Primary total elbow arthroplasty for distal humeral fractures in elderly patients: a nationwide analysis.

BACKGROUND: Displaced intra-articular distal humeral fractures are a challenging injury in elderly patients. High rates of complications have led to the increasing use of total elbow arthroplasty (TEA) for primary treatment. This study presents US nationwide trends in primary TEA for distal humeral fractures in elderly patients (65 years and older) from 2002 to 2012. We hypothesized that there was an increase in the rate of TEA utilization. METHODS: Data were obtained from the Nationwide Inpatient Sample for the years 2002 to 2012. All inpatients 65 years and older with distal humeral fractur

Radial head fractures associated with elbow dislocations treated by immediate stabilization and early motion.

Twenty-one elbow dislocations with an associated radial head fracture were treated with immediate joint reduction, stabilization, and early range-of-motion exercises. In all cases initial treatment involved closed reduction of the ulnohumeral joint. For those cases involving minimally displaced and a few moderately displaced radial head fractures, treatment consisted of benign neglect (4 of 21). Of the more severely displaced fractures (17 of 21), 9 were treated with open-reduction internal fixation and 8 with immediate silicone head replacement. Despite radial head treatment, 6 of these cases

Outcome after surgical treatment of complex elbow fractures: a single-center follow-up study.

INTRODUCTION: Elbow injuries present particular challenges for the treating surgeons. The aim of the present study was to evaluate the postoperative outcome after surgical treatment of complex elbow fractures. PATIENTS AND METHODS: Retrospective analysis with subsequent clinical follow-up examination of all patients with complex elbow fractures admitted to our level I trauma center over a 6-year period. The patients were stratified into three groups: single fractures of the proximal forearm (olecranon fractures type Mayo IIB, radial head fractures type Mason II-IV), distal humerus fractures (

Outcomes of Nonoperative Pediatric Medial Humeral Epicondyle Fractures With and Without Associated Elbow Dislocation.

BACKGROUND: Medial epicondyle fractures may occur in isolation or with associated elbow dislocation. In the absence of open fracture or fragment incarceration, nonoperative management with immobilization has been shown to result in generally successfully outcomes comparable with those reported after surgical fixation. However, no comparative investigation has assessed outcomes after nonoperative treatment based on the presence or absence of elbow dislocation. METHODS: A systematic review was conducted investigating all studies in the literature reporting nonoperative outcomes for isolated med

[Nerve injuries after elbow luxation fractures in childhood : Indication and timing for surgical revision].

BACKGROUND: Supracondylar humerus fractures are very common in pediatric populations. In cases of dislocated fractures, closed reduction and percutaneous K‑wire fixation is recommended. Initially, 10-16 % of the patients also present lesion of the ulnar nerve as well as median nerve palsy in 6.4% and radial nerve palsy in 2.6 % of cases, respectively. METHOD: We present the case of a 10-year-old boy with a dislocation of the elbow and fracture of the medial epicondyle. After closed reduction and K‑wire fixation from medial, he presents a median nerve palsy. That a lesion of the ulnar nerve is

Prophylaxis against heterotopic ossification after elbow and acetabular fractures - Do we really need it.

OBJECTIVE: To compare the efficacy and safety of prophylactic modalities for heterotopic ossification prevention after elbow and acetabular surgeries. METHODS: The retrospective chart review was conducted at the Aga Khan University Hospital and comprised record of patients who underwent open reduction and internal fixation for elbow and acetabular fractures between 2010 and 2013. Data was classified into three groups: Group A patients had received single dose of radiotherapy; Group B patients had received indomethacin, and Group C patients had not received any prophylaxis. Outcome variables i

Surgical management of elbow dislocation associated with non-reparable fractures of the radial head.

BACKGROUND: The "terrible triad" of the elbow is the combination of an elbow dislocation, radial head and a coronoid process fracture. Because of a combined sagittal, frontal and transverse instability, these injuries are notoriously difficult to treat. We report our results with a technique for reconstruction of "terrible triad" injuries with either no facture or a type I fracture of the coronoid process in addition to a non-reparable radial head fracture. The hypothesis of this study was that standard surgical treatment of this lesion using a "deep to superficial" stabilisation by a single l

Pediatric lateral condyle humeral fractures with and without associated elbow dislocations: a retrospective study.

