New challenges for gamete donation programmes: changes in guidelines are needed.

In the UK, the Human Fertilisation and Embryology Act 1990 prevents children born as a result of donor-assisted conception from gaining access to identifying information about their genetic origins. There is growing concern that current screening protocols regarding gamete donation are ill-suited, especially in relation to genetic disease. There are no guidelines addressing the issues of confidentiality that might arise if a disease emerges after insemination and establishment of pregnancy. Donors may become aware that they are at risk of a familial condition after they have donated gametes or

Single versus double insemination: a retrospective audit of pregnancy rates with two treatment protocols in donor insemination.

Our objective was to evaluate the effect of a change in treatment protocols, suggested following an inspection visit by the regulatory authority, from single to double inseminations during donor insemination treatment cycles. We therefore conducted a retrospective audit of pregnancy rates in the reproductive medicine clinic of a major teaching hospital. All patients were treated for male factor infertility by donor insemination, without ovulation induction with gonadotrophins between October 1992 and December 1995. The main outcome measures were cumulative conception and live birth rates. Duri

Motivation, attitudes and experience of donation: a follow-up of women donating eggs in assisted conception treatment.

This paper reports a follow-up of 39 women who had donated eggs to an assisted conception unit. Their experience of donation and their motivation and attitudes were assessed. Comparisons were made with a group of semen donors who were attending a second unit. Female and male donors donated for altruistic reasons and neither group wished to have contact with recipients or donor offspring or have their identity revealed. Female donors were more involved in the donation process and more interested in the outcome of donation. They also appeared to be more motivated by 'helping' than male donors. T

Egg donation beyond the standard model: fertility practitioners' clinical reasoning in single, combined and shared treatment options in Sweden.

The use of donor eggs, sperm and embryos in medically assisted reproduction (MAR) provide new possibilities for reproductive assistance and family-making. In clinical practice, it also brings to light questions of responsibility and ethical conduct. Despite this, fertility practitioners' reasoning in clinical decision-making remains surprisingly understudied. Drawing on Annemarie Mol's work on ontologies in medical practice (2002), we examine how fertility practitioners' clinical reasoning varies across different types of reproductive assistance involving donor eggs. Following Sweden's legal c

[Role of nurses for donor's families: current issue of organ transplant in the United States].

To define the roles of donor coordinators (certified procurement transplant coordinators; CPTCs), we visited University of Florida Colleges of Nursing and Medicine and certified procurement transplant interviewed (CPTCs) and clinical specialists in pediatric critical care. Recipients were dealt with certified clinical transplant coordinators (CCTCs). These coordinators are certified by taking examinations to see if they are equipped with specialized knowledge and communication skills. The small number of organs available for transplant poses a serious problem. In many cases, medical personnel

A review of family donor constructs: current research and future directions.

This review addresses research on gamete donors, recipients, and offspring and demonstrates that the foci on all three within the triad are largely directed at disclosure or anonymity; and each in turn centers on the perceived importance of the genetic link. The importance attached to genetics has led some countries to review the ethics of anonymous gamete donation (e.g. New Zealand's 'open system' of information sharing) and has led other countries (Sweden, Austria, Victoria, Australia; the Netherlands, the UK) to change their laws allowing donor gamete offspring the right to obtain identifyi

Public opinion and organ donation suggestions for overcoming barriers.

Getting organs for transplantation depends on people's decision; thus, public opinion is essential to finding a solution to this problem. Efforts to improve organ shortage focus on: 1) Living, unrelated donation, 2) increasing marginal donors and 3) proposing economic support for donors. Paradoxically, no initiative has been suggested to modify public opinion towards cadaver donors. Several reasons explain the resistance to donating cadaver organs: Lack of awareness, religious uncertainties, distrust of medicine, hostility to new ideas, and misinformation. Education should be used to reshape p

Successful haemopoietic stem cell transplantation does not correct mannan-binding lectin deficiency.

It has been reported that in allogeneic stem cell transplantation, the mannan-binding lectin (MBL) status of the donor has prognostic value for the recipient. Two MBL-deficient patients, with coexisting haematological malignancy, were identified who were treated with bone marrow from donors with normal MBL concentrations. Although both patients engrafted successfully and remain in complete remission, neither seroconverted to the MBL sufficiency status of his donor over a follow-up period exceeding 2 years. This does not support the concept of MBL replacement by stem cell therapy, and does not

Living Donor Ethics and Uterus Transplantation.

This article provides an in-depth ethical analysis of living donor uterus transplantation, incorporating clinical, psychological, and qualitative study data into the discussion. Although the concept of living organ donors as patients in their own right has not always been present in the field of transplantation, this conceptualization informs the framework for living donor ethics that we apply to living uterus donation. This framework takes root in the principles of research ethics, which include respect for persons, beneficence, and justice. It incorporates an analysis based on eight potentia

Precision Medicine and Personalized Approach to Renal Transplantation.

Successful renal transplantation is a highly effective endeavor that improves and prolongs the lives of patients with chronic kidney disease. Transplant surgery and immunosuppression carries risk and the demand for donor kidneys outstrips supply by far. These realities mandate thoughtful allocation and utilization of this limited resource to select candidates. As the criteria for candidates and donor grafts continue to expand, the field must adapt and seek new approaches. The complex process-from evaluation of candidates, transplant surgery, immunosuppression, and follow-up care after transpla

Report From a Multidisciplinary Symposium on the Future of Living Kidney Donor Transplantation.

