Barriers to live and deceased kidney donation by patients with chronic neurological diseases: Implications for donor selection, donation timing, logistics, and regulatory compliance.

Live and deceased kidney donation by the numerous patients with advanced, progressive systemic neurological diseases, and other chronic neurological conditions (eg, high C-spine injury) remains largely unexplored. In a review of our current clinical practice, we identified multiple regulatory and clinical barriers. For live donation, mandatory reporting of postdonation donor deaths within 2 years constitutes a strong programmatic disincentive. We propose that the United Network for Organ Sharing should provide explicit regulatory guidance and reassurance for programs wishing to offer live dona

Real-World Selection of Patients for Allogeneic HCT at a Single Centre: Lack of a Suitable Donor and Other Reasons for Not Proceeding.

The reasons why patients cannot proceed with HCT, including cases where no suitable donor is identified, remain poorly described. We reviewed all referrals for allogeneic HCT to our programme between 1 January 2019 and 31 December 2023. Of 880 patients referred for allogeneic HCT, 494 (61.8%) proceeded to transplant (mean 52 ± 14.8 years, 61.5% male) using HLA-matched unrelated (64.2%) or related (19.4%) donors and HLA-mismatched (13%) or haploidentical (3%) donors. Of patients that did not proceed with HCT (386, 38.2%), disease-related causes (54.2%), patient preference (15.8%), and significa

Assessment of Glomerular Filtration Rate and End-Stage Kidney Disease Risk in Living Kidney Donor Candidates: A Paradigm for Evaluation, Selection, and Counseling.

Living donor kidney transplantation is the preferred treatment option for ESRD. However, recent data suggest a small increase in the long-term risk of kidney failure in living kidney donors when compared to healthy nondonors. These data have led to a need for reconsideration of how donor candidates are evaluated and selected for donation. A Kidney Disease: Improving Global Outcomes (KDIGO) work group completed a comprehensive clinical practice guideline for evaluation of living kidney donor candidates in 2017, based on systematic evidence review, de novo evidence generation, and expert opinion

Vibrationally Mediated Dzyaloshinskii-Moriya Interaction as the Origin of Chirality-Induced Spin Selectivity in Donor-Acceptor Molecules.

Chirality-induced spin selectivity (CISS) was recently observed in photoexcited donor-chiral bridge-acceptor molecules, but a predictive theory able to explain available experiments is still lacking. Here, we show that torsional modes modulating hopping and spin-orbit coupling give rise to a Dzyaloshinskii-Moriya interaction between the transferred electron and the one sitting on the donor, producing high spin polarization for realistic parameters. Our model introduces a low-energy scale in the spin dynamics that explains the magnetic field dependence observed in EPR measurements and predicts

Knowledge and Attitude Regarding Blood Donation among First-Time Blood Donor Students of Selected Colleges.

BACKGROUND: Ensuring a continuous supply of safe blood is critical for effective healthcare. Educating students about blood donation can improve the number of voluntary donors. Understanding the knowledge and attitudes of first-time donors is key to fostering a culture of blood donation. MATERIALS AND METHODS: This descriptive survey utilized a structured questionnaire to assess the knowledge and attitudes of 265 students from selected colleges. Reliability tests yielded R-values of 0.96 for knowledge and 0.95 for attitudes. RESULTS: Of the 265 students, 58.9% had poor knowledge, 26% had ave

A Serological Survey of Selected Papua New Guinea Blood Donors for Hepatitis B and Related Co-Infections.

Hepatitis B virus (HBV) infection is a serious problem and earlier studies in Papua New Guinea have reported a high prevalence of hepatitis B virus infection. These studies were undertaken using insensitive tests and before an expanded immunization program. The current HBV status is therefore uncertain. A retrospective study to investigate the HBV status was carried out using blood donor data at Nonga General Hospital, East New Britain Province, Papua New Guinea, from January 2003 to December 2018. Additional data for Human Immunodeficiency Virus, syphilis and hepatitis C virus were also colle

Infusion of Alloanergized Donor Lymphocytes after CD34-selected Haploidentical Myeloablative Hematopoietic Stem Cell Transplantation.