In the pediatric population, lateral condyle fractures are relatively common elbow injuries, but not nearly as common are traumatic elbow dislocations, and these 2 types of injuries in combination are even less common. Our literature search showed only 2 reports on these concomitant injuries. In the study reported here, we evaluated a consecutive series of pediatric patients with lateral humeral condyle fractures with and without elbow dislocation and compared the groups' results.

Total elbow replacement for distal humerus fractures and traumatic deformity: results and complications of semiconstrained implants and design rationale for the Discovery Elbow System.

Recent advances in elbow replacement design have further extended its usefulness as an expedient reconstructive method for restoring elbow function following trauma. This is particularly true in elderly patients, for whom treatment is complicated by the frequent presence of osteoporosis, severe comminution with small condylar fracture fragments, and diminished bone-healing capacity. This article addresses the role of elbow replacement for fracture, traumatic deformity, post-traumatic arthritis, and instability. Indications, techniques, and results are presented. Complications of semiconstraine

Acute traumatic posterior fracture dislocation of the elbow in pediatric patients: impact of surgery time and associated fractures on outcome.

This study assessed the effect of the time interval from initial injury to surgery and the presence of associated fracture on functional outcomes after acute posterior elbow fracture dislocation. Twenty-six pediatric patients were evaluated with respect to operation time point (within 24 h vs. later) and associated fracture retrospectively. The Mayo Elbow Performance Index (MEPI) score was used to assess functional results. The MEPI score was 91 (80-100) in patients with one associated fracture and 83 (75-95) (P=0.02) in patients with more than one associated fracture. The MEPI score in patien

Assessing deep learning artificial intelligence support for detecting elbow fractures in the pediatric emergency department.

OBJECTIVES: Interpreting elbow radiographs in pediatric trauma cases is challenging for emergency clinicians due to anatomical peculiarities of this joint in children and the risk of complications from missed fractures. Artificial intelligence (AI), particularly deep learning algorithms, has the potential to assist in fracture detection. We aimed to assess the performance of pediatric emergency clinicians in detecting elbow fractures in children without and with the assistance of a deep learning algorithm. PATIENTS AND METHODS: This retrospective study included all children aged 0-15 years ad

Risk factors for elbow stiffness after surgery for AO / OTA type C distal humerus fractures.

OBJECTIVE: The aim of this study is to identify risk factors for elbow stiffness following surgery for AO/OTA type C distal humerus fractures with a follow-up investigation. METHODS: Data were collected from patients who underwent treatment for AO/OTA type C distal humerus fractures between March 2015 and March 2022.The patients were divided into a stiffness group and a control group based on whether their elbow flexion-extension or rotation range of motion was less than 100°at the final follow-up. Univariate analysis and multivariate logistic regression analysis was performed to identify ind

Surgical Management of Radial Head and Proximal Ulna Fractures Without Elbow Dislocation: A Case Report.

Fractures involving the proximal ulna and radial head are common injuries that occur in the upper extremity, often resulting from traumatic incidents such as falls or direct impact. The proximal ulna forms the elbow joint with the humerus, while the radial head articulates with both the humerus and the ulna, facilitating forearm rotation. Fractures in these areas can disrupt the stability and function of the elbow joint, leading to pain, swelling, and limited range of motion. Clinically, it is more common to observe a radial head dislocation with a proximal ulna fracture. This fracture is refe

Mid-term results of total elbow arthroplasties in the treatment of geriatric distal humerus fractures.

BACKGROUND: While double plate fixation is the gold standard treatment for distal humerus fractures in the general population, it is the source of many complications in the elderly. Total elbow arthroplasty (TEA) has been proposed as an alternative treatment, with satisfactory short-term functional outcomes. However, little is known about the longevity of the implant and the mid- and long-term complications of this procedure. MATERIALS AND METHODS: A total of 58 TEAs were performed in 57 patients with comminuted distal humerus fractures between September 2008 and September 2019. All patients