Virtually all clinicians agree that living donor renal transplantation is the optimal treatment for permanent loss of kidney function. Yet, living donor kidney transplantation has not grown in the United States for more than 2 decades. A virtual symposium gathered experts to examine this shortcoming and to stimulate and clarify issues salient to improving living donation. The ethical principles of rewarding kidney donors and the limits of altruism as the exclusive compelling stimulus for donation were emphasized. Concepts that donor incentives could save up to 40 000 lives annually and conside

The strip gingival autograft technique.

In keeping with the concept of rapid epithelialization of close wound edges, the strip technique was developed to maximize the area of gingival grafting with less trauma to the donor site or the recipient site. An incision is made and a partial-thickness flap is reflected so that stable periosteum is left. The apical mucosal border of the recipient site is sutured to the periosteum. Donor tissues are obtained in 2-mm-wide strips, transferred to the recipient site, and sutured. Dry foil and surgical packing are used to stabilize and protect the site during healing. The donor site is rapidly epi

Liver repopulation: a new concept of hepatocyte transplantation.

Hepatocyte transplantation has been recognized as an alternative strategy for organ transplantation because the supply of donor livers is limited. However, in conventional hepatocyte transplantation, only 1%-10% of the liver replaced with transplanted hepatocytes. Recently a novel concept termed "liver repopulation" has been established, where the whole recipient liver can be replaced by a small number of donor hepatocytes. To induce liver repopulation, growth advantage of the donor hepatocytes against the host liver seems to be required according to the data of previous studies. Additionally,

Female-partnered and single women's contact motivations and experiences with donor-linked families.

STUDY QUESTION: What are female-partnered and single mothers' motivations and experiences at one donor insemination (DI) program with regard to contacting other families who share the same sperm donor? SUMMARY ANSWER: By and large, women reported seeking contact to obtain (i) support for their children and/or themselves, and (ii) information about shared traits and medical problems, ultimately describing a range of contact experiences, both positive (e.g. special bond created) and negative (e.g. uncomfortable encounters). WHAT IS KNOWN ALREADY: There is a growing phenomenon of donor insemina

Renal transplantation from extended criteria cadaveric donors: problems and perspectives overview.

The critical shortage of organs available for renal transplantation has led to the consideration of alternative strategies for increasing the donor pool. Recently, the cadaveric kidney donor pool extended to donors who might have been deemed unsuitable in early times, leading to the concept of marginal donors and more recently to the notion of expanded criteria donors. Such organs are eligible for organ donation but, because of extreme age and other clinical characteristics, are expected to produce allograft at risk for diminished post-transplant function. Thus, the challenge is now to reduce

Fragile X Syndrome Secondary to in Vitro Fertilization With a Family Egg Donor: A Case Report and Review of the Literature.

Objective: To evidence the need for screening fragile X syndrome (FXS) in egg donors in assisted reproduction protocols. Case report : This is the report of a boy with FXS who inherited the mutated allele from an ovule donated by the mother´s sister through an assisted reproduction protocol. Identifying premutation (PM) carriers of FXS amongst gamete donors isn't part of the obligatory genetic analysis for donors and is only considered by most of the in vitro fertility societies and guidelines as part of the extension screening tests. Conclusion: It is cost-effective to do pre-conceptional scr

The ethical case for non-directed postmortem sperm donation.

In this article we outline and defend the concept of voluntary non-directed postmortem sperm donation. This approach offers a potential means of increasing the quantity and heterogeneity of donor sperm. This is pertinent given the present context of a donor sperm shortage in the UK. Beyond making the case that it is technically feasible for dead men to donate their sperm for use in reproduction, we argue that this is ethically permissible. The inability to access donor sperm and the suffering this causes, we argue, justifies allowing access to sperm donated after death. Moreover, it is known t

Self-perceived health status in blood donors.

BACKGROUND: Relationships between self-perceived health status and mortality, morbidity, disability and health care utilisation have been well established. The self-perceived health status of blood donors has, however, never been assessed. MATERIALS AND METHODS: Using the Short-Form 12 questionnaire, we assessed self-perceived health status in a sample of blood donors. RESULTS: The mean values for the Physical Component Summary (PCS) score were similar for males and females, being 54 versus 54.12, respectively. There was, however, a gender difference for the Mental Component Summary (MCS) sc

Meteorological elements associated with vasovagal reactions in blood donors: Influence of collection setting and donation type.

BACKGROUND AND OBJECTIVES: Vasovagal reactions (VVRs) are common adverse events associated with blood donation. While previous research has focused on intrinsic donor-related risk factors, we previously identified an association between specific meteorological elements and VVR incidence. This study aims to build on our earlier findings by investigating the potential modifying effects of collection setting and donation type. STUDY DESIGN AND METHODS: This study analysed VVR incidence data from blood donations collected in northern Nara Prefecture, Japan, over a 3-year period (April 2020-March

[Cultural diversity in gamete and embryos donation].

Through gamete and embryo donation have successively emerged new ways of designing individuals who, in turn, have generated mutations in the concept of parenthood. A debate is open to the society, which often raises ideological cleavages. Indeed, donation practices mobilize the conflicting interests of donor of gametes, the recipient couple, child, whose origins are complex, although his filiation is legally clear. Its place in the family genealogy can be examined in relation to other societies, which admit plural concepts called "classificatory" kinship. They set up role partition between par
