominal Aortic Dissection: An Uncommon Complication Following Vertebral Fractures Due to Blunt Trauma.

Aortic dissection is a rare consequence of blunt trauma with potentially fatal consequences requiring prompt identification and management. The most common site for dissection or transection is the thoracic aorta due to anatomical fixation points. Current literature describes four cases of traumatic abdominal aortic dissection with intimal tear associated with vertebral fractures due to falling. We present a 30-year-old gentleman who attended following a fall from a first-floor window. Whole-body computerised tomographic imaging demonstrated superior endplate fractures of L1-L3 vertebral bodie

Abdominal aortic branches of red-rumped agouti (Dasyprocta leporina Linnaeus, 1758).

Understanding the cardiovascular system is fundamental in diagnosing pathologies and interpreting exams, such as contrast radiographs. In this context, the present study describes the collateral abdominal aorta artery branches of red-rumped agouti. Ten red-rumped agoutis, six males and four females, were assessed. The vascular system was perfused with Neoprene 450 latex coloured with a yellow pigment, dissected and analysed. Three euthanized animals were perfused with a barium sulfate solution (1 g mL-1 ) associated with latex Neoprene 450 at a 1:3 ratio to obtain contrast-enhanced radiographs

TEVAR-Only Management of Abdominal Aortic Aneurysm Rupture Caused by Acute Type B Aortic Dissection after EVAR: A Case Report.

Type B aortic dissection after endovascular aneurysm repair (EVAR) is a rare late complication and may cause rupture of a previously stable abdominal aortic aneurysm (AAA). A 79-year-old male developed acute Stanford type B aortic dissection 12 years after EVAR, with distal extension into an infrarenal AAA resulting in rupture and circulatory collapse. A large primary entry tear was located in the descending thoracic aorta. Emergency thoracic endovascular aortic repair (TEVAR) was performed to seal the entry tear, achieving rapid hemodynamic stabilization without abdominal intervention. This c

Redo operations for recurrent aneurysmal disease of the ascending aorta and transverse aortic arch.

Sixty-seven operations were performed in 59 patients for aneurysmal disease occurring after previous operations involving the ascending aorta and transverse aortic arch. The initial aortic pathological condition included the following: fusiform aneurysm due to medial degenerative disease in 34 patients, 12 of whom had Marfan's syndrome; aortic dissection in a previously undilated aorta in 23; and aneurysm persisting or occurring after brachiocephalic bypass in 2. One of the latter had an aneurysm because of aortitis. Various operations initially performed did not completely treat the disease,

Repeated endovascular approaches achieve successful complete remodelling for type IIIb acute aortic dissection.

Thoracic endovascular repair for type IIIb aortic dissection results in the disappearance of false lumen perfusion in the abdominal aorta in only about 20% of cases, with concern for expansion in the long term. Staged endovascular therapy with intervention on all re-entries can lead to complete false lumen thrombosis and remodelling of the entire aorta. This approach could be an option in cases in which long-term expansion is anticipated.

Abdominal Aortic Rupture Secondary to Lymphoma Recurrence.

Neoplasias affecting the aorta are usually due to a variety of thoracic and abdominal tumors, which are more common than primary tumors of the aortic wall. Those tumors that can invade the abdominal aorta are usually sarcomas, which are able to mimic, both clinically and radiologically, an aortic disease such as an aneurysm or a dissection. There are few clinical scenarios where surgical resection and aortic repair needs to be performed, and indications have not still been clearly established in the literature. We describe the case of a patient with a periaortic lymphoma who presented an aorti

Apicoaortic valved conduit bypass for progressing aortic graft stenosis due to malformation of repeated thoracic endovascular aortic repairs.

Since the first report by Cooley and colleagues in 1975 [Cooley DA, Norman JC, Mullins CE, Grace R. Left ventricle to abdominal aorta conduit for relief of aortic stenosis. Cardiovasc Dis 1975;2:376-83], an apicoaortic valved conduit bypass has been usually administrated to selected patients presenting with certain clinical conditions or complications such as aortic stenosis associated with porcelain aorta, unclampable atherosclerotic aorta, resternotomy, or previous coronary bypass surgery. On the other hand, thoracic endovascular aortic repair for various aortic lesions has become a promisin

Haemodynamic effects of non-Newtonian fluid blood on the abdominal aorta before and after double tear rupture.

OBJECTIVES: Intimal tears caused by aortic dissection can weaken the arterial wall and lead to aortic aneurysms. However, the effect of different tear states on the blood flow behaviour remains complex. This study uses a novel approach that combines numerical haemodynamic simulation with in vitro experiments to elucidate the effect of arterial dissection rupture on the complex blood flow state within the abdominal aneurysm and the endogenous causes of end-organ malperfusion. MATERIALS AND METHODS: Based on the CT imaging data and clinical physiological parameters, the overall arterial models