Purpose: Allogeneic hematopoietic stem-cell transplantation (HSCT) is a curative treatment for many hematologic cancers. Use of haploidentical (mismatched) donors increases HSCT availability but is limited by severe graft-versus-host disease (GvHD) and delayed immune reconstitution. Alloanergization of donor T cells is a simple approach to rebuild immunity while limiting GvHD after haploidentical HSCT, but the optimal T-cell dose and impact on immune reconstitution remain unknown.Patients and Methods: We performed a multicenter phase I trial of alloanergized donor lymphocyte infusion (aDLI) af

NO-Donor Iron Nitrosyl Complex with N-Ethylthiourea Ligand Exhibits Selective Toxicity to Glioma A172 Cells.

We studied effects of NO-donor iron nitrosyl complex with N-ethylthiourea ligand (ETM) on normal or tumor-derived cell lines. ETM was mildly toxic to most cell lines studied except the human glioma cell line A172 that proved to be highly sensitive to the complex and underwent cell death after ETM exposure. The high susceptibility of A172 cells to ETM was attributed to its NO-donor properties since no toxicity was detected for the N-ethylthiourea ligand.

Adenoviral infections after transplantation of positive selected stem cells from haploidentical donors in children: an update.

UNLABELLED: We present updated results of stem cell transplantation with highly purified stem cells from haploidentical parental donors and infection with human adenovirus (HAdV) post stem cell transplantation (SCT). Survival post SCT is primarily determined by relapse, infections and far less by GvHD or other transplant related mortality. During the immune reconstitution the host is at significant risk for severe viral infections. HAdV infection is especially in children an important complication post SCT, with significant morbidity and mortality despite new antiviral treatment strategies. Al

UHG crossmatching. A comparison with PCR-SSO typing in the selection of HLA-DPB1-compatible bone marrow donors.

The technique of universal heteroduplex generator (UHG) crossmatching has been developed to permit comparison of HLA-DPB1 alleles between two or more individuals. It offers a rapid and simple method of screening prospective bone marrow donors for HLA-DPB1 compatibility with the recipient. We present the nucleotide sequence and describe the method of construction of the DPB1 UHG. To test its effectiveness, 56 patient-bone marrow donor pairs previously HLA-DPB1-typed by PCR-SSO probing, were tested by UHG crossmatching. In 52/56 (93%) pairs there was concordance between PCR-SSO typing and UHG cr

Identification of antibiotic resistance genes in fecal microbiota selected donors during the establishment of a biobank in the south of Brazil.

Fecal microbiota transplantation (FMT) is an effective therapy for recurrent Clostridioides difficile infection (rCDI) and is increasingly explored for other dysbiosis-related disorders. However, its implementation as a regulated therapeutic strategy requires robust donor screening, biosafety frameworks, and standardized processing workflows. Here, we describe the establishment of the first fecal microbiota biobank in southern Brazil and evaluate the incorporation of metagenomic sequencing as a complementary layer of donor characterization and safety assessment. A structured donor selection pi

Assessing Training Needs of Health Personnel on the Process of Selecting Blood Donors and Transfusion in the West Region of Cameroon.

INTRODUCTION: Blood transfusion is a health care procedure that can expose the donor and receiver to risks. The knowledge and practices of healthcare personnel on blood transfusion are necessary to anticipate and/or prevent the occurrence of avoidable transfusion risks. The aim of this study was to assess the training needs of health personnel on blood donor selection procedure. METHODS: We conducted a cross-sectional study targeting health personnel in health facilities that reported contributing in offering blood transfusion services in the West region of Cameroon between March and May 2022

cis-Selective, Enantiospecific Addition of Donor-Acceptor Cyclopropanes to Activated Alkenes: An Iodination/Michael-Cyclization Cascade.

We present a versatile method for the enantiospecific, cis-diastereoselective intermolecular and intramolecular cycloaddition of donor-acceptor cyclopropanes to electron-poor alkenes with cyclic acceptor groups to afford highly substituted spirocyclopentanes in good to excellent yields. The reaction can be applied to biologically interesting scaffolds, including barbiturates and isoxazolones. Mechanistic investigations were undertaken to explain the unusual diastereoselectivity and enantiospecificity; these suggest an iodination/Michael-cyclization cascade.

Fatty acyl donor selectivity in membrane bound O-acyltransferases and communal cell fate decision-making.

