Spatiotemporal Analysis of Hospitalisations and Mortality From Gynaecological Cancers in Ecuador, 2015-2024.

PURPOSE: To analyse the spatiotemporal patterns of hospitalisations and mortality due to gynaecological cancers in Ecuador (2015-2024), identify provincial geographic disparities, and examine the association between healthcare infrastructure and clustering patterns. METHODS: This population-based ecological study used INEC hospital discharge and general death records for women with gynaecological cancers (ICD-10: C51-C57) in Ecuador from 2015 to 2024. Age-standardised rates per 100,000 women were calculated by year, cause, age group, and province of residence. K-means clustering was applied s

Serologic Assessment of Public Health Action Needed for Trachoma, Coastal Ecuador, 2021-2024.

We evaluated the probability of needing public health actions against trachoma in Esmeraldas Province, Ecuador, by estimating seroconversion rates of Chlamydia trachomatis plasma gene protein 3 IgG in children. Seroconversion rates suggest high probability of the need for public health interventions in rural communities and motivate further assessment.

Testing the Dry Refuge Model: Paleoecological Insights From Late Pleistocene Gomphotheres in Ecuador.

Paleoenvironmental records indicate that Neotropical regions near the Equator experienced reduced rainfall during the Last Glacial Maximum (LGM). However, some records show conflicting environmental signals. To address these discrepancies, previous studies have proposed two demographic/genetic scenarios for cloud forests based on contrasting precipitation regimes. Here, we apply a multiproxy approach to extinct proboscideans, commonly referred to as gomphotheres, from Ecuador to assess the extent to which wet- and dry-forest models shaped environmental conditions on the coast and in the Inter-

Robot-assisted extended thymectomy in juvenile myasthenia gravis: first case performed in Ecuador.

Myasthenia gravis (MG) is a chronic autoimmune disorder characterized by fatigable muscle weakness, typically caused by antibodies against acetylcholine receptors (AChR) or muscle-specific kinas. While it primarily affects adults, juvenile cases represent a significant minority and often pose greater therapeutic challenges. Extended thymectomy has proven beneficial in AChR-positive MG, reducing symptoms and immunosuppressive needs. Robotic-assisted thymectomy offers enhanced precision with lower morbidity. We report the first such procedure in Ecuador, performed on a 21-year-old woman with rec

Pediatric pneumonia across altitudes in Ecuador: a countrywide, epidemiological analysis from 2010-2021.

BACKGROUND: In Ecuador, pneumonia is a significant pediatric health challenge and a leading cause of infant mortality. Little is known about the contribution of altitude to trends in pneumonia incidence and mortality. The objective of this study was to examine the how the disease burden of pneumonia varies across substantial changes in altitude in a large population over time. METHODS: A nationwide descriptive ecological analysis of the burden of pediatric pneumonia was conducted using secondary data from hospital discharges spanning 2010 to 2021. Patients aged 0 to 18 years with ICD-10 diagn

Dyggve-Melchior-Clausen Syndrome in Ecuador: Expanding Knowledge on a Rare Genetic Disorder.

Background: Dyggve-Melchior-Clausen syndrome (DMC) is a rare autosomal recessive skeletal dysplasia characterized by dwarfism, coarse facial features, and intellectual disability. Caused by loss-of-function variants in the DYM gene, which encodes dymeclin, DMC is predominantly reported in consanguineous populations but remains poorly studied in South America. Methods: We report a 21-year-old Ecuadorian male with clinical features suggestive of DMC. Comprehensive clinical, radiological, and genetic evaluations were conducted, including clinical exome sequencing and Sanger sequencing, followed b

Spatial and phylogenetic insights into bovine rabies virus in Ecuador: Regional control implications.

Bovine rabies caused by the rabies virus (RABV) represents a significant veterinary and economic problem in Ecuador, particularly in the Amazonian province. In this study, the transmission dynamics and genetic diversity of the virus are explored through phylogenetic and spatial analysis of Ecuadorian isolates from cattle, contextualized with sequences from neighboring countries. Cerebellum and brain samples were collected from 45 virus-infected farms across Ecuador between 2015 and 2020. Samples were obtained post-mortem from symptomatic animals and immediately processed for virus isolation. T

An intersectionality perspective on tuberculosis: social determinants affecting tuberculosis mortality rate in Ecuador.

INTRODUCTION: The Intersectionality approach to studying tuberculosis (TB) is a complex one. While historical data and indicators consistently suggest a positive correlation between social determinants, such as poverty, and TB mortality, the strength of this association varies across different regions and countries. Overcrowding and population density are also recognized as risk factors for TB transmission. METHODS: In this study, we conducted a descriptive and observational statistical analysis of TB mortality in Ecuador using the most recent public data from 2010. We examined the associatio

Molecular epidemiology of continued Plasmodium falciparum disease transmission after an outbreak in Ecuador.

