Availability, cost and affordability of essential medicines for smoking cessation in low-income and middle-income countries: a cross-sectional study.

Smoking cessation is more effective when supported by medicines. Data on the availability, cost and affordability of these treatments in low-income and middle-income countries (LMIC) are limited. Cross-sectional data for smoking cessation medications were collected from pharmacies, healthcare facilities and central medicine stores in 60 LMIC (2022-2023). Medications had varying availability, large price ranges and were essentially unaffordable. Enabling access to these medications is important in reducing tobacco consumption and associated disease. Strategies for integrating smoking cessation

Advancing Federal Coordination to Address Drug Shortages.

Persistent shortages of essential medicines in the United States, especially generic oncology drugs, continue to compromise timely cancer care and patient safety. The presence of multiple high-level reports from federal agencies and industry experts has outlined similar recommendations, including the creation of a unified essential medicines list, transparent supply chain monitoring, domestic manufacturing incentives, and centralized federal coordination, among others, giving an optimistic direction. This manuscript synthesizes key findings from these reports and highlights misalignment across

Shelf-life extension program (SLEP) as a significant contributor to Strategic National Stockpile Maintenance: the Israeli experience with ciprofloxacin.

In the past decade, the 2001 anthrax incident in the U.S. and the 2003 SARS epidemic have highlighted the biological threat to civilian populations. The risk posed by the natural or manmade spread of biological agents among the population dictates a need for better national preparedness. One key component of this preparation is the establishment of a Strategic National Stockpile (SNS) of pharmaceuticals that would provide appropriate medical countermeasures in case of an outbreak. However, to reduce the expense of such a stockpile and to make it worthwhile, there is also a need for a shelf-lif

Environmental factors and chronic unexplained dyspepsia. Association with acetaminophen but not other analgesics, alcohol, coffee, tea, or smoking.

A subgroup of patients with nonulcer dyspepsia (NUD) have no definite cause for their dyspepsia, termed essential dyspepsia. The aim of the present study was to determine if environmental factors are associated with essential dyspepsia. The patterns of ingestion of analgesic drugs (aspirin, acetaminophen, dextropropoxyphene), nonaspirin, nonsteroidal antiinflammatory drugs, alcohol, coffee, tea, and smoking in 113 essential dyspepsia patients were compared with 113 randomly selected community controls matched for age, sex, and social grade. Associations were studied in two six-month periods, b

Drug Provocation Testing in Allergological Work-Up of DRESS: A Retrospective Real-Life Experience From a Specialist Centre.

BACKGROUND: In drug reaction with eosinophilia and systemic symptoms (DRESS), cutaneous tests help to identify the responsible drug. However, uncertainty may persist and drug provocation tests (DPTs) could enable contraindications for essential drugs to be avoided. OBJECTIVES: To report the experience of DPTs in a series of DRESS patients managed in a referral centre for severe drug reactions. METHODS: We performed a single-centre retrospective study (October 2015-October 2021) of patients referred for an allergological work-up after DRESS. RESULTS: Seventy-five patients were included (49 f

Moving beyond a legacy paradigm: A vision for equitable access to quality medicines in Africa.

Ensuring the availability of quality medicines is fundamental to achieving health outcomes and safeguarding public health. However, in many low- and middle-income countries, especially across Africa, the current quality assurance paradigm-largely driven by historical donor preferences for WHO prequalification and stringent regulatory authority approval-can unintentionally limit access, equity, and sustainability. While these requirements have been effective in ensuring client safety, they can result in higher expenditures, reduced coverage, and deepening inequities. Moreover, such narrow focus

Managing medicines in decentralization: discrepancies between national policies and local practices in primary healthcare settings in Indonesia.

In Indonesia, primary health centres (PHCs) are mandated to provide essential medicines to ensure equal access to medication for all Indonesians, as stated in the national medicine policy. However, limited information is available regarding the actual practices of health workers within the context of decentralized governance. This paper investigates the discrepancies between national policies and local practices in two Indonesian districts, shedding light on coping mechanisms employed in each phase of medicine management within PHCs. The mixed-method study began by identifying pertinent polici

Low- And Middle-Income Countries Experienced Delays Accessing New Essential Medicines, 1982-2024.

Little is known about how long it takes for new medicines to reach countries with different income levels. We analyzed data, sourced from IQVIA, on the timing of new drug launches in seventy-five low-, middle-, and high-income markets from 1982 to 2024. The sample captured the majority of essential medicines (as designated by the World Health Organization in the twenty-third Model List of Essential Medicines) that first came into medical use anywhere globally from 1982 onward. Kaplan-Meier estimates were used to quantify delays in launches across countries. Our analysis comprised 119 medicines

Inclusion of abortion-related care in national health benefit packages: results from a WHO global survey.

INTRODUCTION: Service inclusion in a country's health benefit package (HBP) is an important milestone towards universal health coverage. This study aimed to explore HBP inclusion of abortion interventions globally. METHODS: Secondary analysis of the WHO HBP survey, in which officially nominated survey focal points were asked which interventions were included within the HBP of their country or area's largest government health financing scheme. Abortion inclusion was compared by region, income, legal status of abortion and HBP design process variables. Abortion inclusion was compared with other

Translational Pharmacology of PRAX-944, a Novel T-Type Calcium Channel Blocker in Development for the Treatment of Essential Tremor.

BACKGROUND: Essential tremor is the most common movement disorder with clear unmet need. Mounting evidence indicates tremor is caused by increased neuronal burst firing and oscillations in cerebello-thalamo-cortical circuitry and may be dependent on T-type calcium channel activity. T-type calcium channels regulate sigma band electroencephalogram (EEG) power during non-rapid eye movement sleep, representing a potential biomarker of channel activity. PRAX-944 is a novel T-type calcium channel blocker in development for essential tremor. OBJECTIVES: Using a rat tremor model and sigma-band EEG po

Leishmanicidal activity of carvacrol-rich essential oil from Lippia sidoides Cham.

Leishamaniasis is a disease that affects more than 2 million people worldwide, whose causative agent is Leishmania spp. The current therapy for leishmaniasis is far from satisfactory. All available drugs, including pentavalent antimony, require parenteral administration and are potentially toxic. Moreover, an increase in clinical resistance to these drugs has been reported. In this scenario, plant essential oils used traditionally in folk medicine are emerging as alternative sources for chemotherapeutic compounds. In this study, in vitro leishmanicidal effects of a thymol- and a carvacrol-rich

Territorial inequities in access to essential medicines for neglected tropical diseases in a decentralised European health system: a nationwide assessment in Spain.

BACKGROUND: Neglected tropical diseases (NTDs) represent an increasing but often overlooked challenge for European health systems owing to international mobility, migration, travel, and climate-related changes in disease ecology. Although effective treatments exist for most NTDs and many are included in the World Health Organization (WHO) Model List of Essential Medicines, their availability in decentralised high-income health systems remains poorly characterised. This study assessed the availability, accessibility, and territorial variability of essential medicines for NTDs across the autonom

Parkinson disease treatments on national essential medicines lists, African Region.

The prevalence of Parkinson disease is increasing globally. Despite the existence of effective and affordable medicines for Parkinson disease, access to these medicines is limited in the World Health Organization (WHO) African Region. Several factors influence accessibility, including lack of prioritization of Parkinson disease, shortage of a trained health workforce, barriers to health financing and lack of inclusion of medicines in national essential medicines lists. We determined alignment of the most recent national essential medicines lists of 47 countries in the WHO African Region with t