The post-translational modification of proteins with lipid moieties confers spatial and temporal control of protein function by restricting their subcellular distribution or movement in the extracellular milieu. Yet, little is known about the significance of lipid selectivity to the activity of proteins targeted for such modifications. Membrane bound O-acyl transferases (MBOATs) are a superfamily of multipass enzymes that transfer fatty acids on to lipid or protein substrates. Three MBOATs constitute a subfamily with secreted signalling molecules for substrates, the Wnt, Hedgehog (Hh) and Ghre

Long-term results of biopsy-guided selection and allocation of kidneys from older donors in older recipients.

In our old-for-old program, we discard or allocate older extended criteria donor kidneys to single (SKT) or dual kidney transplantation (DKT) depending on histological Remuzzi's score in recipients older than 60 years. Here, we analyze the long-term results of this program and try to identify independent predictors of patient and graft survival. Between December 1996 and January 2008, we performed 115 SKT and 88 DKT. Discard rate was 15%. Acute rejection incidence was higher in SKT than in DKT (22.6% vs. 11.4%, p = 0.04). Renal function was better in DKT than in SKT up to 5 years after transpl

Favorable short-term outcome of transplanted hearts selected from marginal donors by pharmacological stress echocardiography.

BACKGROUND: Because of the shortage of donor hearts, the criteria for acceptance have been considerably expanded. Abnormal results on pharmacologic stress echocardiography are associated with significant coronary artery disease and/or occult cardiomyopathy on verification by cardiac autopsy. The aim of this study was to establish the feasibility of an approach based on pharmacologic stress echocardiography as a gatekeeper for extended heart donor criteria. METHODS: From April 2005 to April 2010, 39 "marginal" candidate donors (mean age, 56 ± 6 years; 21 men) were initially enrolled. After leg

Allogeneic transplantation of positively selected peripheral blood CD34+ progenitor cells from matched related donors.

Hematopoietic progenitor and stem cells are contained within the CD34+ cellular compartment of the bone marrow. Positively selected cytokine primed peripheral blood derived CD34+ cells have been shown to support autologous hematopoiesis after myeloablative therapy. We investigated hematologic reconstitution and incidence of graft-versus-host disease (GVHD) after transplantation of allogeneic peripheral blood CD34+ cells. CD34+ cells were selected from the peripheral blood of 10 matched related donors after treatment with rG-CSF followed by one to four apheresis procedures and biotin-avidin imm

Improved renal allograft survival using the mixed lymphocyte culture for selection of nonidentical living related donors.

Our results concur with earlier published work, by other groups, showing that LRD-recipient pairs with low MLC stimulation usually have better and more prolonged graft success than do those with higher stimulation. Specific HL-A compatibilities or incompatibilities did not seem to affect these results, nor did the presence of an increased number of common loci, short of increasing the apparent chromosome compatibility. The presence of pre-transplant cytotoxic antibodies, in patients with a high MI, however, may unfavorably affect the LRD transplant. The overall results of our LRD transplant ex

Donor and recipient selection leads to good patient and graft outcomes for right lobe split transplantation versus whole graft liver transplantation in adult recipients.

The outcomes of right lobe split (RLS) liver transplantation are variable in adult recipients. This report is an analysis of outcomes of our initial 5-year experience with the right lobe trisegment split graft. A retrospective analysis was performed of the recipient and graft outcomes from July 2002 to March 2007 of all adult recipients of RLS grafts versus recipients of whole grafts (WGs). All data were analyzed with Stata version 8 (Stata Corp., Texas). There were 43 (19.1%) RLS recipients and 182 (80.9%) WG recipients. The median Model for End-Stage Liver Disease score was 13 (7-23) in the

[The Role of Anti-HBc Testing on Donor Selection and Its Contribution to Current Alghorithms; Can the Gülhane Experience Be a Model for Turkey?].

Additional or different test procedures can be applied to maximize transfusion safety by reducing the risk of transfusion-transmitted infections. Antibodies to hepatitis B core antigen (anti-HBc) is one of the most commonly used serologic marker in this context. The aim of the study was to analyse anti-Hbc test results performed within the scope of routine screening tests in our center and to examine the effect of the donor re-entry protocol established. A total of 57191 people who applied to Gülhane Regional Blood Center to donate blood between 2014-2019 were included in the study. All blood