To better understand the factors underlying the continued incidence of clinical episodes of falciparum malaria in E-2025 countries targeting elimination, we characterized the molecular epidemiology of Plasmodium falciparum disease transmission after a clonal outbreak in Ecuador. Here we study disease transmission by documenting the diversity and population structure of the major variant surface antigen of the blood stages of P. falciparum encoded by the var multigene family. We used a high-resolution genotyping method, "varcoding", involving targeted amplicon sequencing to fingerprint the DBLα

Three new endemic species of Lepanthes (Orchidaceae, Pleurothallidinae) from the highlands of Ecuador.

Three new species of Lepanthes from Ecuador are described and illustrated. These additions to the Ecuadorean flora were recorded in evergreen montane forest and páramo as part of three different research projects conducted during the last five years (2016-2021). Lepanthesoro-lojaensis was discovered in the southwest of El Oro province and is similar to L.jimburae, differing mainly in the much smaller plants, inflorescences and floral parts. Lepanthesmicroprosartima from the western slopes of Pichincha volcano in northern Ecuador resembles L.obandoi but differs in the coloration of the leaves,

Genomic Epidemiology of Salmonella Infantis in Ecuador: From Poultry Farms to Human Infections.

Salmonella enterica is one of the most important foodborne pathogens around the world. In the last years, S. enterica serovar Infantis has become an important emerging pathogen in many countries, often as multidrug resistant clones. To understand the importance of S. enterica in the broiler industry in Ecuador, we performed a study based on phenotypic and WGS data of isolates from poultry farms, chicken carcasses and humans. We showed a high prevalence of S. enterica in poultry farms (41.4%) and chicken carcasses (55.5%), but a low prevalence (1.98%) in human samples. S. Infantis was shown to

Soil-transmitted helminth infection and intestinal inflammation among the Shuar of Amazonian Ecuador.

OBJECTIVES: Little research exists documenting levels of intestinal inflammation among indigenous populations where exposure to macroparasites, like soil-transmitted helminths (STHs), is common. Reduced STH exposure is hypothesized to contribute to increased prevalence of elevated intestinal inflammation in wealthy nations, likely due to coevolutionary histories between STHs and human immune systems that favored anti-inflammatory pathways. Here, we document levels of intestinal inflammation and test associations with STH infection among the Shuar of Ecuador, an indigenous population undergoing

Excessive daytime somnolence and cardiovascular health: A population-based study in rural Ecuador.

In a population-based study conducted in rural Ecuador, 635 stroke-free persons aged ≥40 years were interviewed with the Epworth sleepiness scale and screened to assess their cardiovascular health (CVH) status. Excessive daytime somnolence was present in 22% persons and a poor CVH status in 69%. In a generalized linear model after adjusting for age and sex, excessive daytime somnolence was not associated with a poor CVH status or with any of the individual metrics in the poor range. Excessive daytime somnolence may not be linked to cardiovascular risk factors at the rural level.

Leishmaniasis recidiva cutis due to Leishmania (Viannia) panamensis in subtropical Ecuador: isoenzymatic characterization.

BACKGROUND: Information regarding leishmaniasis recidiva cutis (LRC), a clinical variant of cutaneous leishmaniasis, in the New World is scarce. LRC is characterized by slowly progressing lesion(s) that appear after a variable period of time, from months to years, in or around the scar of an apparently clinically healed sore. PATIENTS AND METHODS: Six patients are reported who presented with crusted, papular lesions located on the edge of a healed scar, with a mean of 18.2 months of slowly progressive evolution. The isolated strains of Leishmania parasites were characterized by enzyme electro

Mapping territorial labour-related well-being inequalities in Ecuador: A multivariate geometric approach.

This article examines the territorial structure and post-pandemic evolution of labour-related well-being inequalities across Ecuador's 24 provinces between 2021 and 2024. Using official provincial data from the National Employment, Unemployment and Underemployment Survey (ENEMDU), we constructed an integrated set of eleven indicators capturing employment quantity, job quality, labour income, monetary and multidimensional poverty, and youth labour market exclusion (NEET population aged 15-24). Methodologically, the study adopts a geometric multivariate approach that combines annual HJ-biplots w

Integrated serosurveillance to assess disease elimination in coastal Ecuador: onchocerciasis, yaws, and trachoma.

BACKGROUND: Testing blood samples with multiplex bead assays can assess elimination and identify residual foci of transmission for multiple pathogens simultaneously. In Ecuador, onchocerciasis and yaws are presumed eliminated, and the status of trachoma is unknown. We assessed their elimination status by measuring IgG antibodies in children 6-24 months. METHODS: In a birth cohort of 404 children measured between 2021-2024 in Esmeraldas province, Ecuador, we tested dried blood spots at ages 6, 9, 12, 18, and 24 months using a multiplex IgG assay that included antigens to Onchocerca volvulus (O
