Spectral analysis of Doppler signal in the prophylaxis of ischemic cerebrovascular accidents in extra- and/or intracranial carotid and vertebral systems.

Using continuous-wave spectral analysis of Doppler signal (SADS) and/or pulsed-wave transcranial Doppler ultrasonography (TCD-US) we examined 263 patients with ischemic cerebrovascular diseases (ICVD) symptoms in the carotid and vertebrobasilar (V-B) systems. The degree of stenosis was established by taking into consideration the parameters of the spectrum of frequency (systolic peak velocity--SV; end-diastolic velocity--DV); the ratio between SV of internal carotid artery (ICA) and common carotid artery (CCA); the ratio between DV of ICA and CCA as well as the value of the spectral broadening

Risk factors associated with the development of peri-intraventricular haemorrhage and periventricular leukomalacia.

81 preterm infants of 34 weeks' gestation or less were prospectively and sequentially examined by means of real-time ultrasound in order to identify which clinical risk factors might be associated with the development of peri-intraventricular haemorrhage (PVH) and periventricular leukomalacia (PVL). Infants were allocated in three groups: group A (n = 44): with normal scans; group B (n = 24): with isolated PVH, and group C (n = 13): with PVL. 28 obstetrical and neonatal factors were compared within the three groups using two methods of statistical analysis (2 x 2 chi 2 analysis and multivariat

[Results of ultrasound brain studies in clinically normal newborn infants].

Ultrasound investigations demonstrating intracranial hemorrhages (periventricular intraventricular hemorrhage-PVH/IHV) were performed in 401 term newborns. Without consideration the mode of delivery incidence of intracranial hemorrhage was 4.7%. There were only subependymal hemorrhages grade 1 and 2. In case of vaginal-operative delivered children frequency of hemorrhage in forceps delivered babies was really lower than in vacuum extracted newborns.

[Ultrasonic resection of brain tumors].

With the CUSA Ultrasonic Aspirator even consistent tumors can be removed with a minimum of tissue traction and almost entirely sparing the adjacent neural and vascular structures. The hand guided titanium tip oscillates at 23 kc and disintegrates the pathological tissue aspirating it continuously through a central channel. Particularly in neurosurgery, where in many instances resection has to be carried out transtumorally this instrument has proved to be of great help. Three cases of large brain tumors are presented in which the CUSA technique allowed an easy and rapid removal of the neoformat

Ultrasound control of progressive hydrocephalus in infancy.

The authors conducted multidimensional echotomography on 119 infants to determine hydrocephalus and other disorders. The process proved quite reliable in showing changes in ventricle size. They raise the idea of conducting broad scale preventive screening among infants in danger groups to achieve early diagnosis.

[Echograpic diagnosis of a rare malformation: iniencephaly (author's transl)].

We describe a case of iniencephaly, which is a maldevelopment rare enough to warrant the publication of known cases. The ultrasound pictures that are taken and presented show a spina bifida and an omphalocoele were associated with the cerebral abnormality. It is further confirmation of the great value of routine ultrasound making early diagnosis of fetal malformations.

Intraoperative 3D contrast-enhanced ultrasound (CEUS): a prospective study of 50 patients with brain tumours.

BACKGROUND: Reliable intraoperative resection control during surgery of malignant brain tumours is associated with the longer overall survival of patients. B-mode ultrasound (BUS) is a familiar intraoperative imaging application in neurosurgical procedures and supplies excellent image quality. However, due to resection-induced artefacts, its ability to distinguish between tumour borders, oedema, surrounding tissue and tumour remnants is sometimes limited. In experienced hands, this "bright rim effect" could be reduced. However, it should be determined, if contrast-enhanced ultrasound can impro

Elevation of serum S100 protein concentration as a marker of ischemic brain damage in extremely preterm infants.

BACKGROUND: Periventricular leukomalacia (PVL) is serious ischemic brain damage that occurs in extreme preterm infants. It is traditionally diagnosed by cranial echography. The purpose of this study was to investigate the relationship between serum S100 calcium-binding protein B (S100B) concentrations and ischemic brain damage, and to find the cutoff value for the early identification of ischemic brain damage in high-risk preterm infants. METHODS: At the age of 3 days, 7 days, 14 days, and 21 days, and before discharge, 22 extremely premature infants (i.e., gestational age <33 weeks) underwen

Interobserver variability in assessment of cranial ultrasound in very preterm infants.

BACKGROUND: Cranial ultrasound (cUS) findings help doctors in the clinical management of preterm infants and in their discussion with parents regarding prediction of outcome. cUS is often used as outcome measure in clinical research studies. Accurate cUS performance and interpretation is therefore required. AIMS: The aims of this study were (i) to assess the interobserver variability in cUS interpretation, and (ii) to evaluate whether level of cUS expertise influences the interobserver variability. METHODS: Fifty-eight cUS image series of preterm infants born below 32 weeks of gestation coll

[Classification of intracranial hemorrhage in premature infants].

The most common classification of intracranial haemorrhage in premature infants into four degrees of severity is based on the results of CT-scans. However, this classification does not adequately account for some pathophysiological and morphological changes. For this reason, the paediatric section of DEGUM developed a new method of classification. This classification distinguishes more precisely between the bleeding itself and secondary changes, such as posthaemorrhagic ventricular dilation, which were excluded from the revised classification. The new system contains three levels: Grade I: sub

[Brain computerized tomographic and ultrasonographic findings in patients with asymptomatic carotid bruits].

This study was conducted to clarify brain and carotid lesions in patients with asymptomatic carotid bruits and their characteristics. We studied 37 patients with carotid bruits, who had various diseases other than stroke and were all neurologically normal, using by brain computerized tomography (CT) and ultrasonography (US). On CT, localized low density areas (LDAs) and their distribution were assessed, as well as the grade of periventricular lucency (PVL). Carotid lesions on US were classified into 3 categories: plaque (locally thickened intima-media complex of 2.1 mm or more in thickness), s
